Aconiti Lateralis Radix Praeparata (Fuzi, 附子), Zingiberis Rhizoma (Ganjiang, 干姜), Ginseng Radix et Rhizoma (Renshen, 人参), Glycyrrhizae Radix et Rhizoma (Gancao, 甘草), Atractylodis Macrocephalae Rhizoma (Baizhu, 白术).
Abdominal pain, blackening of the forehead, contraction and drawing-in of the hands and feet, a deep and submerged pulse, and insufficient breath to sustain respiration indicate sudden death due to severe Cold invading the interior. This formula is indicated for such cases.
Although there are numerous causes for abdominal pain, the combination of a blackened forehead, contracted extremities, and a deep, submerged pulse serves as definitive evidence of Cold invasion. The reason for the inability to breathe adequately lies in the physiology that exhalation is governed by Yang and inhalation by Yin. When the Three Yin meridians are afflicted by True Cold, the pathological process impedes inhalation. If inhalation ceases, exhalation cannot exist independently, leading to sudden death. Since Cold is treated with warmth, Aconiti Lateralis Radix Praeparata (Fuzi) and Zingiberis Rhizoma (Ganjiang) are employed. Ginseng Radix et Rhizoma (Renshen), Atractylodis Macrocephalae Rhizoma (Baizhu), and Glycyrrhizae Radix et Rhizoma (Gancao) are used to generate and restore the Qi of respiration. After administering the medicine, moxibustion should be applied to Guanyuan (CV4). This represents a method of treating both internally and externally. In such conditions, patients who have been dead for one day and night may recover upon treatment; do not neglect this condition due to its critical nature.
Ginseng Radix et Rhizoma (Renshen, 人参), Ophiopogonis Radix (Maidong, 麦门冬), Schisandrae Fructus (Wuweizi, 五味子), Moslae Herba (Xiangru, 香薷).
When the human body is inherently deficient in Yin, and one encounters a journey under summer heat, compounded by dietary distress and fatigue, resulting in sudden collapse and unconsciousness, this formula is indicated.
Inherent Yin deficiency leads to unopposed Yang activity. Encountering summer heat further exacerbates Yang hyperactivity. Added to hunger, distress, and fatigue, Yin becomes increasingly depleted. The sudden collapse and unconsciousness occur for two reasons: first, Yin deficiency causes isolated Yang to verge on dispersion; second, Summer-heat pathogenic factors invade the mansion of the Spirit (Shen) exploiting the deficiency. Therefore, Ginseng Radix et Rhizoma (Renshen) is used to benefit primordial Qi and secure against collapse, while Moslae Herba (Xiangru) dispels pathogens and repels summer heat. The cooling property of Ophiopogonis Radix (Maidong) supports the Lung, which is too weak to function properly. The sour astringency of Schisandrae Fructus (Wuweizi) consolidates the True Qi that is about to disperse.
Ginseng Radix et Rhizoma (Renshen, 人参), Atractylodis Macrocephalae Rhizoma (Baizhu, 白术), Poria (Fuling, 茯苓), Glycyrrhizae Radix et Rhizoma (Gancao, 甘草), Bamboo Sap (Zhuli, 竹沥), Ginger Juice (Jiangzhi, 姜汁).
Sudden death accompanied by phlegm-rattling sounds in the throat is termed Phlegm Jue (Phlegm Syncope); this formula is indicated.
Phlegm Jue refers to the condition where deficient Yang carries Phlegm upward in rebellion. It causes sudden death because stubborn Phlegm obstructs the pathways of Clear Yang ascent and respiration. Once the Phlegm is obstructed and the Qi attempts to flow through, sounds are produced in the throat. The Classics state: "Those who are robust recover when Qi flows freely; those who are frail develop disease when pathogens adhere." Therefore, the warming and tonifying properties of Ginseng Radix et Rhizoma (Renshen), Atractylodis Macrocephalae Rhizoma (Baizhu), Poria (Fuling), and Glycyrrhizae Radix et Rhizoma (Gancao) are used to strengthen Qi. They are assisted by Bamboo Sap (Zhuli) and Ginger Juice (Jiangzhi) to move and resolve Phlegm.
