天津张××，年二十六岁，得肺病咳嗽吐血。
病因 经商劳心，又兼新婚，失于调摄，遂患劳嗽。继延推拿者为
推拿两日，咳嗽分毫未减，转添吐血之证。
证候 连声咳嗽不已，即继以吐血。或痰中带血，或纯血无痰，或有咳嗽兼喘。夜不能卧，心
中发热，懒食，大便干燥，小便赤涩。脉搏五至强，其左部弦而无力，右部浮取似有力，
而尺部重按豁然。
处方 生怀山药（一两） 大潞参（三钱） 生赭石（六钱轧细） 生怀地黄（六钱）
玄参（六钱） 天冬（五钱） 净萸肉（五钱） 生杭芍（四钱） 射干（二钱） 甘草（二钱）
广三七（二钱轧细）
药共十一味，将前十味煎汤一大盅，送服三七末一半，至煎渣重服时，再送服其余一半。
复诊 此药服两剂后，血已不吐，又服两剂，咳嗽亦大见愈，大小便已顺利，脉已有根，不若
从前之浮弦。遂即原方略为加减，俾再服之。
处方 生怀山药（一两） 大潞参（三钱） 生赭石（六钱轧细） 生怀地黄（六钱）
大甘枸杞（六钱） 甘草（二钱） 净萸肉（五钱） 沙参（五钱）
生杭芍（二钱） 射干（二钱） 广三七（钱半轧细）
药共十一味，将前十味煎汤一大盅，送服三七末一半，至煎渣重服时，再送其余一半。
效果 将药连服五剂，诸病皆愈，脉已复常，而尺部重按仍欠实。遂于方中加熟怀地黄五钱，
俾再服数剂以善其后。
病因 因经商劳心费神，加之新婚期间缺乏调养，于是患上了虚劳咳嗽。随后请推拿医生进行了两天的推拿治疗，咳嗽丝毫未减轻，反而增加了吐血的病症。
证候 连续不断剧烈咳嗽，紧接着便吐血。有时痰中带血，有时纯血无痰，有时咳嗽兼有气喘。夜间无法安卧，心中感觉发热，食欲不振，大便干燥，小便短赤涩痛。脉搏跳动频率为五至以上（稍快），左侧脉象弦细而无力，右侧脉象轻取似乎有力，但重按尺部时豁然空虚。
处方 生怀山药（一两） 大潞参（三钱） 生赭石（六钱，轧成细粉） 生怀地黄（六钱） 玄参（六钱） 天冬（五钱） 净萸肉（五钱） 生杭芍（四钱） 射干（二钱） 甘草（二钱） 广三七（二钱，轧成细粉）
药共十一味，将前十味药材煎煮成一盅汤剂，送服一半的三七粉末；等到煎煮药渣再次服用时，再送服剩余的一半三七粉末。
复诊 此方服用两剂后，吐血停止；又服用两剂，咳嗽也大大好转，大小便已通畅，脉象已有根基，不再像从前那样浮弦。于是就在原方基础上略微加减，让他继续服用。
处方 生怀山药（一两） 大潞参（三钱） 生赭石（六钱，轧成细粉） 生怀地黄（六钱） 大甘枸杞（六钱） 甘草（二钱） 净萸肉（五钱） 沙参（五钱） 生杭芍（二钱） 射干（二钱） 广三七（一钱半，轧成细粉）

效果 连续服药五剂后，所有病症痊愈，脉象恢复正常，但重按尺部仍然不够充实。于是在方中加入熟怀地黄五钱，让他再服用几剂以巩固疗效，善后调理。
Patient Zhang XX from Tianjin, aged twenty-six, suffered from lung disease manifesting as cough and hemoptysis.
Etiology Due to mental exertion in business and lack of proper regulation following a recent marriage, he developed consumptive cough (Lao Sou). Subsequently, two days of massage therapy were administered; however, the cough remained unchanged, and hemoptysis was added to the symptoms.
Clinical Manifestations Continuous, unremitting severe cough followed immediately by hemoptysis. Sometimes blood was mixed with sputum, sometimes pure blood without sputum, and sometimes cough accompanied by wheezing. Inability to lie down at night, sensation of heat in the heart region, poor appetite, dry stool, and scanty, dark, and painful urination. The pulse rate was slightly faster than five beats per breath cycle. The left radial pulse was wiry and weak; the right radial pulse seemed forceful on light palpation but felt hollow upon heavy pressure at the Chi position.
Prescription Raw Huai Yam (Sheng Huai Shan Yao) 1 liang, Large Lu Shen (Da Lu Shen) 3 qian, Raw Hematite (Sheng Zhe Shi) 6 qian, finely ground, Raw Huai Rehmannia Root (Sheng Huai Di Huang) 6 qian, Scrophularia Root (Xuan Shen) 6 qian, Asparagus Tuber (Tian Dong) 5 qian, Clean Cornus Fruit (Jing Yu Rou) 5 qian, Raw Hang Peony Root (Sheng Hang Shao) 4 qian, Belamcanda Rhizome (She Gan) 2 qian, Licorice Root (Gan Cao) 2 qian, Guang Sanqi 2 qian, finely ground.
The prescription consists of eleven ingredients. Decoct the first ten ingredients to make one large cup of decoction, taken with half of the Sanqi powder. When the dregs are decocted for the second dose, take the remaining half of the Sanqi powder.
Follow-up Visit After taking two doses of this prescription, hemoptysis ceased. After two more doses, the cough improved significantly, bowel and bladder movements became smooth, and the pulse gained root, no longer floating and wiry as before. Therefore, slight modifications were made to the original formula for continued use.

Outcome After continuous administration of five doses, all ailments resolved, and the pulse returned to normal, although heavy pressure at the Chi position still lacked solidity. Therefore, Cooked Huai Rehmannia Root (Shu Huai Di Huang) 5 qian was added to the formula, and several more doses were prescribed for consolidation and aftercare.
