In the eighth month of the Ren Shen year, Fan Weiran suffered from an exogenous disease for more than ten days, and all physicians were at a loss. His younger brother, Li Men, earnestly requested Meng Ying to treat him. Upon examination, he observed shortness of breath, faint voice, hiccups, spontaneous sweating, and immediate regurgitation of any water swallowed. Meng Ying stated: "This is latent summerheat in the Lung; it must have been exacerbated by the use of warm-dispersing methods. Since the Lung Qi is diseased and its regulatory function fails, the Qi of the entire body loses its mechanism of descending smoothly. Even if the essence of fluids is distributed throughout, it relies on the authority of clear and descending functions to govern. If Qi rebels and rushes upward, fluids will also overflow upward. By clearing the Lung, all ailments will naturally settle." He then reviewed the previously prescribed formulas. Initially, Chai Hu (Bupleuri Radix), Ge Gen (Puerariae Lobatae Radix), Qiang Huo (Notopterygii Rhizoma et Radix), and Fang Feng (Saposhnikoviae Radix) were used to lift and raise, allowing fire to borrow the power of wind, resulting in incessant vomiting. Yet, it was still claimed that there was cold in the Stomach. Guizhi Tang (Cinnamon Twig Decoction) and Gan Jiang (Zingiberis Rhizoma) were then changed to for warming and drying, adding oil to the fire, causing Lung Yin to be nearly exhausted. Naturally, this led to shortness of breath and faint voice, leading to suspicion of Yang deficiency impending collapse. Directly, Renshen (Ginseng Radix), Dang Gui (Angelicae Sinensis Radix), Gejie (Gecko), Shidi (Diospyri Fructus Calyx), and Dingxiang (Caryophylli Flos) were given to tonify and grasp Qi. The more they tonified, the more rebellious the Qi became, and the more the pathogen failed to exit; recovery would be difficult. Hastily discarding the previous medicines, Xie Bai San (White Powder to Drain the Lungs) combined with Qing Zao Jiu Fei Tang (Clear Dryness and Rescue the Lung Decoction) was used. After several doses, the condition stabilized.
Zhuang Banxia, the third son of Zhijie Zhonghan, contracted an exogenous disease after taking the imperial examinations (Wen). He experienced chills and fever seven or eight times daily, with coma and faint spirit, and subtle rashes appearing on the skin. Physicians had no strategy, so Meng Ying was invited. Meng Ying said: "This person usually drinks alcohol, accumulating deep heat. Combined with an exogenous factor, it erupted. It should have been treated with clearing and resolving methods. Misuse of warm tonics caused the heat to rage thus." Inquiry revealed that he had taken Renshen during all three examination sessions and carried dates soaked in liquor into the exam hall. The illness was not severe initially but worsened due to continuous administration of Qiang Huo, Fang Feng, Gan Jiang, and Guizhi. Meng Ying first administered Bai Hu Tang (White Tiger Decoction) for three doses. Modified, the chills and fever gradually ceased. Subsequently, bitter-cold herbs were used to drain the heat accumulation. The black stools passed had the odor of dates. Ten days later, sweet-moistening methods were employed, and full recovery took two months.
Chen Zhitian, the pharmacist at Qiliu Suo, contracted an exogenous disease in mid-summer. Physicians administered warm-dispersing herbs, delaying treatment for ten days until he suffered from coma, delirium, limb convulsions, deafness, black tongue, scorched lips, retracted scrotum, and scanty urination. The chest showed subtle pain. The danger was obvious upon sight. Meng Ying was consulted. The pulse was fine, rapid, and hurried. Meng Ying said: "Yin deficiency with blazing heat; fluids are about to dry up." He prescribed a large dose of Xiyangshen (American Ginseng Radix et Cortex), Xuanshen (Scrophulariae Radix), Shengdi Huang (Rehmanniae Radix Preparata), Dongqing (Asparagi Radix), Tian Dong (Ophiopogonis Radix), Zhimu (Anemarrhenae Rhizoma), Huangbai (Phellodendri Cortex), Lianshi (Meliae Fructus), Shihu (Dendrobii Caulis), Baishao (Paeoniae Radix Alba), Gancao Gao (Glycyrrhizae Radix et Rhizoma tip), Yinhua (Lonicerae Japonicae Flos), Mutong (Akebiae Caulis), Shuijiao (Rhinocerotis Cornu - *Note: Now prohibited, historically used*), and Shichangpu (Acori Tatarinowii Rhizoma).
On the second day, his family reported that for seven or eight days, urine was merely drops, but after six to seven hours of taking the medicine yesterday, half a cup was voided. Meng Ying said: "This is the turning point. However, with depleted Yin, sweet-cool moistening herbs are never too many." He added Guiban (Testudinis Carapax et Plastrum), Biejia (Trionycis Carapax), Baihe (Lilii Bulbus), and Huafen (Trichosanthis Radix) to the previous formula, decocted in a large pot, and administered frequently without stopping. After eight days of this regimen, the spirit cleared, and all symptoms subsided. Pure Yin-nourishing herbs were used for one month until recovery. I consider Meng Ying’s knowledge superior and his compassionate heart unique. In dangerous cases, he never easily gives up, working diligently despite resentment. In this case, if another physician had treated it, even with correct methods, lack of immediate improvement in one or two doses might lead to avoidance, confusing the family and seeking advice from bystanders. Though not dying from the disease, they might die from the medicine. This requires the physician's seasoned judgment and firm resolve, as well as the patient's family's wise trust. How difficult is this? Meng Ying once said: "In warm-heat diseases where fluids are depleted and spirit is confused, some patients require over ten doses of Xijiao Dihuang Tang (Rhino Horn and Rehmannia Decoction) before spirit clears and fluids return." Due to the vast number of such cases, detailed records are omitted here to inform those responsible for lives.
Yao Lu jie was in Jinling when a flood occurred, then returned to Hangzhou. On the way, suffering from intense heat, he contracted an exogenous disease. Gu diagnosed it as dampness and gave Guizhi Tang and Ge Gen for three doses, worsening the condition. Zhao Dilou recognized the misdiagnosis and continuously used clearing and resolving methods, but the patient refused due to [blue dye/color issue]. Meng Ying was invited. The pulse was fine and rapid, and the body was emaciated, indicating chronic Yin deficiency. Heat pathogens, aided by wind herbs, raged, and Nutrient fluids dried up due to warm-dry herbs. The complexion turned cyanotic, indicating extreme danger. A large dose of Shigao (Gypsum Fibrosum), Zhimu, Baiwei (Cynanchi Atrati Radix), Zhizi (Gardeniae Fructus), Qinghao (Artemisiae Annuae Herba), Danpi (Moutan Cortex), Zhuye (Bambusae Folium), Zhuli (Bambusae Caulis in Taenia), and Tongxiao (Infant Urine) was administered, along with Shenxi Dan (Divine Calamity Pill). After three doses, stool passed like glue and lacquer, complexion improved, but confusion, enuresis, and extreme thirst persisted. The same formula was continued with Zixue (Purple Snow), and after several doses, heat receded, spirit cleared, but speech was disordered, resembling sleep-talking. Fear of epilepsy arose. Meng Ying said: "Phlegm remains in the Pericardium." He prescribed Shuijiao, Changpu, Xuanshen, Biejia, Huafen, Zhuru (Bambusae Caulis in Taenia), Huanglian (Coptidis Rhizoma), Shengdi, Mutong, and Gancao, mixed with Zhenzhu (Pearl) and Niuhuang (Calculus Bovis). Gradually, stability was achieved. Yin-preserving methods were adopted for recovery.
Guan Ying'an suffered from chills and fever. Physicians adhered to the theory of the Year's Si Tian and Zai Quan, administering warm-dry herbs, leading to persistent high fever. Ruan used Xiao Chai Hu Tang (Minor Bupleurum Decoction) for harmonization, resulting in spontaneous sweating, coma, black coating, stiff tongue, limb twitching, muteness, calloused lips, and scorched teeth. Zhang said "the rash did not erupt" and proposed Zao Jiao Ci (Gleditsiae Sinensis Spina), Jingjie (Schizonepetae Herba), and Bangzi (Arctii Fructus). Another doctor diagnosed "Grid Yang False Heat" and wanted to use Fuzi (Aconiti Lateralis Radix Preparata) to guide fire back to its source. Xu Zhixing diagnosed latent summerheat, but the family suspected loose stools meant cooling herbs were contraindicated. Meng Ying was consulted again. He said: "In Yin-deficient constitutions, heat pathogens easily consume fluids. Fortunately, the loose stool provides an outlet for the pathogen. This is the opportunity to guide it out. Do not hesitate." He consulted with Xu Zhixing to prescribe Wang Jin San's modified Xijiao Dihuang Tang with Zhimu, Maidong (Ophiopogonis Radix), Huafen, Xiyangshen, Xuanshen, Beimu (Fritillariae Cirrhosae Bulbus), and Shihu. Large doses were taken for eight or nine days before a turning point appeared. Sweet-cool fluid-replenishing herbs were given for six or seven doses. Suddenly, profuse sweating occurred overnight. Everyone suspected collapse. Meng Ying said: "This is Yin recovery and pathogen resolution. Do not panic." Eventually, appetite returned, and tonics led to recovery.
Chen Jurén's son had a milder case but feared Shuijiao and refused it, leading to death. How pitiful!
Shi Songxi contracted an exogenous disease at the end of summer. Multiple physicians and wide-ranging medications increased the severity over a month. Meng Ying was finally consulted. Pulse: Right Cun and Guan were slippery and rapid, overflowing upwards; Left hand was wiry and rapid. Symptoms: Deafness, bitter taste, worse heat at night, chest oppression, frequent vomiting of sticky saliva, throat obstruction when drinking, loose stools, dark urine, occasional delirium. Meng Ying said: "Summerheat and heat remain in the Lung throughout, without transmitting through meridians. One dose of Bai Hu Tang could cure this. Why the delay?" Shi's father, Beiya, showed previous prescriptions. Meng Ying reviewed them, noting only Gu Tingquan's initial clearing of Lung and Wei was correct. Others, though respectable formulas, missed the pathology. Beiya hesitated to let Shi take Meng Ying's prescription containing Shigao due to loose stools. On the second visit, Beiya asked for another method. Meng Ying said: "My method is most appropriate. Your objection is due to Shigao's cold nature. Medicine should fit the disease. For this disease, no other method is more suitable. If you seek compromise, your son's condition will suffer." Beiya was moved but hesitant. The patient saw the prescription, noted Shigao, and said: "I feel a ball of cold air in my chest; even hot water must be sipped. How can this cold herb be taken?" He firmly refused. However, respecting Meng Ying, he invited him again. Meng Ying explained: "Pathogen in Lung meridian fails to descend, condensing fluids into phlegm-saliva, obstructing the chest, blocking Qi movement. The feeling of cold is natural. I judged non-transmission initially, hence the current state. If it transmitted to Pericardium, black tongue and coma would occur, requiring Xijiao Dihuang Tang. Although not transmitting, prolonged heat depletes fluids. Do not doubt Bai Hu Tang; administer it urgently." The patient agreed. Someone warned of a relative who died after taking Shigao. Beiya remained confused and scheduled a consultation with scholars. When Meng Ying arrived, many were present, and Beiya was praying. Meng Ying wrote: "The disease has lingered, medicines ineffective. The master is confused. I avoided Bai Hu Tang thrice but now prescribe it to cure the disease. There is a medicine for every disease. Do not offer conflicting opinions. Ancient texts say 'Stuffy nose treated via Heart, Deafness treated via Lung.' Lung heat transferring to Large Intestine causes dysentery-like symptoms. This is Bai Hu Tang's domain. Why doubt Shaoyang or suspect cold? Take it boldly." Gu Tingquan supported Meng Ying. The prescription was Bai Hu Tang plus Xiyangshen, Beimu, Huafen, Huangqin (Scutellariae Radix), Ziwan (Asteri Radix), Xingren (Armeniacae Semen Amarum), Dongguaren (Benincasae Semen), Pipaye (Loquat Leaf), Zhuye, Zhuru, and Zhuhuang (Bambusae Caulis in Taenia powder). After one dose, throat opened; after three, symptoms resolved, and congee was tolerated. Sweet-moistening methods followed for recovery. This case demonstrates not only effective treatment but insights surpassing ancients.
Liu Lianfang, a scholar from Changzhou, contracted summerheat in West Lake and stayed at Cui Zhongqian's residence. Treatment failed critically. Zhuang Zhijie invited Meng Ying. The patient was naked, comatose, manic, with black tongue, extreme thirst, dark urine, constipation, and rapid, hollow pulse. Modified Xijiao Dihuang Tang was given. Spirit cleared, and he could eat congee. Next day, he answered questions well, showing signs of life. Cool-sweet methods were continued with caution. Days later, Zhuang Banxia visited with Meng Ying. The patient had staring eyes, exposed teeth, scorched lips, panting, sweating, flailing limbs—irreversible. Crowd blamed cold herbs. Meng Ying said: "You do not know whether cool or hot is indicated. You cannot see the pulse details. I disdain arguing. Eyes open, lips scorched are visible; the cause of death is evident." He left. Banxia apologized. Meng Ying said: "Vulgar views need not concern me. Truth will emerge. I feel no remorse. But this person and disease are pitiable." Later, it was learned that someone advocated hot herbs, causing failure. Two years later, Cui Zhongqian fell ill. Meng Ying predicted doom, knowing Yin deficiency with summerheat-dampness, fearing the treating physician wouldn't learn from past mistakes. Indeed, he took warm tonics extensively, exhausting True Yin and dying. Tragic repetition.
Yao Xuejiao's mother, over sixty, suffered from exogenous disease for two months. Physicians gave up. Meng Ying was consulted. She could not turn, swallow water, speak, or pass urine/stool. Meng Ying said: "Heat lingers, fluids are exhausted. At onset, it was peak summer heat. Did they just tonify warmth, seeing only deficiency and ignoring the disease?" Previous prescriptions showed only Gu Tingquan's initial light-clearing of upper Jiao summerheat was correct. Others, though potentially life-saving, did not match the pathology. Ruan's Xiao Chai Hu Tang was taken most. Physicians clung to "harmonizing," avoiding sweating, vomiting, purging risks. Family liked the combination of Renshen and Chai Hu for tonifying and expelling. However, harmonizing Shaoyang transmission formulas cannot apply to all meridians/qi diseases. Renshen and Chai Hu lifted heat upward, disrupting Lung regulation. Ginger and Jujube warmed and clogged Middle Jiao, hindering transformation. Qi stagnation increased, fluids dried. Harmonizing soups worsened disharmony. Now, even good methods fail as the throat accepts only drops. Qi knot and fluid exhaustion reached extremes. Heroes have no place to act. Yao's brothers begged for rescue. Meng Ying prescribed sweet-cool moistening herbs, instructing frequent small spoonfuls regardless of time or amount, to seep down. Only a small cup per day was possible, showing critical severity. After ten days of continuous administration, Qi flow resumed, allowing half a dose. Observers doubted the slow progress, but Meng Ying remained calm. Gradual improvement occurred. Congee intake increased. Constipation persisted for fifty days. Meng Ying said: "Do not fear. Water flows, channel forms. With food and fluids restored, bowel movement will resume. Rushing to purge an empty stomach yields nothing." Late September, urge to defecate appeared. Tonifying Qi and Blood herbs failed to induce movement. Adding one dung beetle (Qianglang) to the hot decoction resulted in immediate passage. Shockingly, fifty days without bowel movement. Follow-up with mild tonics ensured recovery.
Zhao Tieshan's son, Zishan, husband of Kang Kanghou's daughter, suffered depression, chills, and fever. Tang treated for Blood deficiency with Dang Gui, Chuanxiong, Danshen, etc., without effect. Moved to Kang's residence, Meng Ying consulted. Pulse: Slippery, deep, wiry, rapid. Tongue: Coating-less. Mouth: Not thirsty. Urine/Stool: Normal. Appetite: Slightly reduced. Sensation: Fullness/blockage in left hypochondrium and lower abdomen, tender to touch/massage. Meng Ying said: "Yin deficiency with stagnation, latent summerheat inside. Stagnation blocks Qi movement, preventing summerheat release. Sudden blood-invigorating herbs drew pathogen deeper. Blood occupied by pathogen, circulation blocked. Hypochondriac discomfort is the truth. Though disease involves Blood, clearing Qi is priority. Once Qi spreads, heat manifests, stasis moves, and disease heals." Zishan, witnessing his father-in-law's previous illness cured by Meng Ying, trusted him. Clearing Qi herbs were given. Heat intensified, delirium, insomnia, dry mouth, phlegm cough appeared. Meng Ying said: "Pulse changed to wiry-slippery. Stasis and latent pathogen show signs of exiting. Next, cool herbs to clear stasis." Bystanders objected, fearing heavy cooling. Gu Tingquan was invited and agreed. Modified Xijiao Dihuang Tang was decided. Relatives Chen Meisheng, Xu Xiaoqin, and brother Zimian doubted heavy cooling, fearing "ice-stagnation." Gu explained repeatedly without success. Suddenly, severe epistaxis occurred. Meng Ying smiled: "True organ signs obtained. Can doubts be dispelled?" Uncle Chen Guyin said: "Diagnosis is ambiguous, hard to decide. With such bleeding, pathology is revealed. No need to discuss further. Brew and drink."
Next day, bleeding recurred, coating turned black. Meng Ying said: "Three days without stool, stasis-heat cannot descend." Added Huashi (Talcum), Tao Ren (Persicae Semen), Mutong, Haijinsha (Lygodii Spora), Zhuli, Shihu, Yinhua, Zhimu, Huafen. After two doses, stool passed, black as glue. Over three days, over seventy evacuations. Mutong and Tao Ren removed; Xiyangshen and Maidong added to generate fluids. Patient extremely fatigued, slept deeply for three days. Others worried. Meng Ying said: "Let it be. Yin recovery is a good sign." Upon waking, slight heat and delirium persisted. Same method continued. Ten days later, one black dry stool passed, symptoms resolved. Thirst remained; large dose of sweet-cool herbs moistened. Ten days later, stool normalized, color improved. Yin-nourishing filling therapy ensured health.
Sun Weizhong contracted exogenous disease with deafness. Physicians stuck to Shaoyang/Xiao Chai Hu Tang pattern, worsening deafness. Meng Ying diagnosed latent summerheat, unrelated to Cold Damage treatment. Prescribed Lung-clearing herbs. Deafness decreased, condition stabilized. Just as recovery plan was made, deafness returned suddenly. Meng Ying puzzled, asked about diet. Patient ate lotus root starch yesterday. Meng Ying said: "Market lotus root starch is rarely pure, often mixed with other starches. This is equivalent to mistakenly taking Xiao Chai Hu Tang." Prescribed Lung-Stomach regulating herbs. Recovered soon. Recorded to show meticulous diagnosis.
Chen Maocai from Shangyu suffered headache, occurring every three days with chills. Many medicines failed, appetite decreased. Some suggested large doses of Jiang and Fu; others mineral drugs. Ge Zhongxin referred to Meng Ying. Pulse: Wiry, slippery on deep pressure. Meng Ying said: "Heat-summerheat deeply entered Jueyin. Warm tonics are weapons against you." Prescribed Zuojin Wan (Left Metal Pill) modified with Chuanlian (Meliae Toosendan Fructus), Baishao, Shanzhi (Gardeniae Fructus), Sangye (Mori Folium), Lingyangjiao (Capris Cornu), Danpi, Juhua (Chrysanthemi Flos), Juye (Citri Reticulatae Folium), plus Danggui Longhui Wan (Angelica and Gardenia Pill). Three doses reduced symptoms; recovered in ten days.
Gu Zhaoyun contracted exogenous disease in late autumn. Physicians treated as deficiency, tonifying for ten days. Result: curled tongue, spasms, paralysis below waist, imminent death. Relative Shi Songxi invited Meng Ying for a desperate measure. Pulse: Deficient, hurried, near extinction. First, Zixue (Purple Snow) one qian was灌 (instilled), followed by two large doses of Xijiao Dihuang Tang. Convulsions stopped, but tongue covered in black coating, eyes red as dove's, still using previous decoction. In three days, approx. two liang of Shuijiao consumed. Black coating faded, spirit cleared. Frequent hiccups remained, suspected deficiency. Meng Ying said: "Nutrient heat resolved, but Qi pathway not肃 (clear/calm)." Prescribed Shuijiao, Xuanshen, Shihu, Lianqiao (Forsythiae Fructus), Yinhua, Zhuru, Zhimu, Huafen, Beimu, Zhuye. Next day, large amount of black glue-like stool passed, hiccups stopped. Three more doses, four episodes of black stool. Tongue moistened, rice water tolerated, legs slightly movable.
