吴某，男，22岁，住院病案号54。
初诊日期1959年12月15日：发热恶寒二天，伴头痛、咽痛、咳嗽、胸满胸闷， 经X线检查：为右肺下叶非典型肺炎。
既往有肝炎、肺结核、肠结核史。常有胁痛、乏力、便溏、盗汗。前医先以辛 凉解表（桑叶、银花、连翘、薄荷、羌活、豆豉）一剂，服后汗出热不退，仍继用 辛凉解表，急煎服，高烧、自汗、头痛、咳嗽、胸闷、恶风、胁痛诸症加重。血常 规检查：白血球8100，中性70%。14日静脉输液用抗生素，当夜高烧仍不退，体温 39.4度，并见鼻煽、头汗出。又与麻杏石甘汤加栀子豉等，服三分之一量，至夜11 时出现惊悸、肢凉。因请胡老会诊。
胡老据：晨起体温38.2度，下午39度以上，呈往来寒热，并见口苦，咽干，目 眩、头晕、盗汗、汗出如洗、不恶寒，苔黄，舌红，脉弦细数，认为证属表已解， 连续发汗解表，大伤津液，邪传少阳阳明。治以和解少阳兼清阳明，为小柴胡加生 石膏汤方证：
柴胡五钱，黄芩三钱，半夏三钱，生姜三钱，党参三钱，大枣四枚，炙甘草二 钱，生石膏二两。
结果：上药服一剂，后半夜入睡未作寒热及盗汗。16日仍头晕、咳嗽痰多带血 。上方加生牡蛎五钱，服一剂。17日诸症消，体温正常。12月22日x线检查：肺部 阴影吸收。
在会诊时胡老特别指出：辛凉解表只是定了一个大法，并没有进一步辨清具体 的方证，因此治疗用药偏于盲目，过度解表使津液大伤，造成汗出热不退或更甚。 把肺炎的发热分为风寒、风热所致是片面的，即使得知是风寒或风热，也要看患者 所表现的症状，不论是风寒还是风热，都可能在人体产生或热或寒、或虚或实、或 表或里的症状。分析这些症状所应归属的方证，才能明确当用方药。故胡老特别强 调，中医治病辨证论治，不但要辨六经八纲、脏腑阴阳，更要辨方证，辨方证是六 经八纲辨证的继续，它既是辨证的具体实施，也是辨证的基本功。也就是说，治病 不能只是治疗大法如辛温发汗、辛凉清热、清阳明热、宣肺化痰……更重要的是要 明确对证的方药。也就是说，辨方证比治疗大法更重要。对此，历代医家早有认识 ，如方有执研究《伤寒论》曾强调“守一法，不如守一方”，即是强调辨方证。
评：1959年，己亥，土运不及，厥阴风木司天，少阳相火在泉。12月15日正当 六之气，主气太阳寒水，客气少阳相火。主胜客，是为逆。相火被郁，反上冲肺金 ，胡老以小柴胡加生石膏，枢转少阳，并清阳明郁热，又加生牡蛎引火归元，是为 正解，故二剂而愈。经方运用得当效如桴鼓，古人岂欺我哉！

接下来，有必要在这里探讨一下激素产生作用的原理。同样众所周知，运动员 服用激素之后会大大提高运动成绩，但“副作用”之大也与成绩一样惊世骇俗，很 多“健儿”倒地猝死的场面更令世人触目惊心，世界女子百米纪录保持者格里菲斯 ?乔伊纳就是一个典型的例子。其实原因很简单，激素的作用是靠瞬间调动大量元 气完成的，人体长期储备的精微物质突然转化为能量爆发，在这瞬间的爆发之中， 纪录被重新书写，疾病也暂时消失症状。
但是，人体的能量基础毕竟是有限的，长期不断地调动元气来完成人体能力无 法承担的工作，必然会造成人体能量的最终枯竭，导致元气瞬间消散。运动员的暴 毙就是这样的典型例子。
同样，非典病人在元气极度虚弱的情况下，又被激素大量调动来解救肺部的燃 眉之急，造成肾中本部空虚就不可避免了。而肾主骨髓，主藏精。肾气大衰之际自 顾不暇，自然就无法完成藏精生髓的工作了。于是，骨坏死也就不可避免地发生了 。
此论受西医启发而始，彼虽必不能明其所以，然实肇始于彼之发现，功不可没 ，特记之，亦表非盲目排斥彼学者也。然思维方法终难沟通，此亦一二不可混淆之 一证欤？
公元2002年之12月1日，西方所谓之国际艾滋病日也。电视节目中列举艾滋病 研究进展，云美国新发明一种检测方法，简单易行：但于一种溶剂中滴入一滴鲜血 ，普通人之血变成一条直线横断溶液之中，艾滋病感染者则分为两条平行直线，相 距甚远。因触动灵机，作是思维：艾滋病人与普通人之区别无他，但一分为二也。 此一二者何？请从中医之说论之：
天地未判，一气混茫；太极动转，阴阳始分。天一生水，地二生火，此河图之 言也。一者谓何？先天之阳，即天也，在卦为乾，三连者是也；二者谓何？先天之 阴，即地也，在卦为坤，六断者是也。此以三才记阴阳之象也，除之以三，不过一 二而已。一二者，即先天之阴阳也。天地动转，阴阳交媾。天一降于地户，于是水 生。水之体外阴而内阳，在卦为坎，在脏为肾，中满者是也；地二升于天门，于是 火成。火之体外阳而内阴，在卦为离，在脏为心，中虚者是也。水火者，天地阴阳 之用也。先天之阳收于水中，天一是也；先天之阴藏于火内，地二是也。一居下而 二居上，此三才得先天阴阳之常也。一二反动，则天地为否；二一顺转，则水火既 济。一二者，实万物生死离合之枢机也。
血之体为阴而用为阳，阴形之中内藏阳气，坎卦也，其根本实出于肾中。普通 常人验血一条直线者，阴阳相交，一二相合之故也。艾滋病人验血两条直线者，阴 阳离绝，一二相分之故也。又一为阳而二为阴，血成二条，坎中一阳已成阴气，坎 阳者，先天生命之本也，坎阳一散生命失根，死期将至矣。而不即死者，说明尚有 转机。转机谓何？使分者复合，散者复聚，一降二升，天覆地载，水火既济，阴阳 互根，则病可以不作矣。
然治病必求于本，本立而道生。观艾滋病传染之途径有二，二者谓何？不过精 血也。精血谓何？不过水火也。水火谓何？不过离中虚坎中满也。今其病从下而入 ，由内而生。坎中不满，是以一分为二；离中不虚，是以二化为一。表盈里亏，虚 实反作；内寒外热，冰炭斯分。此否隔离绝之象，又非艾滋病所独有者也，西医所 谓之一切免疫系统疾病莫不如此。
然治之道果何如耶？无他，益其损而复其剥而已。益者谓何？风上雷下，否上 之阳复降也。复者谓何？地上雷下，剥上之阳复降也。二卦皆雷居下，雷者谓何？ 龙雷之火生于坎中，即先天之阳也。然则治病之道不过使其将脱之阳复归水中，即 所谓导龙归海也。天一复降于水中，则生生之本复泰矣，又何病之有哉？

消渴正论
Wu, male, 22 years old, inpatient medical record number 54.
Initial consultation date December 15, 1959: Fever and aversion to cold for two days, accompanied by headache, sore throat, cough, chest fullness and oppression. X-ray examination revealed atypical pneumonia in the lower lobe of the right lung.
History of hepatitis, pulmonary tuberculosis, and intestinal tuberculosis. Commonly experiences hypochondriac pain, fatigue, loose stools, and night sweats. The previous physician first prescribed one dose of Xinliang Jiebiao (Acrid-Cool Exterior-Resolving Method) [Sangye (Folium Mori), Yinhua (Flos Lonicerae), Lianqiao (Fructus Forsythiae), Bohe (Herba Menthae), Qianghuo (Rhizoma et Radix Notopterygii), Douchi (Semen Sojae Praeparatum)]. After taking it, sweating occurred but fever did not subside. The Xinliang Jiebiao method was continued with urgent decoction and administration. High fever, spontaneous sweating, headache, cough, chest oppression, aversion to wind, and hypochondriac pain all worsened. Complete blood count examination: leukocytes 8100, neutrophils 70%. On the 14th, antibiotics were administered via intravenous infusion. That night, high fever still did not subside, body temperature reached 39.4°C, and nasal flaring and head sweating appeared. Mazheng Shigan Tang (Ephedra, Apricot Seed, Gypsum, and Licorice Decoction) plus Zhizi Chi (Gardenia and Fermented Soybean Decoction) etc. were given; after taking one-third of the dose, palpitations and cold limbs appeared at 11 PM. Therefore, Elder Hu was invited for consultation.
Based on Elder Hu's assessment: Morning body temperature 38.2°C, afternoon above 39°C, presenting as alternating chills and fever, along with bitter taste in the mouth, dry throat, dizziness of the eyes, vertigo, night sweats, profuse sweating like washing, no aversion to cold, yellow coating, red tongue, wiry-thin-rapid pulse. It was considered that the exterior syndrome had been resolved. Continuous sweating to resolve the exterior severely damaged body fluids, and the pathogen transmitted to Shaoyang (Lesser Yang) and Yangming (Yang Brightness). The treatment principle was to harmonize Shaoyang and simultaneously clear Yangming heat, corresponding to the formula pattern of Xiaochaihu Tang plus Raw Gypsum Decoction:
Chaihu (Radix Bupleuri) 5 qian, Huangqin (Radix Scutellariae) 3 qian, Banxia (Rhizoma Pinelliae) 3 qian, Shengjiang (Rhizoma Zingiberis Recens) 3 qian, Dangshen (Radix Codonopsis) 3 qian, Dazao (Fructus Jujubae) 4 pieces, Zhi Gancao (Radix Glycyrrhizae Preparata) 2 qian, Sheng Shijiao (Gypsum Fibrosum) 2 liang.
Result: After taking one dose of the above medicine, sleep was achieved in the latter half of the night without chills/fever or night sweats. On the 16th, still experiencing vertigo and cough with copious bloody sputum. Raw Muli (Concha Ostreae) 5 qian was added to the above formula, taken as one dose. On the 17th, all symptoms disappeared, and body temperature normalized. X-ray examination on December 22 showed absorption of lung shadows.
During the consultation, Elder Hu specifically pointed out: Xinliang Jiebiao (Acrid-Cool Exterior-Resolving Method) only established a general principle, without further distinguishing the specific formula-pattern, thus making medication somewhat blind. Over-resolving the exterior severely damaged body fluids, causing sweat-out fever not to subside or even worsening. Dividing pneumonia fever into causes of Fenghan (Wind-Cold) and Fengre (Wind-Heat) is one-sided. Even if knowing it is Wind-Cold or Wind-Heat, one must look at the symptoms manifested by the patient. Whether Wind-Cold or Wind-Heat, they can produce symptoms of heat or cold, deficiency or excess, exterior or interior in the human body. Analyzing which formula-pattern these symptoms belong to allows clarifying the appropriate formula and herbs. Thus, Elder Hu particularly emphasized that TCM treating disease through Syndrome Differentiation requires not only differentiating Six Channels, Eight Principles, Zang-Fu organs, and Yin-Yang, but more importantly, differentiating Formula-Patterns. Differentiating Formula-Patterns is a continuation of Six Channels and Eight Principles differentiation; it is both the concrete implementation of differentiation and the basic skill of differentiation. That is to say, treating disease cannot merely rely on general treatment principles such as Xinwen Fahan (Acrid-Warm Sweating), Xinliang Qingre (Acrid-Cool Clearing Heat), Qing Yangming Re (Clearing Yangming Heat), Xuan Fei Huatan (Ventilating Lung and Resolving Phlegm)... More importantly, one must clarify the formula and herbs matching the syndrome. That is to say, differentiating Formula-Patterns is more important than general treatment principles. Regarding this, physicians of past dynasties had early understanding. For example, Fang Youzhi, while studying Shanghan Lun (Treatise on Cold Damage Pathogens), emphasized "Guarding one method is not as good as guarding one formula," which emphasizes differentiating Formula-Patterns.
Commentary: In 1959, Ji Hai year, Earth transport insufficient, Jueyin Wind Wood rules the Heaven, Shaoyang Minister Fire rules the Earth. December 15th fell during the Sixth Qi, Main Qi Taiyang Cold Water, Guest Qi Shaoyang Minister Fire. Main overcoming Guest is adverse. Minister Fire depressed, counter-attacks upward to Lung Metal. Elder Hu used Xiaochaihu Tang plus Raw Gypsum to pivot Shaoyang and clear Yangming depressed heat, plus Raw Muli to guide fire back to its origin, which is the correct solution, hence cured in two doses. When Jingfang (Classic Formulas) are applied appropriately, efficacy is immediate like a drumstick hitting a drum; how could the ancients deceive us!
Why do SARS patients develop bone necrosis?
Why do most SARS patients develop femoral head necrosis after being "cured" using hormones? Actually, the problem lies with the hormones.
As everyone knows, the effect of hormones is very rapid and effective, but hormones have a biggest weakness: after use, the condition often recurs repeatedly, and upon recurrence, the severity is worse, while also causing damage in other aspects. Bone necrosis is undoubtedly one of the more serious damages among these.
Although SARS symptoms manifest in the lungs, weak Yuanqi (Primal Qi) in the Kidney unable to encourage Zhengqi (Upright Qi) to resist pathogens is also an important cause of disease. However, the therapeutic effect of hormones is not completed by supporting Yuanqi, but by continuously mobilizing Yuanqi.
Next, it is necessary to discuss here the principle of hormone action. Also well known is that athletes taking hormones significantly improve sports performance, but the magnitude of "side effects" is equally shocking, with many scenes of "athletes" collapsing and dying suddenly being deeply shocking to the world. Griffith-Joyner, the world women's 100-meter record holder, is a typical example. The reason is actually simple: the effect of hormones relies on instantaneously mobilizing large amounts of Yuanqi. Substances stored finely by the human body over long periods suddenly transform into energy explosion. In this instantaneous explosion, records are rewritten, and disease symptoms temporarily disappear.
However, the energy foundation of the human body is ultimately limited. Continuously mobilizing Yuanqi over long periods to complete work beyond human capacity will inevitably lead to final exhaustion of human energy, causing instantaneous dissipation of Yuanqi. The sudden death of athletes is such a typical example.
Similarly, under conditions of extreme weakness of Yuanqi, SARS patients had their Yuanqi massively mobilized by hormones to save the urgent situation in the lungs, inevitably causing emptiness in the root of the Kidney. The Kidney governs marrow and stores Essence. At the time of great decline of Kidney Qi, attending to itself is difficult enough, naturally unable to complete the work of storing Essence and generating Marrow. Thus, bone necrosis inevitably occurred.
Theory of Yin-Yang, Heaven-Earth, Water-Fire, One-Two, Separation-Union (AIDS Theory)
This theory started inspired by Western medicine. Although they certainly cannot understand its rationale, it truly originated from their discovery, merit cannot be denied. Specifically recorded here, also expressing non-blind rejection of their scholars. However, thinking methods remain difficult to communicate; this is also one proof that One and Two cannot be confused.
December 1, 2002 AD, the so-called International AIDS Day in the West. Television programs listed AIDS research progress, stating America newly invented a detection method, simple and easy: just drop a drop of fresh blood into a solvent, ordinary person's blood becomes a straight line crossing horizontally through the solution, while HIV-infected persons divide into two parallel straight lines, far apart. Touching inspiration, made this thought: The difference between AIDS patients and ordinary people is nothing else, simply dividing into two. What is this One and Two? Please discuss from TCM perspective:
Heaven and Earth undivided, one Qi chaotic; Taiji rotates, Yin-Yang begins to separate. Heaven generates Water One, Earth generates Fire Two, this is words of Hetu (River Map). What is One? Innate Yang, i.e., Heaven, in Trigram is Qian (Heaven), three solid lines; What is Two? Innate Yin, i.e., Earth, in Trigram is Kun (Earth), six broken lines. This uses Three Powers to record images of Yin-Yang, divided by three, merely One and Two. One and Two, i.e., Innate Yin-Yang. Heaven and Earth rotate, Yin-Yang couple. Heaven One descends to Earth Gate, thus Water generates. Water's body is Yin outside and Yang inside, in Trigram is Kan (Water), in Zang organ is Kidney, middle full; Earth Two ascends to Heaven Gate, thus Fire forms. Fire's body is Yang outside and Yin inside, in Trigram is Li (Fire), in Zang organ is Heart, middle empty. Water and Fire are uses of Heaven-Earth Yin-Yang. Innate Yang stored in Water, Heaven One is; Innate Yin hidden in Fire, Earth Two is. One resides below and Two resides above, this Three Powers obtain constant Innate Yin-Yang. One and Two reverse rotation, then Heaven-Earth become Pi (Standstill); Two and One smooth rotation, then Water and Fire become Ji Ji (After Completion). One and Two, truly the pivot of life and death, separation and union of all things.
Blood's body is Yin and use is Yang, within Yin form hides Yang Qi, Kan Trigram, its root truly originates from Kidney. Ordinary normal people testing blood one straight line, due to Yin-Yang intersecting, One and Two combining. AIDS patients testing blood two straight lines, due to Yin-Yang severing, One and Two separating. Again, One is Yang and Two is Yin, blood becomes two lines, Kan middle One Yang has become Yin Qi, Kan Yang is root of innate life, once Kan Yang scatters life loses root, death period approaches. And not immediately dying, indicates there is still turning point. What is turning point? Making separated recombine, scattered re-gather, One descending Two ascending, Heaven covering Earth bearing, Water and Fire Ji Ji, Yin-Yang mutually rooting, then disease can cease.
However, treating disease must seek the root, root established then Dao generates. Observing AIDS transmission routes are two, what are two? Merely Jing-Xue (Essence-Blood). What is Jing-Xue? Merely Water-Fire. What is Water-Fire? Merely Li middle empty and Kan middle full. Now its disease enters from below, arises from within. Kan middle not full, thus One divides into Two; Li middle not empty, thus Two transforms into One. Exterior full Interior deficient, Deficiency-Excess reversed; Interior cold Exterior hot, Ice and Charcoal separated. However, what is the way of treatment? Nothing else, benefiting loss and restoring Peeling. What is Yi (Benefiting)? Wind above Thunder below, Pi upper Yang returns descending. What is Fu (Restoring)? Earth above Thunder below, Peeling upper Yang returns descending. Both trigrams have Thunder residing below, what is Thunder? Dragon-Thunder Fire born in Kan middle, i.e., Innate Yang. Then the way of treating disease is merely making departing Yang return to Water, i.e., so-called Guiding Dragon Returning to Sea. Heaven One returns descending into Water, then root of generation recovers Taiping (Great Peace), what disease would there be?
