Moxibustion for low back pain with inability to bend forward or backward: Have the patient stand upright, prop a bamboo staff on the ground, and measure the distance from the bottom of the staff to the umbilicus, marking it with a black dot. Then use this measurement to gauge the dorsal spine, and apply moxibustion at the corresponding point. The number of moxa cones (one cone burned per count) should correspond to the patient's age. After completion, hide the bamboo staff so others do not know. Moxibustion can also be applied to Shenshu (BL23, Kidney Shu), which is level with the umbilicus, located 1.5 cun lateral to the lower border of the spinous process of the fourteenth thoracic vertebra. Apply three or seven moxa cones.
Kunlun (BL60): Two points, located in the depression posterior to the lateral malleolus and superior to the calcaneus. Apply three moxa cones; treats pain in the loins and sacrum.
Weizhong (BL40): Two points, located in the center of the popliteal crease. Prick to bleed; treats swelling and pain in the loins and legs.
Leihuo Needle Method: On the fifth day of the fifth lunar month, select peach branches growing towards the east, peel off the bark and tips, shape both ends like an egg, retaining a one-cun sharp tip. During treatment, ignite the needle over a lamp, recite the incantation three times, place three or five layers of paper on the affected area, and press the burning needle against the paper. For deep-seated conditions, relighting the needle will result in immediate cure.
Incantation: Heavenly fire, earthly fire, Samadhi true fire. When the needle points to heaven, heaven opens; when it points to earth, earth splits. When it points to ghosts, ghosts perish; when it points to humans, humans attain longevity. All hundred diseases are eliminated, all ten thousand diseases vanish. I command by the strict decree of Taishang Laojun (The Highest Elder Lord).
Zhu Danxi stated: Flaccidity of the waist indicates latent heat in the Liver and Kidney meridians. It is appropriate to use Huangbai (Phellodendri Cortex) and Fangji (Stephaniae Tetrandrae Rhizoma), such as in Buyin Wan (Nourish Yin Pill). One should assess deficiency and excess, and select formulas from the Low Back Pain section accordingly.
Chen Wuyue in *San Yin Fang* (Formulas of Three Causes) stated: In Renzhe (Kidney Fixation) syndrome, the body feels heavy, the waist is cold as ice, appetite remains normal, urination is free and unobstructed, and the area below the waist feels cold and heavy as if bound by five thousand coins. Treatment should focus on draining dampness while simultaneously using warming and dispersing herbs.
(Sourced from *San Yin Fang*) Renshe Wan (Kidney Fixation Pill): Treats illness caused by Kidney deficiency, characterized by heaviness of the body, waist cold as ice, absence of thirst, free urination, normal diet, and heavy/painful sensation below the waist as if bound by five thousand coins.
Baizhu (Atractylodis Macrocephalae Rhizoma) Fuling (Poria) (each 4 liang) Ganjiang (Dried Ginger) Zhigancao (Prepared Licorice Root) (each 1 liang)
Crush the ingredients. Take 4 qian per dose, decoct in water, and take cold on an empty stomach. Shen Shi Tang (Draining Dampness Decoction) also treats Renzhe. (Formula see Dampness Section.)
(Sourced from *San Yin Fang*) Moyao Gao (Waist Rubbing Ointment): Treats low back pain due to Cold-Dampness.
Fuzi (Prepared Aconite Lateral Child Root) Wutoujian (Aconiti Radix Lateralis Preparata Tip) Nanxing (Arisaematis Rhizoma) (each 3.5 qian) Ganjiang (Dried Ginger) (1 qian) Xionghuang (Realgar) Zhaonao (Borneolum Syntheticum) Dingxiang (Caryophylli Flos) (1.5 qian) Shexiang (Moschus) (0.5 fen)
Grind into fine powder, mix with refined honey to form pills the size of a bullet. Use one pill per application, dissolve in fresh ginger juice to a thick porridge consistency, warm it, place in the palm, and rub onto the waist until fully absorbed into the skin. Secure with heated cotton clothing to bind tightly; the waist will feel hot as fire. Use one pill every three days, or add Wuzhuyu (Evodiae Fructus) and Guipi (Cinnamomi Cortex).
