To treat the aforementioned condition, after taking the medicine, the external pathogenic fire has dissipated, yet thirst and diarrhea remain unresolved. This may be due to the damage of the Qi aspect (the functional activities of the body) caused by the ingestion of Qi-breaking medicines (herbs that break up stagnation and consume Qi), resulting in unceasing slippery diarrhea (involuntary loose stools). If the patient additionally presents with panting and rebellious Qi, or cough, or spontaneous sweating, or palpitations, this decoction should be administered urgently.
Raw Dioscoreae Rhizoma (Shanyao) [75g]
Prepared Rehmanniae Radix (Shudi) [75g]
Wild Taishen Ginseng (Taishen) [24g]
Talcum (Huashi) [15g]
Raw Paeoniae Radix Alba (Baishao) [15g]
Licorice Root (Gancao) [6g]
Pomegranate (Suan Shiliu) [1 whole, crushed with peel]
The above seven ingredients are boiled in 1000ml of water. First, boil the pomegranate for about ten boiling cycles, remove the dregs, then add the remaining herbs. Decoct until 400ml remains, divide into two doses, and take warm. If pomegranate is unavailable, Oyster Shell (Muli) [30g, powdered] can be used as a substitute. For excessive sweating, add Cornus Fruit (Shanzhuyu) [18g, pits removed].
In various conditions of Cold-Warm diseases (a general term for exogenous febrile diseases), it is most taboo to mistakenly use Qi-breaking medicines. If there is pain in the epigastrium or chest and hypochondrium, adding Frankincense (Ruxiang), Myrrh (Moyao), Fructus Toosendan (Chuanlianzi), and Salvia Miltiorrhiza Radix et Rhizoma (Danshen) can stop the pain; for abdominal pain, adding Paeoniae Radix Alba (Baishao) is effective. If the pain is caused by the exterior not being resolved, trapping stagnant heat, resolving the exterior and clearing heat will naturally stop the pain. However, if one mistakenly ingests Qi-breaking items such as Areca Seed (Binglang), Green Tangerine Peel (Qingpi), Curcuma Longa Rhizoma (Yujin), and Aurantii Fructus Immaturus (Zhiqiao), damaging the Great Qi (Zongqi) in the chest, wind-cold will sink inward through the deficiency, often transforming into Chest Binding (Jie Xiong, accumulation of phlegm-fluids in the chest/diaphragm). Even if the disease has transmitted deeply through the meridians, and the intestines and stomach have not reached a state of severe excess where purgatives are indicated, using Qi-breaking medicines together with cold-cooling medicines can easily cause slippery diarrhea, leading to myriad complications. I have frequently witnessed such physicians harming patients, feeling deep compassion. Therefore, for those who developed Chest Binding after taking Qi-breaking medicines, I formulated Dangxiong Tang (Chest-Sweeping Decoction) to remedy the error. For those who developed slippery diarrhea after taking Qi-breaking medicines, I formulated this decoction to remedy the error. Ultimately, mild errors can be remedied, but severe errors at a critical juncture are truly difficult to reverse.
To treat a condition where solid heat is originally accumulated in the chest, compounded by an external contraction. The internal heat is constrained by the external pathogen and cannot vent.
The patient occasionally feels irritability, or experiences panting, or chest/hypochondriac pain, with a pulse that is surging (Hong), slippery (Hua), and long (Chang).
Forsythiae Fructum (Lianqiao) [30g]
Raw Gypsum Fibrosum (Shi Gao) [18g, crushed]
Cicada Slough (Chantui) [6g, feet and soil removed]
Arctii Fructum (Niubangzi) [6g, stir-fried and crushed]
For panting, double the dosage of Arctii Fructum (Niubangzi). For chest pain, add Salvia Miltiorrhiza Radix et Rhizoma (Danshen) and Myrrh (Moyao), each 9g. For hypochondriac pain, add Bupleuri Radix (Chaihu) and Fructus Toosendan (Chuanlianzi), each 9g.
The condition primarily treated by this formula corresponds to the condition treated by Da Qing Long Tang (Major Blue Dragon Decoction) in the *Shanghan Lun* (Treatise on Cold Damage). However, Da Qing Long Tang is suitable for Shanghan (Cold Damage), whereas this formula is suitable for Wenbing (Warm Diseases). In cases of Shanghan where chest irritability is extremely severe, this formula can also be used to replace Da Qing Long Tang; hence the name "Youlong" (Resembling the Dragon).
A woman, over thirty years old, suffered from chest pain radiating to the hypochondrium and heat in the heart. Medicines for opening the chest, regulating Qi, and clearing fire were ineffective. Upon my examination, her pulse was floating, surging, and long. I determined that she had pre-existing stagnant heat in the Upper Jiao (Upper Burner), which was constrained by wind-cold, causing the wind-cold and stagnant heat to contend and produce pain. She was treated with this decoction, added with Myrrh (Moyao) and Fructus Toosendan (Chuanlianzi), each 12g. After one dose, she sweated and recovered.
An elderly man, over seventy years old, suffered from a chronic consumptive disease. Due to contracting cold in winter, his consumptive disease relapsed, presenting with panting and cough. Within two to three days, phlegm-saliva became abundant, and there was烦热 (irritability and heat) in the Upper Jiao. His pulse was surging, long, floating, and rapid. He was administered this decoction, added with Scrophulariae Radix (Xuanshen) and Codonopsis Radix (Lushen), each 12g. After one dose, he sweated and recovered.
My student Liu XX once treated a forty-year-old male with severe phlegm-panting due to external contraction. After four to five days, his pulse was surging and slippery, and his tongue coating was white with slight yellow. Liu XX prescribed this decoction, using 30g of Raw Gypsum (Shi Gao) and 9g of Forsythiae Fructum (Lianqiao). After one dose, he sweated profusely, the external heat subsided, but the panting was not fully resolved. Re-examining the pulse, it was peaceful and normal, though slightly weak. Subsequently, I used Conglong Tang (Following Dragon Decoction), removing Perillae Fructum (Suzi) and adding Codonopsis Radix (Lushen) 12g. After one dose, full recovery was achieved. Hearing this, I rejoiced and said: Phlegm-panting due to external contraction is the indication for Xiao Qing Long Tang (Minor Blue Dragon Decoction). My Youlong Tang was originally intended to replace Da Qing Long Tang, but now it can also replace Xiao Qing Long Tang. This was beyond my initial expectation. By slightly adjusting the heaviness and lightness of the herbal ingredients, remarkable efficacy can be achieved anew. This demonstrates that the application of formulas depends on the practitioner; those with wisdom can adapt flexibly.
Note: Forsythiae Fructum (Lianqiao) is inherently not a diaphoretic (sweat-inducing) herb, nor do various materia medica texts claim it induces sweating. Only when the person harbors internal heat does a dosage of 30g inevitably induce sweating, and its diaphoretic power is gentle yet prolonged. Because its power is gentle, it avoids profuse sweating; because its power is prolonged, taking it before sleep allows for a mild muscle-relieving effect throughout the night. Furthermore, it soothes Liver Qi stagnation and drains Lung Qi excess. To view it merely as an essential herb for sores and ulcers is to fail to truly understand Forsythiae Fructum. When using Forsythiae Fructum to induce sweating, green specimens must be used for potency. This herb is green when tender and yellow when mature. Generally, tender plants possess vigorous ascending and dispersing Qi; therefore, Forsythiae Fructum used for inducing sweating must be green Forsythiae Fructum.
