Heaven and Earth encompass all phenomena, the Three Powers (Heaven, Earth, Human) penetrate the physical form. The Five Body Parts correspond to the Five Elements, and the Six Fu Organs correspond to the Six Pitch Pipes (Lü). Human flesh is governed by the Spleen; human skin is managed by the Lung. Diseases of the skin and flesh arise from improper dietary flavors and clothing warmth. If clothing is too thick and warm, it causes the exterior pores to open and invites Cold. If dietary intake is immoderate, it leads to Heat accumulation in the Zang-Fu organs. Once the Zang-Fu are congested, blood vessels fail to circulate properly. When blood circulation stagnates, toxic factors (Du Wa) accumulate and converge at the Shu points. At these Shu points, Yin and Yang energies intersect, and body fluids, due to deficiency, lie latent and guard. This inevitably boils the blood; when blood deteriorates, muscle tissue ulcerates. Rotting muscle forms pus. Solid formations are termed Carbuncle (Yong), while superficial swellings are termed Edema (Zhong). If accompanied by deficiency-Heat of the Liver and Kidney, it develops into a Gangrene (Ju) or Gan sore. Furthermore, Ju adheres to the bone, while Gan attaches to the tendons. Regarding all carbuncles and gangrenes, pulse diagnoses have specific characteristics: floating, slippery, rapid, choppy, weak, or deep. A floating pulse indicates Yang deficiency; a slippery pulse indicates Yang excess; a rapid pulse indicates Yang dryness/heat; a choppy pulse indicates Yin cold; a weak pulse indicates Yin deficiency; and a deep pulse indicates Yin solidity. These are the pulses of the Three Yang and Three Yin meridians. If among the three positions (Cun, Guan, Chi), the pulse shows a combination of one Yin and one Yang that knots together abnormally, this is a sign of carbuncle or gangrene. Moreover, according to pulse methods: the Heart pulse is洪 (Hong/Large), the Lung pulse is 浮 (Fu/Floating), the Liver pulse is 弦 (Xian/Wiry), and the Kidney pulse is 沉 (Chen/Deep). If the Lung, Liver, and Heart pulses appear simultaneously, a carbuncle or gangrene will erupt. Why is this so? Because one Yin and one Yang interact like Water and Fire. Old theories claimed that Cold and Heat lodge in the meridians, causing blood stasis and blockage; this reasoning is flawed. Treatises state that Heat originates internally and manifests externally; external Heat cannot enter internally to cause swelling. Rather, all such conditions arise because exterior deficiency allows Cold pathogens to contend with the body. Thus, wearing thick, warm clothes invites Cold, which is the underlying principle.
The occurrence of any carbuncle is self-inflicted due to four factors: first, irregular breathing; second, emotional imbalance (excessive joy or anger); third, irregular eating habits; fourth, disharmony of Yin and Yang. Violating these four leads to disharmony of the Six Fu organs and dysfunction of Ying (Nutritive) and Wei (Defensive) Qi. Ying is Blood; Wei is Qi. Cold injures Blood, causing stasis; Heat injures Qi, causing hyperactivity. Qi transforms into Fire; Blood transforms into Water. The contention between Water and Fire forms carbuncles and gangrenes. If deficiency is added, Qi rushes to the chest and heart, causing tremors and shaking of the four limbs, resembling syncope with palpitations. This is a sign that pus has not been fully discharged. Over time, this becomes a Fistula (Lou). Even if it heals, it will recur. One should take herbs for draining pus and nourishing the body to avoid disaster. Although carbuncles and gangrenes share similar names, their depth varies, and treatment depends on deficiency or excess. However, through medicinal regulation, different paths lead to the same destination.
In one's prime, the Shaoyang Qi is sparse, the skin is loose and thin, and indulgence in rich foods clogs the intestines and stomach. Congestion generates Heat, which then attracts Cold. The arrival of Cold varies in depth, presenting differently based on location. Superficial cases retain strong interior Yang; moderate cases show slight deficiency; severe cases involve exhaustion of Qi. Diseases in the Yang aspect are easy to resolve; those in the Yin aspect are difficult to eradicate. The divine power to determine life and death, and the principles to distinguish shapes and colors, are recorded in the medical texts of Liu Juanzi. After pus drains from a carbuncle or gangrene, five conditions indicate incurability: 1. The whites of the eyes turn blue-black and the pupils shrink; 2. Vomiting occurs immediately after taking medicine; 3. Severe pain accompanied by extreme thirst; 4. Numbness in the shoulders, neck, and back; 5. Hoarseness and sallow complexion. These represent the extremity of the disease. Additionally, consuming various raw fruits invites pathogens. Health preservation laws state: do not eat fruits without proper pits; do not eat food mixed with impurities. All such practices can cause sores and carbuncles. I briefly outline these principles to inform future generations.
The *Qianjin Fang* states that Back Carbuncles (Fa Bei) are caused by taking Five Minerals Powder (Wushi San), Han Shi Powder (Cold Food Powder), or Gengsheng Powder. Some develop them from taking only Zeolite (Zhongru). Others who never took mineral powders still develop Back Carbuncles due to ancestral consumption of such substances. Symptoms usually begin between the shoulder blades, initially the size of a millet seed, painful or itchy, and red. People often ignore them initially. They grow daily and become fatal within ten days. In critical stages, the sore is 3-4 cun in diameter and 1 cun high, with dozens of holes. Pressing causes pus to flow out from all holes, followed by loss of voice. Therefore, health preservers should, upon feeling slight pain or itching on the back, take clean earth soil, mix with water to make mud, shape into cakes 1 cun in diameter and 0.2 cun thick, apply to the sore, and moxibustion with coarse mugwort. Replace the cake with each moxa cone. If the swelling is millet-sized, seven cakes suffice for cure. If plum-pit sized, forty-nine cones suffice. If coin-sized...
