A Confucian scholar presented with a sallow complexion, acid regurgitation, and belching with a putrid odor. After strictly administering Qi-regulating and phlegm-resolving medicines, he developed loose stools, poor appetite, and fatigue. This is a syndrome of Spleen and Stomach deficiency-cold. He was treated with Liujunzi Tang (Six Gentlemen Decoction) added with Paojiang (Fried Ginger Rhizome) and Muxiang (Aucklandiae Radix), leading to gradual improvement; subsequently, Sishen Wan (Four Spirits Pill) was added, restoring his Yuanqi (Primordial Qi). For such cases complicated by deficiency of Zhongqi (Middle Qi), Renshen Lizhong Tang (Ginseng Regulate the Middle Decoction) or Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) added with Muxiang (Aucklandiae Radix) and Ganjiang (Dried Ginger Rhizome) may be used. If ineffective, Zuojin Wan (Left Metal Pill) or Yueju Wan (Regulate All Pills) may be administered. If there is severe deficiency-cold of the Middle Qi, Fuzi (Prepared Aconite Lateral Root) must be added, or Fuzi Lizhong Tang (Aconite Regulate the Middle Decoction) should be used; these methods are invariably effective.
An official titled "Shangshe" vomited phlegm and saliva, accompanied by fever and thirst, and chest/epigastric fullness. The practitioner employed Qingqi Huatan Jianghuo (Clearing Qi, Resolving Phlegm, and Descending Fire) methods, which exacerbated the previous symptoms, causing spontaneous discharge of phlegm and saliva. I stated: Vomiting phlegm and saliva indicates Deficiency-Cold of Stomach Qi; fever and thirst result from the Stomach's inability to generate body fluids; chest and epigastric fullness stem from Spleen Qi deficiency. One should use herbs such as Renshen (Ginseng Radix et Rhizoma), Baizhu (Atractylodis Macrocephalae Rhizoma), Danggui (Angelicae Sinensis Radix), and Fuling (Poria), to warm and tonify the Spleen and Stomach, and generate Yang Qi, whereby all diseases will recede. The patient did not believe this, and all deficiency syndromes manifested. I further stated: Inability to ingest food, incessant hiccups, diarrhea, abdominal pain, and cold extremities constitute the "Five Deficiencies"; irritability, heat, and thirst indicate the floating outward of Deficient Yang; a洪大 (Huge and Rapid) pulse that feels empty upon deep pressure signifies imminent death. This proved true.
The mother of Lu Chenhu suffered from chronic epigastric and abdominal distension and pain for a long time. Each episode was accompanied by chest fullness, cold extremities, flushed face, numb lips, vomiting, dry throat and tongue, alternating chills and fever, and a洪大 (Huge and Rapid) pulse. The public treated it as Phlegm-Fire, achieving temporary relief but frequent recurrence. By spring, the fever became frequent and severe, and medication aggravated the condition. Some wished to use Renshen (Ginseng Radix et Rhizoma) and Baizhu (Atractylodis Macrocephalae Rhizoma) but doubted that pain could be treated with tonification, so they invited me to mediate. I stated: This is due to cold-cooling medicines damaging True Yang, presenting as Internal True Cold and External False Heat. Furthermore, the pulse is Xian Hong (Wiry and Huge) with abnormal characteristics, indicating Spleen Qi deficiency with Liver Qi overacting upon the Spleen. Only by warming and tonifying the Stomach can it be cured. Thus, Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) was prescribed with Banxia (Pinelliae Rhizoma), Fuling (Poria), Wuzhuyu (Evodiae Fructus), and Muxiang (Aucklandiae Radix). One dose yielded efficacy. Having been restless night after night for over a month since onset, she slept soundly until dawn; the洪大 (Huge and Rapid) pulse immediately restrained, and all symptoms resolved.
A woman over twenty years old had not taken solid food for two years, consuming only clear tea and fruits daily. Her face was slightly yellow and puffy, her body shape remained normal, and she could still walk, though she felt fatigued. Her Liver and Spleen pulses were Xian Fu (Wiry and Floating), feeling weak and obstructed upon deep pressure. I used Liujunzi Tang (Six Gentlemen Decoction), Muxiang (Aucklandiae Radix), and Wuzhuyu (Evodiae Fructus). She discharged a large amount of phlegm accumulation, her appetite improved instantly, and her body began to emaciate. After bed rest for over a month, continued administration of Liujunzi Tang (Six Gentlemen Decoction)-like formulas led to recovery.
