Mr. Wei stated: "Douchuang" (Smallpox/Chickenpox lesions) is named after its resemblance to beans based on its shape. A favorable course presents smooth morphology, while an unfavorable course presents inverted morphology. The pathology originates from the initial formation of essence and blood, and completes after the influence of excessive fire. During conception, desire generates fire; when desire is intense, essence and blood take form, and fire enters the body. Waiting for the right time to erupt, it must rely on qi and blood. Thus, douchuang arises. Toxins cannot move without qi guidance, nor can they be carried without blood support. If qi is not vigorous, how can it expel toxins? If blood is not nourishing, how can it bear toxins? Generally, the function of qi and blood in guiding and carrying relies on Yuanqi (Original Qi). When Yuanqi is substantial, recovery is easy. Variations in density (sparse vs. dense) reflect the depth of toxin acquisition, determining auspiciousness or peril. Sometimes, activation by seasonal pathogens involves heavenly-earthly evil qi interacting with residual toxins in the body, following similar patterns.
Although children inherit turbid toxins from the three fluids in the womb, no case occurs without the interplay of external contraction and internal injury. Initially, fever resembles wind-injury. In this ambiguous stage, mild exterior-releasing methods should be used first to dissipate toxic vapors; otherwise, toxic vapors will accumulate in the zang-fu organs forming douchuang. It is said that mismanagement of various heat conditions leads to this syndrome, due to failure to resolve toxins. For cases starting with external contraction, before red spots appear during early fever, mild sweating is required to resolve the exterior; this is the period when douchuang has not yet erupted and heat is obstructed and excess. For cases starting with internal injury, mild purgation is required to resolve the interior when red spots have not yet appeared and interior heat is obstructed and excess. If sweating is wrongly applied during the eruption phase before resolution, it leads to ulceration; if purgation is wrongly applied, it leads to sinking/collapse. Generally, resolving the exterior should not be too cold, and regulation should not be too hot. Slight deviation in cold or heat gives rise to various diseases. Broadly speaking, heat injures the Lung, which governs skin and hair; cold injures the Spleen, which governs muscles.
Mr. Yang stated: "Douchuang" eruptions occur in the muscles, governed by the Yangming Stomach Meridian. Once the Spleen Earth is warm, Stomach qi flows smoothly. Earth overcoming Water prevents sinking/collapse complications.
Mr. Zhang stated: The Lung governs qi. Uniform qi flow ensures smooth and rapid eruption of "douchuang." Blood follows qi; if qi reverses, stagnation occurs. Before eruption, mild sweating is urgently needed. After eruption but before resolution, warming tonification is urgently indicated to harmonize Ying and Wei (Nutritive and Defensive levels) and ensure smooth qi and blood flow, preventing obstruction, itching, or collapse. This is an excellent method for regulation and treatment.
