Ren Shen (Ginseng Radix), Bai Zhu (Atractylodis Rhizoma), Fu Ling (Poria), Gan Cao (Licorice Root), Dang Gui (Angelicae Sinensis Radix), Chuan Xiong (Ligustici Chuanxiong Rhizoma), Shao Yao (Paeoniae Radix), Di Huang (Rehmanniae Radix).
For patients with Qi and Blood deficiency, even if the smallpox (Dou Zheng) course is favorable, this formula is indicated.
Medicine values prevention before disease occurs. Even if the smallpox course is favorable, if the patient's Qi and Blood are deficient, failure to tonify may lead to subsequent complications. Therefore, Ren Shen, Bai Zhu, Fu Ling, and Gan Cao are used to tonify Qi; Dang Gui, Chuan Xiong, Shao Yao, and Di Huang are used to nourish Blood.
Chuan Shan Jia (Pangolin Scale, stir-fried, 1 liang), She Xiang (Musk, 1 qian).
Take 1 qian, mixed with Zicao Decoction (Arnebiae seu Lithospermi Radix Decoction).
For cases presenting with black, sunken pustules between the 5th and 6th days of onset, this formula is indicated.
Black and sunken lesions indicate a critical condition. "Black" refers to the color change caused by contact with foul or turbid pathogens. "Sunken" indicates that the healthy Qi has sunk and failed to raise the pustules. Chuan Shan Jia and She Xiang belong to categories considered pungent, fishy, and foul. Why use them? It is because the pathogen of smallpox, when touched externally by foul factors, tends to sink inward; when touched internally by foul factors, it tends to protrude outward. The root cause is deficiency and timidity of Blood and Qi, leading to such manifestations. Ren Ya San (Human Tooth Powder) follows the same principle.
Zi Cao (Arnebiae seu Lithospermi Radix), Xiong Huang (Realgar), Ba Dou (Crotonis Fructus) (equal parts).
Grind into fine powder, apply mixed with oil-based rouge.
For cases of Dou Ding (Smallpox Carbuncle) or Dou Mu (Smallpox Mother), puncture with a needle and apply a small amount of this medicine.
Dou Ding presents in two colors: Zi Ding (Purple Carbuncle) and Bai Ding (White Carbuncle). Dou Ding appears in three locations: first on the face, then on the abdomen, and finally on the feet. In this formula, Zi Cao clears heat and resolves toxicity while unblocking orifices; Xiong Huang clears heat and resolves toxicity while promoting Qi movement; Ba Dou transforms toxicity and extracts the carbuncle. This is an expedient formula for treating Dou Ding. "Dou Mu" refers to the initial eruption where the body is smooth and lustrous, with few eruptions that have not yet raised. Among them, one or two lesions develop and swell as large as those on the 8th or 9th day, named Dou Mu. Immediately puncture and apply this medicine. Otherwise, the other pustules will gradually fade and disappear, leaving only dregs on the skin surface, accompanied by cold limbs, spontaneous sweating, vomiting, diarrhea, restlessness, and death.
Ren Shen (Ginseng Radix), Huang Qi (Astragali Radix), Gan Cao (Licorice Root), Bai Shao Yao (White Peony Root), Chuan Shan Jia (Pangolin Scale), Dang Gui (Angelicae Sinensis Radix), Chuan Xiong (Ligustici Chuanxiong Rhizoma), Mu Xiang (Aucklandiae Radix), Jin Yin Hua (Lonicerae Japonicae Flos), Zao Jiao Ci (Gleditsiae Spina), Shan Zha (Crataegi Fructus), Gan Jiang (Zingiberis Rhizoma).
For cases between the 7th and 8th days of onset presenting with withered, pale pustules that fail to raise and lack pus, this formula is indicated.
By the 7th or 8th day, smallpox should be filling with pus and raising. If the base of the lesion is pale, it is attributed to weak Blood. If the lesions fail to raise and lack pus, it is attributed to deficient Qi. Therefore, Ren Shen, Huang Qi, and Gan Cao are used to greatly tonify Qi. Additionally, Dang Gui, Chuan Xiong, and Bai Shao Yao are used to greatly tonify Blood. Chuan Shan Jia, Zao Jiao Ci, and Jin Yin Hua are effective at resolving toxicity. Gan Jiang, Mu Xiang, and Shan Zha are effective at resolving stagnation.
