Cough caused by autumn coolness and dryness qi. Initially, the skin and hair felt chilly. Hoarseness occurred in winter and persisted until summer without resolution, although appetite remained quite good. It is presumed that repeated injuries from sudden cold and warmth are the cause, rather than a deficiency of the two qi (Yin and Yang). Treatment is modeled after the principle of "Metal Solid No Sound" (Hoarseness due to excess pathogenic factors obstructing the lungs).
Ephedrae Herba (Mahuang), Armeniacae Semen Amarum (Xingren), Gypsum Fibrosum (Shigao), Glycyrrhizae Radix et Rhizoma Preparata with Honey (Sheng Gancao), Belamcandae Rhizoma (Shegan), Coicis Semen (Yiren)
Consumptive disease leading to panting and hoarseness is entirely due to weakness of the lower origin (Kidney), where true qi fails to ascend and nourish the lungs. Frequent use of clearing heat and treating the lung has led to reduced appetite and loose stools. Switching to hot herbs would further deplete body fluids, hence the throat pain and mental fatigue. The method should be to tonify the Three Yin (Liver, Spleen, Kidney).
Rehmanniae Radix Preparata (Shudi), Schisandrae Fructus (Wuweizi), Dioscoreae Rhizoma Stir-fried (Chao Shanyao), Euryales Semen (Qianshi), Poria (Fuling), Nelumbinis Plumula Jianlianrou)
The Lung resembles an empty suspension; qi obstruction prevents sound production. The tongue is the sprout of the Heart; heat scorching causes the root of the tongue to stiffen and fail to extend. Treating the lips and mouth as diseases of the Lung family, which are actually obstructions from acrid-hot wine toxins, requires treatment with light and ascending herbs. This is a disease without substantial pathology.
Antelope Horn (Lingyangjiao), Forsythiae Flos Cor (Lianqiao Xin), Bambusae Folium Cor (Zhuyexin), Vigna umbellata Seed Coat (Ye Chidoupi), Fritillariae Thunbergii Bulbus (Chuan Beimu), Lonicerae Japonicae Flos (Jinyinhua), Weixi Wan)
Ulceration with pus discharge for years, fine pulse, and sallow complexion. Hoarse voice, reduced food intake, cough with sensation of obstruction and pain in the throat, all indicate leakage and damage of fatty fluids, with Yin failing to hold internally and Yang failing to guard externally. Pulmonary萎 (Fei Wei - Lung Atrophy/Withering) is unlikely to fully recover.
Ginseng Radix (Renshen), Astragali Radix (Huangqi), Coicis Semen (Yiren), Glycyrrhizae Radix et Rhizoma Preparated with Honey (Zhi Cao), Angelicae Sinensis Radix (Guishen), Bletillae Rhizoma (Baiji)
Sudden expectoration of saliva and foam in pulmonary atrophy, inability to swallow food, and no thirst for drinking. Could this be excess fire? Body fluids are exhausted, and urination and defecation decrease daily. Following Zhang Zhongjing's method of sweet-warm regulation of the stomach, since deficiency requires tonifying the mother (Earth generating Metal), accompanied by herbs to unblock stagnation in the epigastrium.
Ginseng Radix (Renshen), Ophiopogonis Tuber (Maidong), Pinelliae Rhizoma Preparated (Shu Banxia), Glycyrrhizae Radix et Rhizoma Preparated with Honey (Sheng Gancao), Oryza Sativa Var. Glutinosa Grain (Bai Jingsmi), Jujubae Fructus Flesh (Nanzao Rou)
Laborious management leads to flaccidity and expansion of Yang qi. Even if cough is caused by winter-warmth external pathogens, it is due to qi leakage and pathogen invasion. Acrid herbs disperse pathogens, bitter herbs direct rebellion downward, hoping to stop the cough. However, the Lung desires acridity; excessive acridity scatters healthy qi, causing voice to fail to project, complexion to fade, expectoration of saliva, and throat bi. This indicates difficult-to-treat pulmonary atrophy. Modeled after the Neijing principle that excessive acrid flavor requires sweet-moderating treatment.
Glehniae Radix (Beishashen), Ophiopogonis Tuber Stir-fried (Chao Maidong), Saccharum Indicum (Yitang), Jujubae Fructus (Nanzao)
For coughs caused by external heat entering the Lung, exterior-dispersing herbs can be used. If internal injury affects the Lung causing cough, it must be treated from the perspective of internal injury. Sweating will leak Yang qi. Low voice in pulmonary atrophy clearly indicates medication error.
Astragali Radix (Huangqi), Polygonati Odorati Rhizoma (Huangjing), Ziziphi Spinosae Semen (Zaoren), Bletillae Rhizoma (Baiji)
The patient stated staying in Baigou River, Zhili, last May. The North differs from the South; humid steaming brings heat, while night sitting remains cool. Presumably, alternating cold and heat then was also a mild pathogen. The physician used Guntan Wan to purge and drain, causing exterior pathogens to become closed and unable to exit. Pulmonary atrophy resulted in hoarseness, collapsed throat, and difficulty swallowing. Modeled after Xu Zhicai's "Light can remove solidity," using herbs with aroma but no strong taste.
Belamcandae Rhizoma (Shegan), Glycyrrhizae Radix et Rhizoma Preparated with Honey (Sheng Gancao), Arctii Fructus (Dali Zi), Ephedrae Herba Prepared with Honey (Mahuang Zhi), Bombyx Batryticatus (Chanyi), Talcum Granulosum with Skin (Hulun Huashi), Armeniacae Semen Amarum with Skin (Lianpi Xingren)
