(Zhang Zhongjing named it Sanwu Nongpo Decoction)
Indicated for Yangming syndrome of cold damage, characterized by delirium, hard stool, tidal fever, and panting. Also indicated for miscellaneous diseases involving upper jiao (upper burner) stagnation and fullness with obstruction.
Rhubarb Root (Da Huang) (4 liang) Magnolia Bark (Nongpo) (2 liang, stir-fried with ginger) Immature Bitter Orange (Zhishi) (3 pieces, stir-fried with bran).
These are herbs for treating Shaoyang (Lesser Yang) and Yangming syndromes. When pathogenic factors are in the upper jiao, they cause fullness; in the middle jiao, they cause distension. Stomach excess heat leads to tidal fever (like the tide, arriving at regular intervals; Yangming Dry Metal qi is prosperous during Shen and You hours [3-7 PM], hence tidal fever occurs in the late afternoon. Tidal fever in cold damage indicates stomach excess, not deficiency). When yang pathogenic factors invade the Heart, mania ensues, resulting in delirium; Stomach heat harassing the Lung causes...
To avoid injuring the true Yin of the lower jiao (Da Chengqi Tang treats all three jiaos; Xiao Chengqi Tang does not affect the lower jiao; Tiaowei Chengqi Tang does not affect the upper jiao. Note: Yangming syndrome includes Zhengyang Yangming, Taiyang Yangming, and Shaoyang Yangming. When pathogens transmit from the Yangming channel to the Stomach fu organ, presenting without aversion to cold but with abdominal fullness and hard stool, Da Chengqi Tang should be used to purge; if there is profuse sweating, slight fever, and mild aversion to cold, the exterior is not yet resolved, the fever is not tidal, and Chengqi Decoctions should not be given; if there is significant abdominal fullness and obstruction, Xiao Chengqi Tang may be given to gently harmonize Stomach Qi; do not cause severe purging. This implies that Yangming disease may remain in the channel and not fully enter the fu organ, requiring particular caution. Yangming and Shaoyang diseases often transmit from Taiyang. Cheng Wuji stated: Transmission from Taiyang and Shaoyang is commonly known, while transmission from the Three Yin is rarely recognized. There are many urgent purgation indications in Three Yin diseases; is this not Zhang Zhongjing's subtle intent? The Classic states: In cold damage, if the pulse is floating and slow, and hands and feet are warm, it relates to Taiyin, and jaundice should develop. If urination is free, jaundice will not develop. After seven or eight days, if the stool becomes hard, it is Yangming disease. Cheng Jiaoqian stated: This syndrome is called Taiyang-Yangming. Yangming disease is fundamentally Stomach excess. Not only can pathogens in the Three Yang channels transfer to Yangming, but those in the Three Yin channels can also transfer to Yangming. Extending this logic, the use of three major Chengqi formulas in Shaoyin and one Xiao Chengqi formula in Jueyin—none are not diseases transferring to Yangming).
The *Jingui Yaolue* uses this formula to treat chest fullness caused by fluid retention (Zhi Yin), renaming it Nongpo Dahuang Decoction. Adding Qianghuo to this formula creates Sanhua Decoction (*Yi Fa Ji Yao*): Indicated for excess conditions due to wind-stroke pathogenic factors, with obstruction of both bowel and bladder movements ("Sanhua" means to unblock the three jiaos and restore their normal transformative functions. Qianghuo is added because the root of the condition lies in Wind. However, wind-stroke...)
