A patient experienced a bowel movement every morning upon rising, with occasional abdominal pain or no pain at all. Taking hot-natured medicines on an empty stomach proved ineffective. Later, a wise observer noted this and instructed the patient to take another dose of hot-natured medicine before the evening meal, after which the condition stabilized. By continuing this regimen long-term, the patient remained healthy. This is because although warming medicines taken on an empty stomach in the early morning have dissipated by evening, leaving the body unable to resist the yin-cold qi of the night. Administering hot-natured medicine again in the evening ensures that warming agents remain in the abdomen throughout the night, thereby overcoming the yin-cold qi. This principle applies to the treatment of all cold diseases.
There are three types: first, diarrhea caused by summerheat injury; second, diarrhea caused by cold substances, food, melons, fruits, and drinking water; third, excessive heart fire in summer leading to heat in the Heart meridian, causing difficult urination, which then affects the Large Intestine, termed "failure to separate clear from turbid" (Shuigu Bufen). For diarrhea due to summerheat injury, accompanied by mental restlessness, headache, and thirst, Xiangru Yin (Elsholtzia Decoction) and Wujin San (Black Gold Powder) are suitable. For diarrhea due to cold substances, accompanied by abdominal pain, watery stool, and undigested food, warming medicines such as Fuzi (Prepared Aconite Root), Lizhong Wan (Regulate the Middle Pill), Erqi Dan (Two Qi Pill), Zhengwan, and Danzisuan Wan (Red Perilla Pill) are appropriate. For failure to separate clear from turbid, Dashun San (Great Harmony Powder) and Wuling San (Five Ling Powder) are indicated. These two medicines specialize in separating clear from turbid. This disease is common in summer months, hence people often use these remedies. In cases of diarrhea, one must not hastily stop it with hot-natured medicines, lest it transform into dysentery (Li Ji).
Treatment for severe lower abdominal pain caused by Stomach qi disharmony. During the Yuanfeng era, Chancellor Wang Xungong suffered from incessant lower abdominal pain. Imperial physicians were dispatched to treat him thoroughly, but all efforts failed. Even hot-natured medicines like Fuzi (Prepared Aconite Root), Liuhuang (Sulfur), and Wuye Yi Wan (Five Nights One Pill) yielded no improvement. The Prince Consort Zhang Duwei ordered women's hair to be burned to ash, finely ground and sifted, then mixed with warm wine and administered (2 qian). The pain ceased immediately (Liang Fang).
Diseases of the head and heart include Jue Tong (Rebellious Pain) and Zhen Tong (True Pain). When the Hand-Taiyang meridians are invaded by wind-cold, it is named Jue Tou Tong (Rebellious Headache); if the pain penetrates deeply into the brain, it is named Zhen Tou Tong (True Headache). When qi from the Five Viscera interferes with each other, it is named Jue Xin Tong (Rebellious Heartache); if the pain is severe, localized solely in the heart region, and accompanied by cyanosis of the hands and feet, it is named Zhen Xin Tong (True Heartache). Patients with True Heartache who manifest symptoms in the morning will die by evening, and those who manifest symptoms in the evening will die by morning.
Zhang Shishun was transferred to the capital during midsummer. Suffering from intense heat, he consumed excessive ice and snow and drank papaya juice, causing cold accumulation in the interior. Consequently, he contracted Pi Teng (Spleen Pain). Medicines were taken continuously without any alleviation. Years later, Daoist Renzhai encountered another Daoist, who said, "I do not begrudge your offerings. Simply take twenty-one grains of Hanjiao (Sichuan Pepper), soak them in Jiangshui (fermented grain water) overnight, strain them out, and swallow them with the Jiangshui. That is all." Zhang followed the advice. The next day, as soon as the pepper entered his abdomen, the pain vanished completely, and it never recurred (Leibian).
