Glibenclamide, Glyburide

Diabetes mellitus is a disorder of visceral metabolic function. As is well known, sugar is the energy source of the human body, and its taste is sweet. From the perspective of Traditional Chinese Medicine (TCM) theory, the metabolism of sugar is related to the Spleen and Stomach because the Spleen and Stomach belong to Earth, and the taste of Earth is sweet. The Stomach governs the reception of food, while the Spleen governs transportation and transformation. The Middle Qi (Middle Jiao Qi) rotates, ascending the clear and descending the turbid, allowing the internal environment of the body to eliminate the old and generate the new. Therefore, TCM theory considers the Spleen and Stomach as the "Postnatal Root" (the root acquired after birth; the Prenatal Root lies in the Kidney).
Furthermore, the Spleen governs the control of blood. Since the Spleen Meridian connects with the Heart Meridian, the Spleen also manages the circulation of blood to a certain extent (Zhonghua Bubai's statement that the heart's power is insufficient to drive blood circulation is correct; actually, the primary driving force for blood circulation lies in the Kidney, transmission occurs in the Liver and Spleen, delivery is handled by the Heart, and regulation is managed by the Lung. The mechanism of blood circulation is associated with all five Zang organs). The power of the Spleen comes partly from the ascending force of the Liver and Kidney, and partly from the energy transformed from diet. The main function of the Spleen is to convert sugar from food into energy and transport it into the blood to become the body's motive force, similar to burning coal into fire, evaporating water into steam, and converting steam into power; this principle is quite similar to that of Western steam engines. Now, due to dysfunction in the Spleen's transformation ability, sugar cannot be normally converted into energy but instead remains in the blood, becoming a burden to the blood. At this time, blood tests will show high blood glucose levels, and what Western medicine calls diabetes mellitus occurs.
The Spleen originally governs the ascending of clear Qi, which means transmitting the original motive force from the Kidney and Liver to the Heart. However, now due to insufficient ascending force, it cannot push the essence of food and drink upward to transform into energy. Instead, nutrients sink into the Kidney and are excreted from the body via urine. At this point, urine tests will also show high urine sugar levels, and what Western medicine calls diabetes mellitus becomes名副其实 (literally: lives up to its name). However, limited by their theoretical framework, Western medicine cannot understand the root cause that prevents sugar from being normally converted into energy (equivalent to water turning into vapor), and thus cannot cure this disease fundamentally. Fortunately, they discovered insulin. Insulin is naturally produced during the Spleen's process of transporting and transforming essences; it is the material form of the Spleen's function. The only method Western medicine uses to treat diabetes is injecting synthetic insulin into the human body. Insulin can lower blood glucose and alleviate some symptoms in the short term. However, it still cannot cure the disease fundamentally because insulin cannot be self-generated, requiring continuous injections. Long-term injection not only causes the complete loss of normal Spleen function but also damages Liver and Kidney functions, ultimately leading to complications described by Western medicine such as gangrene of hands and feet, renal failure, blindness, and other severe syndromes, which remain incurable. Therefore, when Western medicine says diabetes is a lifelong disease and a terminal illness, it is indeed so from their perspective.
As mentioned earlier, the root cause of this disease lies in the destruction of the Spleen's function of ascending clear Qi. The reason why Spleen Qi fails to ascend is also due to the failure of Kidney Yang to ascend (equivalent to the furnace fire of an internal combustion engine being too small to ignite coal). Yang Qi governs ascending (because heat is light, it ascends), and Yin Qi governs descending (because cold is heavy, it descends); this is an obvious principle. The failure of Kidney Qi to ascend is often because Kidney Yang is injured by cold pathogens. This has theoretical basis and observable signs; combining four diagnostic methods will reveal clues. Therefore, I say that early-stage diabetes that has not affected the Middle Jiao can be cured using Jin Gui Shen Qi Wan (Golden Cabinet Kidney Qi Pill), because Shen Qi Pill is an excellent medicine for warming and tonifying Kidney Yang (igniting the fire in the furnace).
As for the severe syndromes in mid-to-late stages, they are accumulated through continuous mismedication and have already damaged the root. However, if medication is appropriate and follows the correct principles, restoration is possible, though it would take at least three to six months. If recovery is not achieved within a year, thorough restoration becomes difficult.
Under the influence of famous doctors like Shi Jinmo, current TCM practitioners generally adopt the treatment principle of nourishing Yin and descending Fire for diabetes. This further attacks the already damaged Spleen Yang and Kidney Yang, akin to adding frost to snow or compounding illness upon illness. How can such a disease be cured? Thus, for these incompetent practitioners, diabetes becomes a terminal illness, damaging the reputation of TCM and objectively harming patients even today. Alas! The disaster caused by human misunderstanding is severe enough!
Questioning the Mechanism of Action of Glibenclamide
Mechanism of Action
Pancreatic Internal Effect: Stimulates pancreatic B cells to release insulin (What does "stimulate" mean? Why don't pancreatic B cells release insulin? If a slave collapses from exhaustion, can you restore his strength merely by whipping him?). Pancreatic B cells increase sensitivity to secretagogues (such as blood glucose), thereby increasing insulin secretion (What is sensitivity? If a collapsed worker is whipped, does that increase his sensitivity to work?).
Pancreatic External Effect: Reduces the liver's clearance function of insulin, increasing blood insulin concentration (Why does the liver clear insulin? Is it because insulin itself doesn't work?).
Increases the affinity of insulin for receptors, promoting glucose uptake and utilization (How is this promoted? By whipping to increase sensitivity?).
Increases glycogen synthesis and reduces glycogen breakdown (By what means? A whip?).
Reduces platelet aggregation and viscosity, reducing the occurrence and development of vascular complications (By what means? A whip?).
Indications
Type II diabetic patients whose blood glucose cannot be satisfactorily controlled despite strict dietary management and serious exercise therapy, especially non-obese individuals (Why are obese people not suitable? Is it because thin people have more "Fire"? Do you know what "Fire" is?).
Type II diabetic patients using less than 40 units of insulin daily can switch to sulfonylurea drugs (All damage the Yuan Qi (Original Qi); different paths lead to the same destination, differing only by fifty paces!).
By the way, do you know what Yuan Qi is?
Glibenclamide (Glyburide)
Characteristics: Fast onset, strong hypoglycemic effect, 200 times that of D860, long duration, hence prone to causing hypoglycemia. Oral administration takes effect within 5-20 minutes, peaks in 2-6 hours, and lasts up to 24 hours. Excretion is 50% via the liver and 50% via the kidney. (Why does hypoglycemia occur? If blood glucose is converted into energy, why is it still excreted via the liver and kidney? This is what I want to tell you: "Correct treatment should help the body convert blood glucose into motive force, i.e., metabolizing sugar, rather than clearing seemingly increased static blood glucose through so-called hypoglycemic therapy.")
Side Effects: Hypoglycemia, especially attention needed in the elderly. Long-term use requires caution in patients with liver or kidney dysfunction.
[Note]: Xiaoke Wan consists of Liu Wei Di Huang Tang plus Glibenclamide; each 10 pills contain 2.5mg of Glibenclamide. Tangwei Capsules: Each 5 capsules contain 2.5mg of Glibenclamide (Do you know what Liu Wei Di Huang Tang is? Do you know what happens when you pour a basin of cold water on a dying fire? Why does it damage liver and kidney function? Is it the result of converting glycogen into energy? Or is it excessively stealing the already insufficient Liver Qi and Kidney Qi? It is like whipping a dying slave relentlessly to keep him working.)
The correct approach is: When a slave is about to collapse from exhaustion, he should be allowed to rest, replenish energy through diet, recover slowly, and then resume labor. Not whipping him to get up and using his remaining meager strength for work. The positive aspect of Western medicine's action is equivalent to a whip; essentially, it is similar to a juicer, vowing not to stop until the Yang Qi of the Liver and Kidney is squeezed dry, resembling the greedy and cruel nature of landlords and capitalists.
Difference Between Metabolizing Sugar and Lowering Blood Glucose
During the process of metabolizing sugar, blood glucose will not drop rapidly because the process of converting ingested sugar into glycogen and finally into motive force has been delayed. The conversion process slows down; if intake is not reduced, prolonged retention of blood glucose (and consequently higher blood glucose levels) is normal. As long as continuous conversion occurs, slightly elevated blood glucose will not cause significant negative effects or acidosis. If symptoms of weakness and fatigue diminish, it indicates that blood glucose is being converted into motive force. Conversely, if hypoglycemic drugs normalize blood glucose indicators but the patient feels weak and fatigued, it shows that blood glucose was excreted via the Liver and Kidney without being converted into motive force.
The nature of these two results is completely different: one is true weakness, the other is false health. Relatively speaking, true weakness is healthier than false health. Weakness and aging are normal metabolic phenomena. Attempting to resist aging through beautification or cosmetic surgery is impossible and only makes one look bizarre and unnatural. The so-called therapeutic effects of Western medicines are mostly of this superficial nature, lacking real therapeutic value, much like anesthetizing nerves to relieve pain—it merely covers up the truth.
Ideally, expecting elderly people's blood glucose levels to match those of young people is unrealistic. Just as it is impossible to expect elderly people to have the same reproductive capacity and appearance as youths, the blood glucose standards for diabetes itself are an unrealistic and unscientific concept.
An average value constantly changing in a dynamic process loses all meaning if detached from its dynamic background, just like using orange glass to simulate burning flames; even if the scale is close, essentially they are not the same thing. The same applies to blood glucose levels; what matters is whether the dynamic process proceeds normally, not whether static scales are similar. Like photographs recording life, they are not life itself; the two should not be confused.
On the Nature of Insulin
Insulin should be considered the material form of the function that promotes the conversion of blood glucose into energy. The key issue is whether insulin works independently during this conversion process or simultaneously with other energies. If it works independently, ensuring sufficient exogenous supply of insulin would suffice. But if insulin must rely on another energy of the body to function, then the state and capacity of this other energy are directly related to the blood glucose conversion process. If this energy lacks the capacity to participate in this conversion, insulin will not only fail to complete the task but also become a new burden to the blood. If insulin is inherently the material form of this energy—meaning its production is a product of this energy's transportation and transformation process—then without fundamentally solving the problem of energy deficiency, insulin cannot be self-generated. Exogenously supplied insulin lacks this energy. For exogenous insulin to complete the blood glucose conversion task, the body must provide another energy to facilitate this work. The current problem is that insulin production failed due to energy deficiency, yet exogenous insulin consumes this already deficient energy further. Who will ultimately provide this increasingly depleted energy?
To illustrate with an example:
A room needs heat, so a stove is lit. There is coal (energy), but it is wet and cannot be ignited. A tinder—flammable paper or dry wood—is needed. This is not enough; a spark is required. Using a torch to ignite the tinder, building the fire sufficiently strong, then adding coal. The coal dries gradually under the heat of the fire, then slowly ignites, followed by adding more coal. Thus, the furnace burns, successfully converting energy into heat, and the room becomes warm.
In this process, coal corresponds to blood glucose, tinder corresponds to insulin, and the spark corresponds to energy. Only with a spark can the tinder burn the coal. Without a spark, neither tinder nor coal can burn and convert into energy. In this process, the role of the spark is primary. If there is abundant tinder and coal but the spark is too weak to ignite the tinder, subsequent combustion cannot proceed smoothly.
The current question is: If the root cause lies in the failure of the spark, can adding tinder (supplementing insulin) or reducing coal (lowering blood glucose) solve the problem?
Regardless of the energy source, ignition is required to convert it into usable energy. The same applies to human energy. Without fire, only coal or gasoline exists, and internal combustion engines cannot work; this is a simple principle. Western medical theory consistently treats humans as machines. Does machine operation not require power? Does energy itself not need conversion to become motive force?
Differentiation of Leukemia
Leukemia is a Western medical diagnosis, and Western medicine currently claims ignorance of its etiology. However, I believe this disease is actually the result of Western medical mismanagement.
Generally, in the early stages of this disease, common cold symptoms appear. With continuous injection of Western antibiotics, the condition worsens, accompanied by persistent low-grade fever. Doctors often suspect immune system dysfunction. Further examination reveals massive proliferation of white blood cells, appearance of immature cells, and granulocyte variation. Thus, Western medicine diagnoses leukemia. After multiple rounds of radiotherapy and chemotherapy, the patient's Zheng Qi (Upright Qi) continuously weakens, eventually leading to death due to extreme depletion of Zheng Qi.
However, analyzing from TCM theory, the patient has congenital Yuan Qi deficiency and insufficient Kan Yang (Kidney Yang). After contracting Wind-Cold, the pathogen penetrates deeply into the bone marrow through simultaneous involvement of the Taiyang and Shaoyin channels. This leads to internal coagulation of Yin-Cold in Water and leakage of Warm Qi. Consequently, Cold Water fails to generate Wood, manifesting as malfunction of hematopoietic mechanisms in the bone marrow. This results in the loss of storage, control, and governance of blood functions in organs such as the Liver, Heart, and Spleen. Finally, blood flows outside the vessels due to failing to follow its proper pathways, leading to death from internal organ hemorrhage. Without the continuous damage to Zheng Qi by Western anti-inflammatory drugs, which draw the Cold pathogen deeper layer by layer, rapid death would not occur.
A vivid example illustrates this: In 2000, a young man from Harbin was diagnosed with leukemia. He resolutely refused treatment, deciding to cycle across China with his remaining life. Upon returning to Shenyang via Shenzhen and undergoing hospital checks again, a miracle occurred: his blood indices returned to normal. This fact demonstrates that, without mismanagement, so-called incurable diseases like leukemia can recover under the regulation of one's own Zheng Qi.
On the other hand, deceased patients all died after timely combined chemotherapy. Even TCM practitioners are often misled by the apparent fever, using large amounts of so-called Heat-clearing and Blood-cooling herbs, damaging Zheng Qi, achieving the same result as Western medicine through different paths.
If differentiation is accurate, leukemia is certainly not an incurable disease. Treatment involves consolidating the root and supporting Yuan Qi, guiding according to the trend, allowing deep-seated Cold pathogens to move from interior to exterior and dissipate externally, thus curing the disease spontaneously.
