论曰经言心无正痛，真心痛即难治。小儿心痛者，心包络脉受邪也。包络者，心之别脉

治小儿蛔虫攻心腹痛。楝根汤方

上三味，以水三盏，煎取一盏，去滓空心顿服。以药盏合之，即泻虫出，仰之即吐虫出

论述说：医经中讲，心脏本身没有真正的疼痛症状，所谓的“真心痛”是难以治愈的危重症。小儿出现的心痛症状，是因为心包络脉受到邪气侵袭所致。心包络是心的别行脉络，当邪气客居其中时，会导致厥逆之气上冲，造成气机痞塞而不能消散，因此引发心痛。
治疗小儿因冷邪攻冲引起的心痛，伴有食欲不振的症状。桃仁汤方：

治疗小儿因冷气导致的心痛，不思饮食。槟榔散方：
槟榔（锉碎，三分） 当归（切细后焙干，一两） 莪术（煨制后锉碎，三分） 吴茱萸（热水清洗后焙干，微炒，一分） 阿魏（一分，用面粉包裹煨制，直到面粉熟透为止，去掉面粉） 木香（三分）
以上六味药，捣碎过筛制成细散剂。一至两岁的幼儿每次服用半钱匕的量，用温酒调服，不限时间。
治疗小儿心痛剧烈难忍。荜茇散方：

以上六味药，先将吴茱萸用米醋煮一二十沸后，挖一个地坑，将吴茱萸放入坑内。用半秤炭火烧红地坑，加入半盏米醋和吴茱萸进入坑内，用瓷碗盖住，四周用灰土封实，防止漏气，待冷却后取出，与前几味药一起捣碎过筛成粉末。加入麝香混合均匀，用醋煮面糊调和做成丸药，大小如黄米粒。一至两岁幼儿每次服用五丸，三四岁幼儿每次服用七丸，用温酒送服，不限时间。
治疗小儿心痛剧烈难忍。桃仁汤方：
桃仁（去皮尖、双仁，用麸皮炒后研碎） 赤芍药 桔梗（炒制） 桂枝（去除粗皮） 甘草（炙制，各半两）
以上五味药，将除桃仁外的四味药捣碎过筛成粉末。与桃仁一同研磨均匀，每次服用一钱匕的量，加水一盏，煎煮至剩余半盏，滤去药渣温服，根据患儿大小酌情增减，不限时间。
治疗小儿心痛。木香汤方：
木香 高良姜（炒制，各一分） 白术 桔梗（炒制） 白茯苓（去除黑皮，各半两）
以上五味药，捣碎并筛成粗末。一至两岁幼儿每次服用一钱匕的量，加水一盏，煎煮至剩余五分之一的药液，滤去药渣，分两次温服，发作时服用，并根据患儿大小酌情增减。
治疗小儿心痛。紫沉消积丸方：
沉香（锉碎） 砂（各四两） 阿魏 巴豆霜（各一两，以上各单独研为细末，用蜂蜜一斤、酒化开熬成膏状） 丹砂（研碎） 丁香 干姜（炮制，各二两） 硫黄（研碎） 青橘皮（热水浸泡后去白瓤焙干） 高良姜 槟榔（锉碎） 木香 人参 胡椒 桂枝（去除粗皮，各四两）
以上十五味药，除去前面的膏剂外，其余十一味药捣研成细末，加入前面的膏剂中调和捣匀，做成如绿豆大小的丸药，用瓷盒密封保存。每次服用两丸，饭前用橘皮汤送服，并根据情况酌情增减。
治疗小儿因积气和冷气引起的心痛。 砂丸方：
砂（二钱） 干漆（微炒） 五灵脂 胡椒 桂枝（去除粗皮） 京三棱（炮制锉碎） 莪术（炮制，各一分） 巴豆（二十一枚，去皮心膜，用醋煮出油后研碎） 斑蝥（二十一枚，去翅足，炒制）
以上九味药，捣碎过筛成粉末。研磨均匀，用醋煮面糊调和做成丸药，大小如黄米粒。一至两岁幼儿每次服用五丸，三四岁幼儿每次服用七丸，用橘皮汤送服，其余剂量依情况加减。
治疗小儿呕吐逆气，心腹疼痛，帮助消化水谷，消除积聚。沉香煎丸方：
沉香（锉碎） 丁香 木香 胡椒 没药（研碎） 丹砂（研碎，水飞法） 高良姜 槟榔（面粉包裹煨熟后去面锉碎） 砂（拣选干净，水飞法，慢火熬干） 青橘皮（热水浸泡后去白瓤焙干） 硫黄（研碎，水飞法，各一两） 缩砂（去皮） 吴茱萸（热水浸泡后焙干） 阿魏（醋浸后研碎，澄清去沙，和面做成饼子炙焦，各半两） 巴豆（去皮心膜，研出尽油，一分）
以上十五味药，分别捣研成粉末并混合均匀。炼蜜做成如绿豆大小的丸药，用瓷盒密封保存。每次服用两丸，饭前临睡前用生姜橘皮汤送服，根据虚实和年龄大小酌情增减。
治疗小儿心痛。胡椒丸方：
胡椒（四十九粒） 槟榔（一枚，锉碎） 斑蝥（七枚，去翅足）
以上三味药，捣碎过筛成细末，用烧熟的粟米饭做粘合剂制成丸药，大小如黄米粒。一至两岁幼儿每次服用三丸，三岁以上幼儿每次服用五丸，用煎好的菖蒲汤送服。
治疗小儿心痛。姜黄散方：
姜黄 槟榔（锉碎）
以上两味药等分，捣碎过筛成散剂，用温酒调服，一至两岁幼儿每次服用半钱匕以下，其余剂量依情况加减。
治疗小儿蛔虫攻冲引起的心腹疼痛。楝根汤方：
楝根皮（有果实的） 酸石榴根 槐根（各一握，切碎，选用向东生长的根部）
以上三味药，用水三盏，煎取一盏药液，滤去药渣，空腹顿服。用药杯扣住容器口，虫子就会泻出；如果倒扣容器，虫子就会吐出，效果神奇，幼儿需减少剂量服用。
治疗小儿疳积虫不时咬噬引起的心痛，日夜无法入睡。人参汤方：
人参 桂枝（去除粗皮） 桃柳枝（锉碎焙干） 野狼牙 乳香（研碎） 青橘皮（去白瓤焙干炒制） 吴茱萸（热水浸泡后焙干炒制，各一分） 古老钱（四文，火烧后淬入醋中）
以上八味药，捣碎并筛成粗末。每次服用一钱匕的量，加水六分，煎煮至剩余四分，滤去药渣温服，根据患儿大小酌情增减。
Commentary states: Medical classics mention that the Heart itself does not have true pain; "Zhenxin Tong" (True Heart Pain) is difficult to treat. Pediatric Xin Tong (Heart Pain) results from pathogenic factors invading the Pericardium Collateral (Pericardium Meridian of Hand-Jueyin). The Pericardium is an alternate meridian of the Heart. When pathogenic factors lodge therein, it causes rebellious Qi to ascend, leading to stagnation and obstruction that cannot dissipate, thus manifesting as Xin Tong (Heart Pain).
Treatment for pediatric Xin Tong (Heart Pain) caused by cold pathogenic factors attacking the heart, accompanied by loss of appetite. Tao Ren Tang (Peach Kernel Decoction):
Tao Ren (Peach Seed, Semen Persicae) - 37 seeds, soaked in hot water, skin and pointed end removed, double kernels discarded, stir-fried until yellow. Houpo (Magnolia Bark, Cortex Magnoliae Officinalis) - coarse skin removed, processed with ginger juice, 1 liang. Renshen (Ginseng Root, Radix Ginseng) - 0.5 liang. Chenjupi (Aged Tangerine Peel, Pericarpium Citri Reticulatae) - soaked in hot water, white pith removed, dried, 1 qian. Maiya (Sprouted Hordeum, Fructus Hordei Germinatus) - lightly stir-fried, 0.5 liang. Binglang (Areca Nut, Semen Arecae) - crushed, 0.5 liang. Fuzi (Prepared Aconite Root, Radix Aconiti Lateralis Preparata) - processed until cracked, skin and navel removed. Guizhi (Cinnamon Twig, Ramulus Cinnamomi) - coarse skin removed. Danggui (Angelica Sinensis Root, Radix Angelicae Sinensis) - sliced and dried, each 1 liang.
The above nine herbs are coarsely crushed and sifted. For infants aged one to two years, administer half a qianbi (measure) per dose. Add half a zhan (measure) of water, decoct until one-third remains, filter dregs, and take warm at any time. Adjust dosage according to the child's condition.
Treatment for pediatric Xin Tong (Heart Pain) due to cold qi, with no desire to eat. Binglang San (Areca Nut Powder):
Binglang (Areca Nut, Semen Arecae) - crushed, 3 qian. Danggui (Angelica Sinensis Root, Radix Angelicae Sinensis) - sliced and dried, 1 liang. Ezhu (Curcuma Rhizome, Rhizoma Curcumae) - baked and crushed, 3 qian. Wuzhuyu (Evodia Fruit, Fructus Evodiae) - washed with hot water, dried, lightly stir-fried, 1 qian. Aswei (Asafoetida, Resina Ferulae) - 1 qian, wrapped in flour and baked until flour is cooked, remove flour. Muxiang (Costus Root, Radix Aucklandiae) - 3 qian.
The above six herbs are pounded into a fine powder. For infants aged one to two years, administer half a qianbi per dose, mixed with warm wine, at any time.
Treatment for unbearable pediatric Xin Tong (Heart Pain). Bibai San (Long Pepper Powder):
Bibai (Long Pepper Fruit, Fructus Piperis Longi) - 1 qian. Hujiao (Pepper Fruit, Fructus Piperis Nigris) - 1 qian. Guizhi (Cinnamon Twig, Ramulus Cinnamomi) - coarse skin removed, 1 qian. Tao Ren (Peach Seed, Semen Persicae) - soaked in hot water, skin and pointed end removed, double kernels discarded, stir-fried until yellow, 0.5 liang. Muxiang (Costus Root, Radix Aucklandiae) - 0.5 liang. Danggui (Angelica Sinensis Root, Radix Angelicae Sinensis) - sliced and dried, 3 qian.
The above six herbs are pounded into a fine powder. For infants aged one to two years, administer half a qianbi per dose, mixed with warm wine, at any time.
Treatment for pediatric Xin Tong (Heart Pain) with cyan-yellow complexion and loss of appetite. Shexiang Wan (Musk Pill):
Shexiang (Musk, Moschus) - finely ground, 1 qian. Binglang (Areca Nut, Semen Arecae) - crushed. Chenjupi (Aged Tangerine Peel, Pericarpium Citri Reticulatae) - soaked in hot water, white skin removed. Roudoukou (Nutmeg, Semen Myristicae) - shell removed. Wuzhuyu (Evodia Fruit, Fructus Evodiae) - washed with hot water, dried, stir-fried. Muxiang (Costus Root, Radix Aucklandiae) - each 1 liang.
For the above six herbs, first boil Wuzhuyu (Evodia Fruit, Fructus Evodiae) in rice vinegar for twenty boils. Dig a pit, place the Wuzhuyu inside. Heat half a sheng of charcoal until red-hot, add half a zhan of rice vinegar and the Wuzhuyu into the pit. Cover with a porcelain bowl, seal around with ash to prevent gas leakage, wait until cool, then remove. Mix with the other herbs, pound into powder. Add Shexiang (Musk, Moschus), mix evenly, bind with vinegar-cooked paste into pills the size of millet grains. Infants aged one to two years take five pills; children aged three to four years take seven pills, taken with warm wine, at any time.
Treatment for unbearable pediatric Xin Tong (Heart Pain). Tao Ren Tang (Peach Kernel Decoction):
Tao Ren (Peach Seed, Semen Persicae) - skin and pointed end removed, double kernels discarded, stir-fried with bran and ground. Chishao (Red Peony Root, Radix Paeoniae Rubra). Jiegeng (Platycodon Root, Radix Platycodonis). Guizhi (Cinnamon Twig, Ramulus Cinnamomi) - coarse skin removed. Gancao (Licorice Root, Radix Glycyrrhizae) - honey-processed, each 0.5 liang.
The above five herbs: pound the four herbs (excluding Tao Ren) into powder. Grind together with Tao Ren (Peach Seed, Semen Persicae) until uniform. Take one qianbi per dose, add one zhan of water, decoct until half a zhan remains, filter dregs, and take warm. Adjust according to the child's size, at any time.
Treatment for pediatric Xin Tong (Heart Pain). Muxiang Tang (Costus Root Decoction):
Muxiang (Costus Root, Radix Aucklandiae). Gaoliangjiang (Galangal Rhizome, Rhizoma Alpiniae Officinarum) - stir-fried, each 1 qian. Baizhu (Atractylodes Macrocephala Rhizome, Rhizoma Atractylodis Macrocephalae). Jiegeng (Platycodon Root, Radix Platycodonis) - stir-fried. Baifuling (Poria Sclerotium, Polyporus) - black skin removed, each 0.5 liang.
The above five herbs are coarsely crushed and sifted. For infants aged one to two years, take one qianbi per dose, add one zhan of water, decoct until one-fifth remains, filter dregs, divide into two warm doses. Take during attacks, adjust according to the child's size.
Treatment for pediatric Xin Tong (Heart Pain). Zichen Xiaoji Wan (Purple Agarwood Accumulation-Dissolving Pill):
Chenxiang (Agarwood Wood, Lignum Aquilariae Resinatum) - crushed. Zhusha (Cinnabar, Mineralis Cinnabaris) - 4 liang each. Aswei (Asafoetida, Resina Ferulae). Badoushuang (Barium Bean Frost, Semen Crotonis Preparatum) - 1 liang each. Above ingredients are separately ground into fine powders. Use 1 jin of honey and dissolve in wine, simmer into a paste. Dansha (Cinnabar, Mineralis Cinnabaris) - ground. Dingxiang (Clove, Flos Caryophylli). Ganjiang (Dried Ginger, Rhizoma Zingiberis Preparatum) - processed, 2 liang each. Liuhuang (Sulfur, Sulfur). Qingjupi (Green Tangerine Peel, Pericarpium Citri Reticulatae Viride) - soaked in hot water, white pith removed, dried. Gaoliangjiang (Galangal Rhizome, Rhizoma Alpiniae Officinarum). Of the fifteen herbs, excluding the paste, grind the remaining eleven herbs into fine powder. Mix with the paste and knead into pills the size of mung beans. Store in a sealed porcelain container. Take two pills per dose before meals with tangerine peel decoction, adjust as needed.
Treatment for pediatric Xin Tong (Heart Pain) caused by accumulated qi and cold qi. Zhusha Wan (Cinnabar Pill):
Zhusha (Cinnabar, Mineralis Cinnabaris) - 2 qian. Ganqi (Dry Lacquer, Resina Rhus Verniciferae) - lightly stir-fried. Wulingzhi (Trogopterus Dung, Faeces Trogopterori). Hujiao (Pepper Fruit, Fructus Piperis Nigris). Guizhi (Cinnamon Twig, Ramulus Cinnamomi) - coarse skin removed. Jingsanleng (Burreed Tuber, Rhizoma Sparganii). Ezhu (Curcuma Rhizome, Rhizoma Curcumae) - processed, each 1 qian. Badou (Croton Seed, Semen Crotonis) - 21 seeds, skin, heart, and membrane removed, boiled in vinegar to extract oil, then ground. Banmao (Mylabris, Mylabris) - 21 insects, wings and feet removed, stir-fried.
The above nine herbs are pounded into powder. Grind uniformly, bind with vinegar-cooked paste into pills the size of millet grains. Infants aged one to two years take five pills; children aged three to four years take seven pills, taken with tangerine peel decoction, adjust rest as appropriate.

Binglang (Areca Nut, Semen Arecae) - wrapped in flour, baked until cooked, remove flour, crush. Zhusha (Cinnabar, Mineralis Cinnabaris) - selected clean, water-insoluble sediment, slow-fire dried. The above fifteen herbs are each pounded and ground into powder, then mixed. Bind with refined honey into pills the size of mung beans. Store in a sealed porcelain container. Take two pills per dose before meals and before sleep with ginger and tangerine peel decoction, adjust according to deficiency/excess and age.
Treatment for pediatric Xin Tong (Heart Pain). Hujiao Wan (Pepper Pill):
Hujiao (Pepper Fruit, Fructus Piperis Nigris) - 49 grains. Binglang (Areca Nut, Semen Arecae) - 1 piece, crushed. Banmao (Mylabris, Mylabris) - 7 insects, wings and feet removed.
The above three herbs are pounded into fine powder. Bind with scorched millet rice into pills the size of millet grains. Infants aged one to two years take three pills; children over three years take five pills, taken with decocted Changpu (Acorus Root, Rhizoma Acori Tatarinowii) soup.
Treatment for pediatric Xin Tong (Heart Pain). Jianghuang San (Turmeric Powder):
Jianghuang (Turmeric Rhizome, Rhizoma Curcumae Longae). Binglang (Areca Nut, Semen Arecae) - crushed.
The above two herbs in equal parts are pounded into powder. Take with warm wine. Infants aged one to two years take less than half a qianbi per dose, adjust rest as appropriate.
Treatment for pediatric abdominal pain caused by roundworms attacking the heart. Lian Gen Tang (Toona Root Decoction):
Lian Genpi (Toona Root Bark, Cortex Toosendan) - from trees bearing fruit. Suan Shiliu Gen (Pomegranate Root, Cortex Granati). Huai Gen (Chinese Turpentine Tree Root, Cortex Meliae Azedarach) - each one handful, chopped, use roots growing eastward.
The above three herbs are decocted in three zhan of water until one zhan remains. Filter dregs and take on an empty stomach in one dose. Place the medicine cup upside down over the vessel; worms will be expelled via stool. If placed right-side up, worms will be vomited out. This is highly effective. Reduce dose for small children.
Treatment for pediatric gan disease (malnutrition) where parasites bite causing heart pain, preventing sleep day and night. Renshen Tang (Ginseng Decoction):
Renshen (Ginseng Root, Radix Ginseng). Guizhi (Cinnamon Twig, Ramulus Cinnamomi) - coarse skin removed. Taoliuzhi (Peach and Willow Branches, Ramulus Pruni Persicae et Salicis) - crushed and dried. Yelangya (Wild Wolf's Tooth Herb, Herba Siegesbeckiae). Ruxiang (Olibanum, Resina Boswelliae) - ground. Qingjupi (Green Tangerine Peel, Pericarpium Citri Reticulatae Viride) - white pith removed, dried and stir-fried. Wuzhuyu (Evodia Fruit, Fructus Evodiae) - soaked in hot water, dried and stir-fried, each 1 qian. Gulaoqian (Ancient Coins, Moneta Antiqua) - four coins, heated in fire and quenched in vinegar.
The above eight herbs are coarsely crushed and sifted. Take one qianbi per dose, add 0.6 zhan of water, decoct until 0.4 zhan remains, filter dregs, and take warm. Adjust according to the child's size.
