The etiology of "Zhong E" (sudden attack by evil qi) refers to a condition where the patient's spirit and righteous qi are weakened, making them susceptible to a sudden invasion by ghostly or evil pathogenic factors. Generally, when the human body maintains yin-yang harmony and balances Ying (nutritive qi) and Wei (defensive qi), with the spirit remaining guarded internally, the righteous qi becomes strong, preventing pathogens from invading. However, if improper care leads to spiritual weakness, one may be afflicted by toxic ghostly qi. The symptoms manifest as sudden stabbing pain in the heart and abdomen, accompanied by severe distress and a sensation of impending death. In cases of sudden Zhong E with abdominal distension and fullness, if the pulse is tight, large, and floating, the prognosis is poor (death); if the pulse is tight, thin, and faint, the prognosis is good (survival). Additionally, if Zhong E presents with hematemesis of several sheng, a sinking, rapid, and thin pulse indicates a poor prognosis (death), whereas a floating and frantic pulse suggests survival. If residual pathogenic forces linger after recovery from Zhong E and flare up later, it may transform into "Zhu" disease (a term for lingering, contagious, or relapsing diseases).
Furthermore, critical signs of Zhong E include sudden cardiac and abdominal cramping pain leading to syncope and suffocation upon invasion by ghostly evil qi. This occurs because the exogenous pathogen is violently excessive, causing a separation of yin and yang and an obstruction of upper and lower qi mechanisms. Therefore, the *Guangji Fang* provides a prescription for treating sudden Zhong E, heart and abdominal stabbing pain, and expelling evil qi:
Moschus (1 fen, ground)
Aquilariae Lignum Resinatum (Qingmuxiang, 2 fen)
Rhinocerotis Cornu (Shengxijiao, 2 fen, crushed)
Grind these three ingredients into a powder. Take one fangcun bi (a square-inch measure) with warm water on an empty stomach. Administer twice daily; it takes effect immediately. If symptoms persist, continue administration. Observe usual dietary restrictions. (From Volume IV)
The *Zhouhou Beiji Fang* and Hua Tuo offer a prescription for Zhong E characterized by shortness of breath approaching cessation:
Moxibustion at the hair tufts behind the nails of both big toes. Apply 14 moxa cones (two sevens) to each toe for immediate cure. Alternatively, apply 21 moxa cones (three sevens). (Identical to Zhang Wenzhong's *Beiji* in *Jiyan Fang*)
Another prescription:
Semen Armeniacae Amarum (Xingren, 70 seeds, remove tips and skins)
Cinnamomi Cortex (Guixin, 2 liang)
Glycyrrhizae Radix et Rhizoma (Gancao, 1 liang, honey-fried)
Ephedrae Herba (Mahuang, 3 liang, remove joints; another version uses 4 liang)
Chop the four ingredients. Decoct in 8 sheng of water until reduced to 3 sheng. Divide into three doses. Hold in the mouth and swallow slowly. This formula effectively treats various forms of syncope and Ke Wu (fright-induced illness due to external stimuli). Observe usual dietary restrictions.
Another prescription:
Radix Allii Tuberosi (Jiu Gen, 1 bundle)
Fructus Mume (Wumei, 14 pieces)
Fructus Evodiae (Zuyu, 0.5 sheng)
Chop the three ingredients. Decoct in 1 sheng of Laoshui (water that has been boiled repeatedly or stored for a long time). Submerge the patient's comb in the decoction and boil three times. If the comb floats, the patient will survive; if it sinks, the patient will die. Decoct until 3 sheng remain, and have the patient drink it. It is highly effective.
Another prescription:
Cinnamomi Cortex (Guixin, 1 liang)
Zingiberis Rhizoma Recens (Shengjiang, 3 liang)
Gardeniae Fructus (Zhizi, 14 pieces)
Sojae Semen Praeparatum (Chi, 0.5 he)
Crush the four ingredients. Simmer gently in 2 sheng of wine. Strain the dregs and administer in a single dose. Induce vomiting as the therapeutic limit. (All from Volume I)
The *Jiyan Fang* treats Zhong E Dunshi (a wandering evil pathogen causing latent pain), characterized by pain in the heart, abdomen, and body. Severe cases involve shortness of breath and muteness. Palpation identifies the painful area; if the facial expression changes and the patient dislikes proximity to others, this confirms the painful site as the target.
Take Artemisia argyi leaves (Ai Ye), crush them, and place them on the painful area to a thickness of about one cun. Boil water in a pan, mix with ash to form a hot mud paste, and apply over the mugwort. Replace when cool. No more than two applications are needed for a cure. (From Volume VII)
Another prescription for Zhong E involving heart pain, chest and hypochondriac pain, and urgent panting:
Cortex Ramuli Persicae Orientalis (Taodongxingzhi Baipi, 1 handful)
Margarita (Zhenzhu, 1 liang)
Endocarpon Gardeniae (Zhiziren, 14 pieces)
Zingiberis Rhizoma Recens (Shengjiang, 2 liang)
Angelicae Sinensis Radix (Danggui)
Cinnamomi Cortex (Guixin) [Both 3 liang]
Aconiti Lateralis Radix Preparata (Fuzi, 1 liang, processed)
Sojae Semen Praeparatum (Xiangchi, 0.5 he)
Evodiae Fructus (Wuzhuyu, 0.5 he)
Chop the nine ingredients. Decoct in 8 sheng of water until reduced to 2 sheng. Strain the dregs, add Margarita powder, and divide into two doses. Observe usual dietary restrictions. (Identical to *Duanju Fang*)
Another prescription:
Have the patient lie supine and block both ears with an object. Insert two bamboo tubes into the nostrils of a deceased person. Have two people blow forcefully into the tubes, ensuring no air escapes from the sides around the nose. After half a day, the deceased person will emit a sigh ("yi") and revive. Stop blowing. (Identical to *Qianjin Fang*)
Another prescription:
Grind Gleditsiae Sinensis Fructus (Zaojia) and Asari Radix et Rhizoma (Xixin) into fine powder. Blow into both nostrils. Using only Zaojia powder is also effective. (Identical to *Qianjin Fang*, all from Volume IV)
The *Shanfan Fang* provides a prescription for Zhong E with excruciating pain:
Fundi Nigredo (Kufudi Mo, soot from the bottom of a cauldron, 0.5 he)
Sal (Yan, 1 pinch)
Mix and grind the two ingredients. Stir with 1 sheng of water and administer in one dose. (Identical to Fan Wang's formula)
Another prescription:
Extract juice from fresh cow dung (Niushi), 0.5 he per dose. If the mouth is clenched and cannot open, strike the teeth to create a gap for administering the medicine. If fresh dung is unavailable, use dry dung soaked in water to extract the juice. (All from Volume X)
Cui Shi's prescription for sudden Zhong E with respiratory arrest:
Grind Margarita (Zhenzhu) into powder. Write the character "Gui" (Ghost) on the tongue. Writing "Gui" on the forehead is also verified as effective.
Another prescription:
Apply moxibustion to the high bone of the right shoulder. The number of moxa cones should correspond to the patient's age (Sui Nian Zhuang). (All from Volume IV)
