This is due to the decline of the Fire of the Gate of Life (Mingmen Huo), with deficiency-cold in the Spleen Earth manifesting as false heat. However, phlegm arises because the Spleen's deficiency fails to control and return fluids to their origin. Treatment should employ Ba Wei Wan (Eight-Ingredient Pill with Rehmannia) to tonify fire to generate earth, and Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction) to simultaneously tonify Lung Metal, thereby strengthening the Spleen and Stomach.
A woman suffered from dysentery in May, with no limit to frequency day and night, accompanied by a dragging pain in the lower abdomen, fever, and aversion to cold. She was treated with Liu Jun Zi Tang (Six-Gentleman Decoction) administered with Xiang Lian Wan (Fragrant and Coptis Pill); after two doses, she gradually improved. Subsequently, the previous decoction was used to administer Si Shen Wan (Four Spirit Pill); after four doses, she recovered completely. By the end of July, she experienced lethargy and somnolence, flaccidity of the four limbs, heaviness of the body and joint pain, dryness of the mouth and tongue, loss of taste for food, loose stools, frequent urination, chills resembling the sensation of cold water being sprinkled on her, and a despondent mood. This indicates deficiency-cold of the Lung and Spleen-Stomach, resulting in the failure of Yang Qi to extend. She was cured using Sheng Yang Yi Wei Tang (Raise the Yang and Tonify the Stomach Decoction).
