To treat dysentery that has persisted for a long time, where stagnant heat generates toxins, causing putrefaction in the intestines. Symptoms include frequent severe colic, tenesmus (a feeling of incomplete defecation), and stools resembling burnt flesh, accompanied by a foul, putrid odor.
Jinyinhua (Lonicerae Japonicae Flos) (1 liang) Sheng Hangshao (Paeoniae Radix Alba) (6 qian) Fen Gancao (Glycyrrhizae Radix et Rhizoma Preparatum cum Melle) (3 qian) Sanqi (Notoginseng Radix et Rhizoma) (2 qian, finely ground) Yadanzi (Strychni Semen) (60 grains, peeled and selected for plumpness).
Take the above five herbs. First, administer Sanqi and Yadanzi with water sweetened by Baishatang (white rock sugar). Then, decoct the remaining herbs and take the soup orally. For severe cases, two doses per day are required to see efficacy.
This syndrome represents the most severe form of dysentery. If treated at the onset when Qi and Blood are not yet depleted, using my formulated Huazhi Tang (Stasis-Transforming Decoction), or adding Dahuang (Rhei Radix et Rhizoma) and Puxiao (Natrii Sulfas) to purge downward will lead to recovery. If not fully cured, continuing with Xieli Tang (Harmonizing Decoction) for several doses can also result in complete recovery. However, if treatment is delayed and the condition persists for a long time, leading to depletion of both Qi and Blood, progressing to intestinal putrefaction and diminishing vitality, resulting in stools that are both discolored and odorous due to putrefaction, the previous two formulas will no longer be effective. This formula focuses on resolving putrefaction and generating tissue to save the rotting intestines; thus, it achieves remarkable effects.
A fifty-two-year-old man suffered from bloody stools after a fit of anger caused internal stagnant heat. Sleeping in a cold room, this internal heat was constrained by external cold, becoming more stagnant and unable to dissipate. He sought medical treatment. The physician, attributing the illness to the cold environment, diagnosed it as Spleen Cold sinking downward and prescribed warming and tonifying herbs such as Renshen (Ginseng Radix et Rhizoma), along with Shengma (Cimicifugae Rhizoma) to lift Yang. After two doses, the condition worsened: severe abdominal pain, frequent tenesmus, and stools resembling burnt flesh. Another physician was consulted, who diagnosed Kidney-Yang deficiency and Cold in the lower burner, prescribing Bawei Dihuang Wan (Eight-Ingredient Pill with Rehmannia) as a decoction. The condition aggravated further. Upon my examination, his pulse was rapid and forceful, especially pronounced at the Chi position (cun-kou pulse location corresponding to the kidneys/lower jiao). I confirmed that toxic heat was stagnant in the intestines, causing putrefaction. I prescribed this formula, and he recovered after two doses.
A woman, approximately fifty years old, had a habit of smoking opium. Following a period of anger, she initially developed red dysentery with uncontrolled frequency. Due to improper treatment, her blood gradually became putrid, with stools intermittently resembling burnt flesh. She experienced nausea, loss of appetite, and severe pain in the lower abdomen. Her pulse was surging and rapid, indicating pure Heat syndromes. This case was also treated with the aforementioned formula, modified by adding Zhimu (Anemarrhenae Rhizoma) and Baitouweng (Pulsatillae Radix), each 4 qian, decocted and taken orally. Additionally, 60 grains of Yadanzi and 2 qian of Sanqi were administered with water. Taking this medication twice daily resulted in full recovery within three days.
