The pulse at the Cun Kou (radial artery) is floating and moderate. Floating indicates wind, while moderate indicates bi-syndrome (obstruction). This obstruction is not wind-stroke; rather, the limbs suffer from vexation. The color of the spleen must turn yellow, and stagnant heat circulates throughout the body. The pulse at the Fu Yang (dorsalis pedis artery) is tight and rapid. Rapid indicates heat, which consumes grain (increases appetite); tight indicates cold, leading to fullness after eating. A floating pulse at the Chi position indicates kidney injury, while a tight pulse at the Fu Yang position indicates spleen injury. Wind and cold contend with each other; after eating, dizziness occurs. Grain qi fails to digest, causing bitterness from turbid qi in the stomach. Turbid qi flows downward, blocking urination. Yin is affected by cold, while heat flows into the bladder, resulting in whole-body yellowing, known as Grain Jaundice (Gu Dan). If the forehead is black, there is slight sweating, heat in the hands and feet, onset at dusk, urgency in the bladder but free urination, it is known as Labor Jaundice (Nu Lao Dan). If the abdomen is distended like water, it is difficult to treat.
Vexation and stifling sensation in the heart region, accompanied by heat and inability to eat, with frequent desire to vomit, is known as Wine Jaundice (Jiu Dan). In Yangming disease, if the pulse is slow, one cannot eat to fullness; upon becoming full, vexation, dizziness, and difficulty in urination occur. This suggests impending Grain Jaundice. Even if purgatives are used, abdominal fullness remains. This is due to the slow pulse.
Patients suffering from Wine Jaundice must have difficult urination. Their signs include heat in the heart and heat under the feet. For Wine Jaundice, some may present without obvious heat, with quiet and clear consciousness, but with abdominal fullness, desire to vomit, and dry nose. If the pulse is floating, induce vomiting first; if deep and wiry, purge first.
If a patient with Wine Jaundice has heat in the heart and desires to vomit, inducing vomiting will lead to recovery.
If Wine Jaundice is mistakenly treated with purgatives, it may eventually transform into Black Jaundice (Hei Dan), characterized by blue-black eyes, dark complexion, a sensation in the heart as if consuming garlic and leeks, black stool, and skin numbness. The pulse is floating and weak. Although the complexion is black, a faint yellow hue reveals its origin.
The Master said: When jaundice presents with fever, vexation, panting, chest fullness, and dry mouth, it is because fire therapy was used to force sweating during the onset, combining two sources of heat. However, jaundice generally arises from dampness. If the whole body is hot, with yellow face and heat around the umbilicus, the heat is interior, and purgation should be applied.
If the pulse is deep, thirst with desire to drink, and difficult urination occur, jaundice will develop.
Abdominal fullness, sallow-yellow complexion, restlessness preventing sleep, belongs to the category of Jaundice.
The Master said: The course of jaundice should last eighteen days. Treatment for more than ten days should result in improvement; if symptoms worsen, it is difficult to treat.
Furthermore, jaundice with thirst is difficult to treat, while jaundice without thirst is curable. If the disease originates in the Yin aspect, vomiting will occur; if in the Yang aspect, aversion to cold and fever will occur.
[Yinchen Hao Tang] Treats Grain Jaundice, characterized by alternating chills and fever, anorexia, dizziness after eating, mental unrest, and eventual yellowing of the body.
Yin Chen Hao (Artemisiae Scopariae Herba, 6 liang), Da Huang (Rhei Radix et Rhizoma, 3 liang), Zhi Zi (Gardeniae Fructus, 14 pieces, broken).
Crush the three herbs. Boil in 12 sheng of water. First boil Yin Chen Hao until the volume reduces by half. Add the other two herbs and boil until 3 sheng remains. Strain. Divide into three warm doses. Urine will become copious, resembling soap pod juice, bright red. After one night, abdominal fullness decreases, and yellow pigment is excreted via urine.
[Xiaoshi Fanshi San] Treats patients with jaundice who experience fever at Shen hour (3-5 PM) but feel aversion to cold, indicating Labor Jaundice. Symptoms include bladder urgency, lower abdominal fullness, whole-body yellowing, black forehead, and heat under the feet, evolving into Black Jaundice. If the abdomen is distended like water, stool is black or loose. This is Labor Jaundice, not fluid retention. Abdominal fullness makes it difficult to treat.
Xiao Shi (Natrii Sulfas, calcined), Fan Shi (Alumen, calcined), equal parts.
Grind into powder. Mix with barley porridge. Take one fang cun bi (square-inch spoonful) per dose, three times daily. Pathogens exit via stool and urine; black stool and yellow urine indicate efficacy.
[Zhi Zi Da Huang Tang] Treats Wine Jaundice with vexation/stifling sensation in the heart or painful heat.
Zhi Zi (Gardeniae Fructus, 14 pieces, broken), Zhi Shi (Immaturae Citri Fructus, 5 pieces, roasted), Dou Chi (Sojae Preparata, 1 sheng, wrapped in silk), Da Huang (Rhei Radix et Rhizoma, 1 liang).
Crush the herbs. Boil in 6 sheng of water to obtain 2 sheng. Strain. Divide into three warm doses, taken at any time.
[Yinchen Wuling San] Treats Jaundice. Another version combines Yinchen Tang and Wuling San.
Wuling San (Poria Powder, 5 fen), Yin Chen Hao Mo (Artemisiae Scopariae Herba powder, 10 fen).
Mix evenly. Take one fang cun bi before meals with warm water, three times daily.
[Dahuang Xiaoshi Tang] Treats Jaundice with abdominal fullness, difficult and reddish urination, and spontaneous sweating. This indicates exterior deficiency and interior excess, requiring purgation.
Da Huang (Rhei Radix et Rhizoma), Huang Bai (Phellodendri Cortex), Xiao Shi (Natrii Sulfas, 4 liang each), Zhi Zi (Gardeniae Fructus, 14 pieces, broken).
Crush the herbs. Boil in 6 sheng of water to obtain 2 sheng. Strain. Add Xiao Shi and boil to 1 sheng. Take in one dose.
[Huanglian San] Treats Jaundice with constipation and internal heat, proven effective.
Chuan Da Huang (Rhei Radix et Rhizoma, stir-fried with vinegar), Huang Lian (Coptidis Rhizoma, 2 liang each), Gan Cao (Licorice Root, roasted), Huang Qin (Scutellariae Radix, 1 liang each).
Grind into powder. Take 2 qian per dose with warm water after meals, three times daily.
[Bi Gou Di San] Treats Jaundice where the entire body turns golden. Proven effective.
Gou Di (Trichosanthis Pericarpium, 14 pieces), Mu Ding Xiang (Eugeniae Caryophyllata Flos, 1 piece), Shu Mi (Panicum miliaceum, 49 grains).
First grind Gou Di into fine powder. Add the other two ingredients and grind together. Sieve into powder. Before bed, have the patient hold a mouthful of water. Blow a small amount of powder (less than half a character's width) into both nostrils. Spit out the water and sleep. By midnight or the next morning, yellow fluid will be discharged. Wipe clean with cooked silk. Once discharge stops, take Huanglian San. Mild cases improve in five days; severe cases in half a month.
