如果病位在筋,表现为屈伸不利,对应肝脏,症状包括夜间睡眠易惊悸、饮食减少、小便频数;
如果病位在脉,表现为血液凝滞而不流通,对应心脏,症状使人面色萎黄,心下悸动剧烈,气逆上冲导致喘息不通,咽喉干燥且容易嗳气;
如果病位在骨,表现为身体沉重难以抬举,对应肾脏,症状为手足活动不便且多痛,心腹胀满;
如果病位在皮肤,表现为怕冷,遇寒冷则肌肉拘急,遇热则松弛,对应肺脏,症状为皮肤知觉丧失,气逆喘促胸满;

Pulse Diagnosis
A pulse that is choppy (Se) and tight (Jin) indicates Bi syndrome (Obstruction Syndrome). A Shaoyin (Kidney channel) pulse that is floating (Fu) and weak (Ruo) indicates blood deficiency due to weakness, and wind pathogen due to floating. When wind and blood contend with each other, it causes pulling pain. The combination of wind, cold, and dampness pathogens forms Bi syndrome; when the pulse presents as floating, choppy, and tight, all three pulse characteristics are present, confirming the diagnosis.
Disease Manifestations
Bi syndrome refers to pain in the hands and feet accompanied by numbness and lack of sensation. This arises primarily from internal deficiency of primordial essence and blood, allowing wind, cold, and dampness pathogens to invade. If these pathogens cannot be dispersed promptly, they flow into the meridians and enter the interior to form Bi syndrome.
In terms of symptoms: excessive cold causes pulling pain; excessive wind causes wandering pain; excessive dampness causes heaviness and stagnation.
If the pathology is in the sinews, there is inability to flex and extend, corresponding to the Liver. Symptoms include frequent startles during sleep, reduced appetite, and frequent urination.
If the pathology is in the vessels, blood coagulates and fails to circulate, corresponding to the Heart. Symptoms include sallow complexion, palpitations or throbbing in the epigastrium, rebellious Qi causing asthma and dyspnea, dry throat, and frequent belching.
If the pathology is in the bones, there is heaviness and inability to lift the body, corresponding to the Kidneys. Symptoms include paralysis or difficulty moving the limbs with much pain, and abdominal distension and fullness.
If the pathology is in the skin, there is aversion to cold; muscles contract upon exposure to cold and relax upon exposure to heat, corresponding to the Lungs. Symptoms include loss of cutaneous sensation, rebellious Qi causing asthma and chest fullness.
If the pathology is in the muscles, there is numbness and lack of sensation, corresponding to the Spleen. Symptoms include flaccidity and lassitude of the four limbs, coughing, and vomiting. This is called the "Five Bis."
Regarding Baihu Lijie Feng (White Tiger Joint Pain), it is named so because the pain wanders and the joints of the four limbs feel as if bitten by a tiger. Its etiology is none other than the invasion of wind, cold, and dampness. Drinking alcohol while exposed to wind, or entering water while sweating, can also cause this disease. If prolonged and untreated, it leads to joint dislocation and deformity.
Zhu Danxi stated: Generally, this occurs due to blood deficiency with internal heat, where the blood is already boiling. If one wades through water or is exposed to dampness, the blood becomes turbid and stagnant due to cold, failing to circulate properly, thus causing pain. Pain worsens at night because the pathogen travels through the Yin aspects. Treatment should employ acrid-hot herbs to disperse cold and dampness, open up stagnation, and restore smooth Qi and Blood circulation for recovery. Patients must observe dietary restrictions and moderate sexual desire, avoiding meat, as meat is Yang in nature and strongly aids fire. With such regulation, recovery is assured.
General Treatment Principles
The primary formula uses Cangzhu (Atractylodis Rhizoma), Nanting (Arisaematis Rhizoma Praeparatum), Chuanxiong (Ligustici Chuanxiong Rhizoma), Baizhi (Angelicae Dahuricae Radix), Danggui (Angelicae Sinensis Radix), Huangqin (Scutellariae Radix), and Jiu (Alcohol). For upper body conditions attributed to Wind, add Qianghuo (Notopterygii Rhizoma et Radix), Guizhi (Cinnamomi Ramulus), Jiegeng (Platycodonis Radix), and Weilingxian (Sinomenii Caulis). For lower body conditions attributed to Dampness, add Mutong (Akebiae Caulis), Niuxi (Achyranthis Bidentatae Radix), Fangji (Stephaniae Tetrandrae Radix), and Huangbai (Phellodendri Cortex).
Formulas
Jiebiao Shengma Tang (Release the Exterior Decoction with Cimicifugae): Treats high fever throughout the body and bone-joint pain.
Ingredients: Shengma (Cimicifugae Rhizoma, 1 sheng), Qianghuo (Notopterygii Rhizoma et Radix, 1 qian), Cangzhu (Atractylodis Rhizoma, 1 qian), Fangfeng (Saposhnikoviae Radix, 8 fen), Chaihu (Bupleuri Radix, 7 fen), Gancao (Glycyrrhizae Radix et Rhizoma, 7 fen), Danggui (Angelicae Sinensis Radix, 5 fen), Bencao/Luoben (Ligustici Rhizoma, 5 fen), Chenpi (Citri Reticulatae Pericarpium, 3 fen), Mahuang (Ephedrae Herba, 3 fen).
Preparation and Administration: Coarsely crush the ingredients for one dose. Decoct with fresh ginger and scallion white water. Take hot. Aim for a slight sweat.
Lingxian Chutong Yin (Dispel Pain Drink with Sinomenium) [Commentary: A formula for dispelling pain and inducing sweating]: Treats swelling and pain in limbs and joints. Pain is attributed to Fire, swelling to Dampness. It develops from concurrent wind-cold invasion, moving within the meridians, with damp-heat flowing into the joints incessantly.
Ingredients: Mahuang (Ephedrae Herba), Chishao (Paeoniae Radix Rubra, 1 qian each), Fangfeng (Saposhnikoviae Radix), Jingjie (Perillae Folium), Qianghuo (Notopterygii Rhizoma et Radix), Duhuo (Angelicae Pubescentis Radix), Baizhi (Angelicae Dahuricae Radix), Cangzhu (Atractylodis Rhizoma), Weilingxian (Sinomenii Caulis), Pian Huangqin (Scutellariae Radix, sliced), Zhishi (Aurantii Fructus Immaturus), Jiegeng (Platycodonis Radix), Gegen (Puerariae Lobatae Radix), Chuanxiong (Ligustici Chuanxiong Rhizoma, 5 fen each), Guiwei (Tail of Angelicae Sinensis Radix), Shengma (Cimicifugae Rhizoma), Gancao (Glycyrrhizae Radix et Rhizoma, equal parts).
Preparation and Administration: Coarsely crush for one dose. Decoct in water and take. For lower jiao involvement, add Huangbai (Phellodendri Cortex) stir-fried with wine. For women, add Honghua (Carthami Flos). For severe swelling, add more Binglang (Arecae Semen), Dafupi (Tsaoko Pericarpium), and Zexie (Alismatis Rhizoma), plus Moyao (Olibanum, 1 qian) to stop pain. Alternatively, if the pulse is choppy and rapid, indicating blood stasis, use Taoren (Persicae Semen), Honghua (Carthami Flos), Chuanxiong (Ligustici Chuanxiong Rhizoma), Danggui (Angelicae Sinensis Radix), and Dahuang (Rhei Radix et Rhizoma) to mildly purge.
Shujin Huoxue Tang (Unbind Sinews and Invigorate Blood Decoction) [Made by Yunlin]: Treats wandering stabbing pain throughout the body like needle pricks, especially severe in the left foot. The left side corresponds to Blood. This is often caused by damage from alcohol and sexual excess, leading to empty sinews and vessels invaded internally by wind, cold, damp, and heat. Heat wrapping cold causes pain; injury to meridians causes worsening at night. This formula unbinds sinews, invigorates blood, and moves dampness. This is not Baihu Lijie Feng.
Ingredients: Chuanxiong (Ligustici Chuanxiong Rhizoma, 6 fen), Danggui (Angelicae Sinensis Radix, 1.2 qian, washed with wine), Baishao (Paeoniae Radix Alba, 2.5 qian, washed with wine), Shengdihuang (Rehmanniae Radix Recens, 1.5 qian, washed with wine), Qianghuo (Notopterygii Rhizoma et Radix, 6 fen), Baifuling (Poria, 7 fen, peeled), Cangzhu (Atractylodis Rhizoma, 1 qian, soaked in rice wash water and stir-fried), Taoren (Persicae Semen, 1 qian, stir-fried), Niuxi (Achyranthis Bidentatae Radix, 2 qian, stir-fried with wine), Hanfangji (Stephaniae Tetrandrae Radix, 6 fen), Chenpi (Citri Reticulatae Pericarpium, 1 qian, pith removed), Baizhi (Angelicae Dahuricae Radix, 6 fen), Longdancao (Gentianae Radix, 8 fen, washed with wine), Weilingxian (Sinomenii Caulis, 1 qian, washed with wine), Fangfeng (Saposhnikoviae Radix, 6 fen), Gancao (Glycyrrhizae Radix et Rhizoma, 4 fen, honey-fried).
Modifications: Add Nanting (Arisaematis Rhizoma Praeparatum) and Banxia (Pinelliae Rhizoma Praeparatum, 1 qian each); decoct with ginger juice, Baifan (Alumen), and Zaorou (Stemonae Radix) and soak for one day. For upper body and arm pain, add Bo Gui (Thin Cinnamon Twig, 3 fen). For lower body and foot pain due to wind-cold-damp-heat, add Mugua (Chaenomelis Fructus), Mutong (Akebiae Caulis salt-stir-fried), Huangbai (Phellodendri Cortex salt-stir-fried), and Yiyiren (Coicis Semen stir-fried, 1 qian each). For Qi deficiency, add Renshen (Ginseng Radix et Rhizoma), Baizhu (Atractylodis Macrocephalae Rhizoma), and Guiban (Testudinis Carapax et Plastrum, 7 fen each).
Tongjing Miaoling Wan (Unblock Meridians Marvelous Spirit Pill) [Made by Yunlin]: Treats the same as above, additionally treating pain in the upper, middle, and lower jiao.
Ingredients: Huanglian (Coptidis Rhizoma, wine-stir-fried, 1 liang), Cangzhu (Atractylodis Rhizoma, rice wash-soaked and stir-fried, 2 liang), Huangbai (Phellodendri Cortex, salt-wine-stir-fried, 2 liang), Rougui (Cinnamomi Cortex, bark removed, 4 liang), Nanxiong (Chuanxiong, 5 fen), Danggui (Angelicae Sinensis Radix, wine-washed, 1 liang), Baishao (Paeoniae Radix Alba, salt-wine-stir-fried, 1.3 qian), Hanfangji (Stephaniae Tetrandrae Radix, wine-washed, 3 qian), Baizhi (Angelicae Dahuricae Radix, 2.5 qian), Taoren (Persicae Semen, peel and tip removed, 3 qian), Weilingxian (Sinomenii Caulis, 1 liang, wine-soaked, steamed, and sun-dried nine times), Qianghuo (Notopterygii Rhizoma et Radix, wine-washed, 3 qian), Longdancao (Gentianae Radix, wine-washed, 1 qian), Honghua (Carthami Flos, wine-washed, 5 qian), Fangfeng (Saposhnikoviae Radix, wine-washed, 5 qian), Guiban (Testudinis Carapax et Plastrum,酥-roasted, 5 qian), Duzhong (Eucommiae Cortex, ginger juice-stir-fried, 8 qian).
Preparation and Administration: Grind into fine powder. Use wine paste to make pills the size of Sophora seeds. Take 100 pills per dose on an empty stomach with aged wine. Salt water is also acceptable.
