(Twelve Syndromes Illustrated)
Nei Zhong Suo Hou Feng (Internal Swelling Locking the Throat Wind)
This syndrome is caused by the binding of Yin and Yang Qi in the Lung and Stomach meridians, resulting in internal obstruction without visible external signs, accompanied by phlegm-rattling and wheezing in the throat. Initially, apply the expectorant method (see Men of Chan Hou Feng) for induced vomiting. Subsequently, administer Liu Wei Tang (Six-Ingredient Decoction) with additions: Ephedrae Herba (Mahuang, 麻黄) (2 qian), Raw Rhizoma et Rhizoma Rhei (Sheng Dahuang, 生大黄) (5 qian), Asari Radix (Xixin, 细辛) (1 qian), Perillae Folium (Suye, 苏叶) (2 qian), Cinnamon Twig (Guizhi, 桂枝) (1 qian), and Notopterygii Rhizoma et Radix (Qianghuo, 羌活) (2 qian). Boil briefly and ingest; it is optimal if this induces diarrhea or vomiting. If neither occurs, needle ten acupoints on both hands: Shaoshang (LU 11), Shangyang (LI 1), Guanchong (SJ 1), Quchi (LI 11), and Hegu (LI 4). The presence of blood indicates a favorable prognosis, while its absence indicates a poor prognosis. A pulse that is wiry, tight, surging, or large at the Cun and Guan positions on both left and right sides indicates a favorable prognosis, whereas a deep and slow pulse suggests difficult treatment. Finally, insufflate Xionghuang (Realgar) phlegm-resolving powder.
Chan Hou Feng (Throat-Wrapping Wind)
Chan Hou Feng arises when the Lungs are affected by seasonal pathogens, causing wind-phlegm to surge upward and Yin-Yang to become closed and bound, leading to blockage of internal and external Qi flow. Symptoms resemble a snake wrapping around the head, with congestion below the glottis; severe cases present with opisthotonos (body bent backward like a bow) and trismus (lockjaw). Initially, use Kaiguan San (Opening Gate Powder): Spicaria Spinula (Zaojiaoci, 皂角刺) (1 qian), Asari Radix (Xixin, 细辛) (5 fen), and Borneolum Syntheticum (Bingpian, 冰片) (2 fen), ground into fine powder and insufflated into the nasal cavity. Then, needle the Jiache acupoint (ST 6) on both sides and moxibustion applied several times (zhuang); once the jaw opens, administer an emetic mixture of egg white and Alum solution, or induce vomiting using tung oil-soaked goose feathers, or employ the Chalcanthite (Danfan, 胆矾) method. The goal is successful emesis. If vomiting does not occur, needle five acupoints on the fingertips (Shaoshang [LU 11], Shangyang [LI 1], Guanchong [SJ 1], Shaoxin [HT 8], and Shaochong [HT 9]) until blood appears; lack of blood indicates difficult treatment. Subsequently, administer Liu Wei Tang (Six-Ingredient Decoction) with added Raw Rhizoma et Rhizoma Rhei (Sheng Dahuang, 生大黄) (1 liang), Ephedrae Herba (Mahuang, 麻黄) (2 qian), Notopterygii Rhizoma et Radix (Qianghuo, 羌活) (2 qian), Perillae Folium (Suye, 苏叶) (2 qian), and Fructus Chebulae (Hezi, 诃子) (2 qian). Boil briefly and ingest; inducing diarrhea or vomiting is highly effective. If there is no vomiting or diarrhea, no blood upon needling, and the six pulses are deep and thin, the condition is incurable. Finally, insufflate Chalcanthite (Danfan, 胆矾) phlegm-resolving powder.
