There is a type of heatstroke syndrome characterized by spontaneous sweating, coldness in the back, and redness of the face. The face must appear thoroughly red, purplish, and swollen. The whites of the eyes must be yellow. It may also be accompanied by headache and coarse breathing, restlessness, and even mania or confusion. Upon careful differentiation confirming it as a Yangming disease (disease of the Yangming channel), Baihu Tang (White Tiger Decoction) should be used.
(If the pulse is not surging and large, and there is no excessive thirst, it is feared that Baihu Tang may not be suitable. If a surging pulse and excessive thirst are both present, along with symptoms such as back coldness and yellowing of the eyes, Guizhi combined with Baihu Tang is appropriate, or Cangzhu (Atractylodis Rhizoma) can be added. Otherwise, Li Dongyuan's Qingshu Yiqi Tang (Clear Summerheat and Boost Qi Decoction) is more stable.)
There is a condition involving headache, delirious speech, coldness reversing up from the lower legs, fullness in the chest and abdomen, and profuse sweating. Such individuals are often injured by dampness, and subsequently contract summerheat; this disease is named Shiwēn (Damp-Heat). The pulse is superficial (cun position) soggy and weak, and deep (chi position) small and rapid.
The treatment focuses on the Taiyin spleen; diaphoresis (sweating method) is contraindicated. Baizhu Jia Cangzhu Tang (Baihu Decoction plus Atractylodis Rhizoma) is the primary formula. *Baoming Ji* states: From after Lixia (Start of Summer) to Chushu (End of Heat) around Liqiu (Start of Autumn), those suffering from Cold Damage often have slightly cool bodies, spontaneous sweating, and heavy limbs; this is called Shiwēn (Damp-Heat). Cangzhu Shigao Tang (Atractylodis Rhizoma and Gypsum Decoction) is the main treatment. (This is identical to Baizhu Jia Cangzhu Tang. Alternatively, several doses of Renshen Baihu Tang (Ginseng White Tiger Decoction) can be administered first. The Shiwēn syndrome is extremely difficult to treat: clearing heat causes the pathogen to sink deeper, drying dampness depletes body fluids, and tonification aids the pathogen's tyranny, inevitably leading to entangled complications. The Sanren Tang (Three Seeds Decoction) from the recent *Wenbing Tiaobian* (Systematic Differentiation of Warm Diseases) employs an extremely exquisite method, as it specifically establishes laws based on the Triple Burner, surpassing the Six Channels approach of Baihu plus Cangzhu.)

If one sits in cool pavilions, water pavilions, high towers, or grand halls during summer and indulges in eating melons and raw/cold foods, one can also suffer from Cold Injury. One must not rigidly adhere to the notion that hot medicines are forbidden in midsummer. Fuzi Lizhong Tang (Aconite Decoction for Regulating the Middle) can be used. (Dashun San (Great Harmony Powder) and Lengxiang Yinzi (Cool Fragrance Beverage) are also excellent.)
In summer, due to overexertion, irregular diet, and illness caused by drinking frozen beverages, the pulses at the right hand's three positions are deep, fine, and faint, indicating a Taiyin disease. The pulses at the left hand's three positions are slightly floating and wiry, indicating deficient Yang floating on the exterior. The body feels heavy, limbs are cold, there is spontaneous diarrhea with undigested food, the patient prefers wrapping themselves in clothes, breath is difficult to distribute, and there is reluctance to speak. This is due to the Spleen being invaded by Cold-Dampness and insufficiency of Middle Qi. The method should employ Sini Tang (Reverse the Extremity Decoction). However, the *Neijing* (Inner Canon) has the warning "avoid using hot remedies when the season is hot"; therefore, it is appropriate to treat from the Middle Jiao. Treating from the Middle means warming it. Qian Shi Baizhu San (Qian’s Atractylodes Powder) with Shengma (Cimicifugae Rhizoma) added is the main treatment. After one or two doses, if diarrhea stops, the body becomes warm, and spirit returns, Yigong San (Extraordinary Assistance Powder) or Zhizhong Tang (Regulate the Middle Decoction), which are acrid-warm substances, can be used for two or three doses.
