Discourse states: Jianri Nue (Intermittent Malaria occurring every other day) occurs when pathogenic factors adhere to the Yin aspect and cannot emerge together with the Wei Qi (Defensive Qi). Since Wei Qi circulates in the Yang aspects during the day, the pathogenic factors are unable to exit; thus, they must wait for a subsequent convergence of Wei Qi before manifesting symptoms, which is why the condition arises every other day.
Treatment for Jianri Nue, characterized by initial chills followed by high fever, utilizes Changshan Tang (Crownvetch Root Decoction).
Ingredients: Hedyotisae Herba (Changshan, 3 liang, processed), Glycyrrhizae Radix et Rhizoma (Gancao, 3 liang, honey-fried), Testudinis Carapax et Plastrum (Biejia, 1 liang, vinegar-processed after removing the skirt), and Lophatheri Herba (Danzhuye, 3 sheng, cut and washed).
Preparation: The four ingredients are coarsely chopped, soaked in 3 sheng of wine, covered, and placed outdoors overnight. At dawn, add 7 sheng of water and simmer over hot water until half the liquid remains. Divide the wine-decoction into five doses. Administer before the onset of symptoms; vomiting is expected. Discontinue administration once vomiting occurs, without needing to consume the entire dose.
For treating Jianri Nue, Chun Yao Tang (Astringent Herb Decoction):
Ingredients: Rhei Radix et Rhizoma (Dahuang, 0.5 fen, crushed and stir-fried), Glycyrrhizae Radix et Rhizoma (Gancao, 0.5 fen, honey-fried), and Hedyotisae Herba (Changshan, 0.5 fen).
Preparation: Coarsely chop the three ingredients. Decoct with 3 zhan of water to obtain 1 zhan of liquid, then strain. Decoct the dregs again with 1 zhan of water to obtain 0.7 zhan. Take the first decoction before the onset of symptoms, followed immediately by the second decoction (the "astringent" medicine). This sequence cures the disease.
For treating Jianri Nue, Changshan Jiu (Crownvetch Root Wine):
Ingredients: Hedyotisae Herba (Changshan, 1 liang), Allii Sativi Bulbus (Suan, one single-clove garlic, root and stem removed, sliced horizontally), Oryzae Fructus Glutinosa (Nuomi, 100 grains), and Sojae Semen Nigrum (Heidou, 100 grains).
Preparation: Coarsely chop the four ingredients. Soak them in 1 sheng of clear wine in a bowl the day before the expected attack. Cover the bowl with white paper and place a knife horizontally across the top. When the attack is imminent, divide the liquid into three parts. Drink one part first; if no vomiting occurs, drink another part. Vomiting indicates recovery.
For treating Jianri Nue, Beijiyi Yuan San (Emergency Single-Grain Powder):
Prepare ash from burning the tail of a black cow. Take one fangcun bi (approx. 2-3 grams) with wine, three times daily.
For treating the chills and fever of Jianri Nue, Changshan Tang (Crownvetch Root Decoction):
Ingredients: Hedyotisae Herba (Changshan, 0.5 fen), Lophatheri Herba (Zhuye, 0.5 fen, finely chopped), Sorghi Fructus (Shumi, 50 grains; glutinous rice may be used instead), and Gypsum Fibrosum (Shigao, 2 liang, crushed).
Preparation: Coarsely crush and sieve the four ingredients. For each dose, take 5 qianbi, soak in 1.5 zhan of water overnight. Decoct at dawn until 1 zhan remains, strain, and take warm. Take one dose before the onset of symptoms and another just before the attack. On that day, avoid washing or eating/drinking anything other than the remaining medicinal juice, which should be applied to the palms, soles, chest, head, and face. Place the dregs near the pillow.
For treating Jianri Nue, Nuomi Zhu San (Glutinous Rice Boiled Powder):
Ingredients: Oryzae Fructus Glutinosa (Nuomi, 40 grains), Hedyotisae Herba (Changshan, chopped), Pruni Mume Carnis (Wumei Rou, fried), Lophatheri Herba (Zhuye, chopped), Glycyrrhizae Radix et Rhizoma (Gancao, 0.5 liang, honey-fried), and Gypsum Fibrosum (Shigao, 1.5 liang, crushed).
Preparation: Crush and sieve the six ingredients into a powder. For each dose, take 5 qianbi, decoct with 1.5 zhan of water until 0.7 zhan remains, strain, and take warm before the onset of symptoms.
For treating malaria occurring every other day, Dahuang Tang (Rhubarb Decoction):
Ingredients: Rhei Radix et Rhizoma (Dahuang, raw, 0.5 liang), Glycyrrhizae Radix et Rhizoma (Gancao, honey-fried and chopped), Hedyotisae Herba (Changshan, chopped), and Cinnamomi Ramulus (Gui, bark removed, 0.5 fen).
Preparation: Coarsely crush and sieve the four ingredients. For each dose, take 5 qianbi, decoct with 1.5 zhan of water until 0.7 zhan remains, strain, and take warm before the onset of symptoms.
For treating Jianri Nue, Shenwen Wan (Divine Pill):
Ingredients: Sojae Semen Nigrum (Heidou, 21 small beans, soaked in water to remove skins), Realgar (Pishuang, 1 qian, ground), and Ziziphi Jujubae Fructus (Dazao, one date, boiled, peeled, and pitted).
Preparation: These ingredients must be compounded on the Dragon Boat Festival (Duanwu) or Qixi Festival. Grind them together and form pills the size of pea seeds, coating them with Cinnabar (Dansha). Avoid exposure to women, chickens, and dogs during preparation. On the morning of the attack, face north, pass the pill over incense smoke, and swallow with cold tea. Avoid hot food or drink for one shichen (two hours).
