Whenever roundworms (Ascaris) move up and down and are expelled, it belongs to the Jueyin (Terminal Yin) channel invading the Yangming (Yang Brightness) channel. Food entering the stomach causes vomiting of phlegm-drool and turbid foam. This is analogous to the case described in the Jueyin chapter of Zhang Zhongjing's *Shanghan Lun* (Treatise on Cold Damage), where cold extremities precede fever. Let us discuss: after the alternating cold and heat, there is no resolution through sweating. This indicates that the latent pathogen hidden in the Supreme Yin has penetrated deeply; how can it bypass the other channels to reach the Yang aspect? Reviewing previous medical prescriptions, initially treating the Lung and Stomach, then using Wendan Fuling Decoction (Warm Gallbladder and Poria Decoction) merely harmonized the Stomach and treated phlegm, failing to expel the pathogen deeply latent in the Jueyin channel. Subsequently, Xie Xin Tang (Drain the Epigastrium Decoction) was administered. Bitter flavors can remove dampness, and pungent flavors can unblock stagnation, but the pathology remained in the Upper and Middle Jiao. After ingestion, the chest felt slightly relieved, and after a while, slight sleep occurred. Upon waking, turbid phlegm was vomited, exhibiting yellow-black characteristics. Generally, colors with green-black hues indicate reversal surging out from the bottom of the Stomach or intestines. In the elderly, the Chong Mai (Penetrating Vessel) qi has declined. As stated, "When the Chong Mai moves, all vessels reverse." The patient reported that during vomiting, the whole body was pulled and painful, extending to the soles of the feet. The Yin and Yang Qiao Mai (Slipping Vessels) and Wei Mai (Linking Vessels) could not maintain their integrity, which is certain. Zhang Zhongjing used Chai Hu (Bupleuri Radix) and Huang Qin (Scutellariae Radix) for pathogens at the half-exterior and half-interior level. Now, with obstruction between upper and lower parts, one should adopt the method of Guizhi Huanglian Tang (Cinnamon Twig and Scutellaria Decoction), incorporating Jueyin channel-guiding herbs as agents to regulate Qi flow. This allows the Chong Mai to relax, followed by tonifying and unblocking the Yangming channel. This constitutes the principle of treating Jueyin disorders.

After malaria onset, sweating occurred, indicating that the pathogen in the Yin aspect has emerged into the Yang channels. Although phlegm and vomiting have not decreased, the absence of green color in the vomitus is most gratifying. The white coating on the center of the tongue is not yet cleared, while the edges are gradually turning red. Spirit is fatigued and exhausted. Clearing pathogens while assisting healthy Qi is a established method.

The pulse is deep and wiry. Abdominal pain and vomiting occur. The nose appears black like coal, the tongue is crimson, and the complexion is dark and dull. Summer-autumn latent summerheat caused fever. It was not winter, but rather the error of using exterior-releasing methods and fasting, injuring Stomach Qi. This led to Liver Wood overacting on Stomach Earth, causing roundworms to ascend and be expelled, resulting in severe illness. There is constant concern about impending Jue Ni (Cold Extremities). The proposed treatment is to drain the Liver and harmonize the Stomach, hoping for cessation of pain and reduction of vomiting, anticipating a turn for the better.
