5. Externally obstructing eye diseases involve no cold syndrome, a point concluded in one sentence; the five wheels (referring to the pupil, cornea, canthi, sclera, and eyelids) turning red is due to the generation of fire pathogen. Internally obstructing eye diseases involve deficiency, specifically weakness of the Heart and Kidney; thus, the eyes appear as if unaffected by illness, yet their brightness is impaired. Fire serves as an external mirror, while Water illuminates internally; nourishing the Spirit (Shen) and strengthening Kidney Water will naturally yield results. Various opacities caused by the internal transformation of the Five Winds are readily treatable with specific ophthalmological methods.
[Annotation] Externally obstructing eye diseases: Zhang Zihe stated, "The eyes do not become diseased without the influence of Fire." Therefore, when the five wheels turn red: the Qi Wheel (sclera) reflects Fire invading the Lung; the Flesh Wheel (eyelids) reflects Fire invading the Spleen; the Wind Wheel (cornea) reflects Fire invading the Liver; the Wood Wheel (pupil) reflects Fire invading the Kidney; and the Blood Wheel (canthi) reflects excessive Fire. Thus, treating Fire resolves the issue in one step. Methods for treating Fire: Pharmacologically, use salty-cold herbs to induce vomiting or purgation; acupuncturally, needle Shenting (GV24), Shangxing (GV23), Xinhui (GV22), Qian Ding (GV21), and Baihui (GV20). Opacities can be made to recede immediately, pain to cease immediately, dimness to clear immediately, and swelling to dissipate immediately. Internally obstructing eye diseases: Although they also lack cold syndromes, they involve deficiency. Deficiency may accompany heat, constituting deficient-heat; thus, there is no redness, swelling, or pain. The eyes resemble those of a healthy person but lack luster and brightness. Heart deficiency leads to insufficient Spirit; since Spirit belongs to Fire, which is dim within but bright externally, it fails to reflect externally, resulting in loss of brightness. Kidney deficiency leads to insufficient Essence; since Essence belongs to Water, which is dim externally but bright internally, it fails to illuminate internally, resulting in loss of brightness. For Heart deficiency, nourish the Heart Spirit; for Kidney deficiency, strengthen Kidney Water, thereby restoring vision. Various opacities arising from the internal transformation of the Five Winds, such as Round Opacity (Yuan Yi), Ice Opacity (Bing Yi), Clear Opacity (Qing Yi), Rough Opacity (Se Yi), Scattered Opacity (San Yi), Horizontal Opacity (Heng Yi), Floating Opacity (Fu Yi), Sunken Opacity (Chen Yi), Crescent Moon Opacity (Yan Yue), Date Flower Opacity (Zao Hua), Yellow Center Opacity (Huang Xin), and Black Wind Opacity (Hei Feng), are all listed in ophthalmological texts. Standard formulas provide treatment methods, which cannot be exhaustively described here; this is merely a general overview.
