For those with Qi deficiency, Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction) is used (see Qi). For those with Blood deficiency, Bu Gan Yang Rong Tang (Nourish the Liver and Enrich the Nutritive Decoction) or Si Wu Tang (Four Substances Decoction) (see Blood) added with Jing Jie Sui (Schizonepeta Spike) is prescribed. For Kidney Yang deficiency, Ba Wei Wan (Eight-Ingredient Pill) (see Deficiency Damage) or Hei Xi Dan (Black Tin Pill) (see Hiccup) are used. For Kidney Yin deficiency, Liu Wei Wan (Six-Ingredient Pill) (see Deficiency Damage) is prescribed. For phlegm stagnating in the Zhongwan region causing vomiting and reflux, Xuan Fu Hua Tang (Inula Flower Decoction) is used. When phlegm obstruction prevents expulsion, emesis is induced using Du Sheng San (Unique Holy Powder) (see Food Injury). After emesis, herbs that clear the upper jiao with acrid-cool properties can be used, such as Fang Feng Tong Sheng San (Saposhnikovia Universal Rescue Powder) (see Wind Stroke) added with Ban Xia (Pinellia Tuber), etc. Qing Dai San (Indigo Naturalis Powder) blown into the nose to extract phlegm-saliva is highly effective. When phlegm-saliva is abundant and both large and small bowel movements are obstructed, purgation is employed. The presence of saliva expectoration indicates phlegm; Ban Xia (Pinellia Tuber), Ju Hong (Tangerine Peel), Xuan Fu Hua (Inula Flower), etc., are used. For wind-phlegm, Nan Xing (Arisaema Rhizome) and Jiang Can (Bombyx Batryticatus) are used. For vertigo caused by water retention, refer to the Edema section. For cases caused by dampness, the head feels heavy and difficult to lift, especially severe in deficient individuals; Wu Ling San (Five-Ingredient Powder with Poria) (see Cold-Dampness) and Chu Shi Tang (Remove Dampness Decoction) (see Internal Dampness) are prescribed. For cases caused by heat, characterized by irritability and thirst, Zhi Zi (Gardenia Fruit), Huang Lian (Coptis Rhizome), and Gan Ju (Chrysanthemum Indicum) are used. In excess patterns, Da Huang (Rhubarb Root and Rhizome) stir-fried three times with wine, powdered, and taken with tea, one to two qian per dose, is used. For cases caused by Qi stagnation, resulting in depressed spirit, Xiao Yao San (Free and Easy Wanderer Powder) (see Stagnation) added with Bo He (Mint) and Ju Hua (Chrysanthemum Morifolium) is prescribed. For deficiency-cold patterns, San Wu Qi San (Three-Five Seven Powder) or Xiong Fu Tang (Ligusticum and Aconite Decoction) (see Blood) are suitable. Sheng Liao Zheng Yuan Yin (Raw Material Upright Origin Drink) added with 1 qian of Lu Rong (Deer Antler Velvet) is administered, followed by Ling Sha Dan (Spirit Cinnabar Pill) (see Vomiting). Alternatively, Zheng Yuan Yin (Upright Origin Drink) added with 15 fried Chuan Jiao (Sichuan Pepper) grains is given, followed by Rong Zhu Wan (Velvet Pearl Pill). If ineffective, Lu Rong (Deer Antler Velvet) alone is used, five qian per dose, decocted in Huifu Jiu (Ash-free Wine) with a small amount of She Xiang (Musk) added for consumption. (Since deer antler grows on the head, it treats vertigo.) Profuse diarrhea leading to Yin exhaustion and floating deficient Yang, causing frequent vertigo or blurred vision, is critical. Vertigo cannot be treated without Tian Ma (Gastrodia Rhizome); it must not be omitted.
Neck stiffness and pain mostly result from Wind-Cold pathogens lodging in the Three Yang meridians, or from phlegm-damp stagnation, Blood deficiency with sprain, prolonged sitting, or pillow misalignment. For Wind-Cold invasion, Qu Xie Tang (Expel Pathogen Decoction) is used. For abundant phlegm, Xiao Feng Huo Tan Tang (Dispel Wind and Resolve Phlegm Decoction) is used. For abundant dampness, Jia Wei Sheng Shi Tang (Modified Win-Dampness Decoction) is used. For Blood deficiency with excessive Fire and dry tendons, manifesting as neck stiffness and tension, slight pain upon movement, worse on the left side, with a wiry and choppy pulse, Shu Feng Zi Xue Tang (Dispel Wind and Nourish Blood Decoction) is prescribed. For neck stiffness and inability to turn due to sprain, prolonged sitting, or pillow misalignment, this is often caused by Kidney deficiency failing to nourish the Liver, Liver Blood deficiency failing to moisten the tendons; Liu Wei Wan (Six-Ingredient Pill) (see Deficiency Damage) should be taken regularly.
