Intestinal wind with diarrhea and bleeding, if the blood originates from the Spleen, use Nongpo Wan (Thick Bark Pill). If Heart Fire invades the Spleen and the blood originates from the Heart, use Guipi Tang (Restore the Spleen Decoction). Due to damp-heat caused by alcohol consumption, use Huangqin Tang (Scutellaria Decoction) plus Chuan Huanglian Wan (Coptis Pill). Due to internal injury and overwork, use Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) plus Diyü (Sanguisorbae Radix). For Yin knot causing lower bleeding, where the amount gradually increases and abdominal pain persists, use Diyü Tang (Sanguisorba Decoction). For long-standing wind entering the interior, use Qinjiao Wan (Gentian Pill). For chronic intestinal wind, use Shiquan Dabu Tang (Perfect Ten Tonifying Decoction). True intestinal wind is wind entering the interior transforming into fire; Diyü (Sanguisorbae Radix) and Huaihua (Sophorae Flos) are appropriate herbs. For persistent bleeding from intestinal wind, decoct Baizhi (Angelicae Dahuricae Radix) and Wumei (Mume Fructus) for oral administration, because the Liver stores blood and the Spleen controls blood.
Nongpo Wan (Thick Bark Pill)
Ingredients: Houpo (Magnoliae Officinalis Cortex), Shengjiang (Zingiberis Rhizoma Recens) (each 4 liang, pounded together and stir-fried until scorched yellow); Baizhu (Atractylodis Macrocephalae Rhizoma), Shenqu (Massa Medicata Fermentata), Maiya (Hordei Fructus Germinatus) (each 1 liang). Grind all ingredients into fine powder, make pills with rice paste, and take 100 pills on an empty stomach with rice soup. It also treats bleeding from five types of hemorrhoids, preventing recurrence forever.
Diyu Tang (Sanguisorba Decoction)
Ingredients: Diyü (Sanguisorbae Radix) (4 liang); Sharen (Amomi Fructus) (7 pods, stir-fried); Gancao (Glycyrrhizae Radix et Rhizoma) (3 liang, half raw, half honey-fried). Grind into powder, take 5 qian per dose, decoct in water and drink.
Qinjiao Wan (Gentian Pill)
Ingredients: Chuanxiong (Ligustici Chuanxiong Rhizoma) (2 liang); Baishao (Paeoniae Radix Alba) (4 liang); Guishen (Angelicae Sinensis Radix) (4 liang); Xiangfu (Cyperi Rhizoma) (4 liang, vinegar-fried); Qinjiao (Gentianae Macrophyllae Radix) (4 liang); Huaihua (Sophorae Flos) (4 liang, stir-fried). Make pills with honey and take.
Clinical Case Record
One patient suffered from intestinal wind with persistent bleeding, dizziness, and vertigo, lasting three to four years without cure; others claimed it was incurable. I palpated his pulse: the left pulse was deep and fine, while the right pulse was wide and large. This was due to excessive internal exposure to cold and coolness, causing Yang to fail to mobilize, hence the deep and fine left pulse; blood failing to return to its vessels and floating fire within, hence the wide and large right pulse. I prescribed Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) for ten doses, followed by Jingjie (Schizonepetae Spica) 4 liang and Chuanwu (Aconiti Radix) 1 liang, made into pills with vinegar-paste, taken on an empty stomach, resulting in a cure.
A forty-eight-year-old woman contracted dysentery in August. The cooling and digestive-promoting medicines she took injured her Spleen and Stomach, causing blood to lose its control. She passed several bowls of stool daily, sometimes stopping for one or two days, but upon any trigger, bleeding would resume uncontrollably. By October, her muscles had wasted away. To tonify blood was difficult due to cold Spleen and Stomach; to guide blood back to its vessels was difficult due to blood exhaustion. Only warming and nourishing the Middle Qi was suitable, following the principle that when Yang grows, Yin grows. I used Lizhong Tang (Regulate the Middle Decoction) for one or two doses, followed by Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) plus Fangfeng (Saposhnikoviae Radix) 3 fen and Paojiang (Zingiberis Rhizoma Preparatum) 8 fen.
Another patient passed enough blood in stool to fill a basin, fainting immediately upon bleeding, caused by dietary irregularities and overwork. In cases of blood loss requiring Qi tonification, use Renshen (Ginseng Radix et Rhizoma), Paojiang (Zingiberis Rhizoma Preparatum), Huangqi (Astragali Radix) each 1 qian, and Gancao (Glycyrrhizae Radix et Rhizoma) 7 fen. Add Baishao (Paeoniae Radix Alba) if abdominal distension occurs. Decoct in water and drink. Afterwards, regulate with Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction).
