The tube is rolled from thin copper sheet, with a thickness similar to that of an ivory tendon, and a length of slightly over two cun. It is hollow in the center, resembling a slender bamboo tube, with tightly welded seams. One end of the seam is filed flat, while the other is filed into a slanted point. During use, the pointed end is inserted into the lesion's opening; after a short moment, pus will shoot out from the tube like an arrow.
Stagnant heat in the Spleen and Stomach steams upward, causing white, rotting lesions on the mouth and tongue that pile up like snowflakes; in children, this condition is known as E Kou Chuang (Goose Mouth Ulcer). Initially, Niu Jie Tang (Ox Tuber and Platycodon Decoction) [see formula below] should be used to disperse and ascend the fire. If bitter-cold herbs are administered too early, they may freeze and trap the fire, leading to dangerous complications such as throat ulceration, asthma, and hoarseness.
After an illness, if both Yin and Yang are deficient, floating fire from the Middle Jiao flares upward, causing the gums to rot and smell foul. In such cases, Liu Wei Di Huang Wan plus Rou Gui (Six-Ingredient Rehmannia Pill with Cinnamon Bark) [see formula below] should be used to guide the fire downward. This differs from Excess Fire, which can be controlled using bitter-cold herbs to suppress its intensity.
Hard, distending, and painful masses within the abdomen and hypochondrium begin the size of a plum and gradually enlarge to the size of a cup. These arise from Spleen deficiency following an illness; consumption of chicken, meat, wheat products, or glutinous rice—which cannot be properly digested—stagnates outside the Zang-Fu organs but inside the body shell. Over time, blood vessels wrap around and glue these accumulations together, making them difficult to dissipate. Individuals with strong constitutions may undergo acupuncture several times and take Xiao Jian San (Eliminate Hardness Powder) [see formula below] to slowly resolve the masses. However, for those with weak constitutions and poor appetite, mistaken acupuncture will inevitably lead to abdominal fullness, becoming incurable.
Generally, the area below the knees is where dampness gathers. When dampness is excessive, it generates heat; heat accumulation corrodes muscle tissue, resulting in alternating itching and pain, with blood and fluid mixing and flowing out. Such conditions are difficult to treat effectively, much like low-lying depressions where sunlight struggles to dry standing water. Tu Xing San (Promote Circulation Powder) [see formula below] should be used to clear blood heat and promote the drainage of dampness.
All "Fa Wu" (Trigger Foods) must be strictly avoided in external diseases. Violating this prohibition causes immediate worsening of swelling, ulceration, and pain, and wounds that were healing will re-ulcerate. Physicians must instruct patients on these dietary restrictions beforehand. If children violate these restrictions after recovering from smallpox, they may experience vague pain and diffuse swelling in the limbs and joints, resembling Phlegm pouring through channels. The condition becomes chronic and difficult to cure; some patients may die from Fa Zheng (Trigger Disease), while others suffer permanent physical disability. Trigger foods include: beef, mutton, fish, crab, shrimp, clams, chicken, duck, seafood, pig head, Wang Gua (Trichosanthes cucumerina), mustard greens, celery, eggplant, pumpkin, adzuki beans, beetroot, spinach, taro, coriander, mushrooms, shiitake, daylily, red beans, bamboo shoots, tofu, wheat products, bean flour, gluten, duck eggs, and black cowpeas.
