The *Shanfan* text states: In cases of "Rou Ji" (Extreme Muscle Disorder), the pathology is primarily located in the Spleen. The Spleen corresponds to the muscles, and the muscles are harmonized with the Spleen. If the Spleen becomes diseased, the color of the muscles will change. It is also stated: When a body at the extreme of Yin encounters disease, it manifests as muscle issues and scrofula-like lumps below the nose, starting externally but ultimately becoming incurable. (This content appears identically in Volume 8 of the *Qianjin Fang*).
Furthermore, *Shanfan* treats "Rou Ji Re" (Heat-type Extreme Muscle Disorder). Symptoms include a numb sensation in the skin and muscles, feeling as if mice are crawling on the body, depletion of body fluids, open interstices (pores), and profuse sweating; this condition is termed "Pi Feng" (Spleen Wind). Pathogenic wind hides within the skin and muscles, causing the discoloration of the flesh and a yellowish hue on the nose. The formula used is Mahuang Zhihan Tongrou Jiefengbi Tang (Ephedra Decoction for Stopping Sweat, Unblocking Flesh, and Resolving Wind-Bi).
Ingredients: Ephedrae Herba (Mahuang, 麻黄) [nodes removed], Immature Bitter Orange Fructus (Zhishi, 枳实) [processed with bran], Saposhnikoviae Radix (Fangfeng, 防风), Atractylodis Macrocephalae Rhizoma (Baizhu, 白术), Asari Radix et Rhizoma (Xixin, 细辛) [each 3 liang], Gypsum Fibrosum (Shigao, 石膏) [8 liang, crushed and wrapped in silk], Zingiberis Rhizoma Recens (Shengjiang, 生姜), Aconiti Lateralis Radix Praeparata (Fuzi, 附子) [processed] [each 4 liang], Glycyrrhizae Radix et Rhizoma (Gancao, 甘草) [processed], Cinnamomi Ramulum (Guixin, 桂心) [each 2 liang].
Chop the above ten ingredients. Add nine sheng of water. First boil Mahuang, skim off the foam, then add the remaining herbs. Boil until three sheng remain. Divide into three doses. Avoid pork, seaweed, Brassica rapa var. pekinensis (Songcai, 菘菜), raw green onions, raw vegetables, plums, peaches, and sparrow meat. (Identical to *Qianjin Fang*).
Another treatment for "Rou Ji Re": Symptoms include a crawling sensation on the body, or Wind-Bi, ulceration of the lips and mouth, and changes in skin color. The formula used is Shinan San (Shinan Powder).
Ingredients: Aristolochiae Herb (Shinan, 石南) [processed, 5 fen], Dioscoreae Rhizoma (Shuyu, 薯蓣), Aconiti Radix (Tianxiong, 天雄) [processed], Peach Flower (Taohua, 桃花), Chrysanthemi Flos (Juhua, 菊花), Glycyrrhizae Radix et Rhizoma (Gancao, 甘草) [processed] [each 4 fen], Astragali Radix (Huangqi, 黄芪) [3 fen], Corni Fructus (Shanzhuyu, 山茱萸) [7 fen], Margarita (Zhenzhu, 真珠) [2 fen], Gypsum Fibrosum (Shigao, 石膏) [8 fen, crushed], Cimicifugae Rhizoma (Shengma, 升麻), Polygonati Rhizoma (Weirui, 葳蕤) [each 6 fen].
Pulverize the above twelve ingredients into a powder. Take one fangcun bi (approx. 1 cubic inch) per dose. Administer after meals, twice daily. Take with warm clear wine. Avoid pork, seaweed, and Songcai. (Identical to *Qianjin Fang*, both from Volume 8).
*Qianjin Fang* treats "Rou Ji Re": Symptoms include depletion of body fluids, open interstices, profuse sweating, invasion by severe wind pathogens, weakness in the lower jiao leading to weak legs. The formula used is Yuebi Tang (Yuebi Decoction).
Ingredients: Ephedrae Herba (Mahuang, 麻黄) [6 liang, nodes removed], Gypsum Fibrosum (Shigao, 石膏) [8 liang, crushed and wrapped in silk], Zingiberis Rhizoma Recens (Shengjiang, 生姜) [2 liang], Glycyrrhizae Radix et Rhizoma (Gancao, 甘草) [processed, 2 liang], Aconiti Lateralis Radix Praeparata (Fuzi, 附子) [processed, 1 piece], Jujubae Fructus (Dazao, 大枣) [15 pieces, split].
Chop the above six ingredients. Add seven sheng of water. Boil until two sheng and five he remain. Strain the dregs. Divide into three doses. This formula is also known as Qipi Tang (Splee-Raising Decoction). Avoid pork, seaweed, and Songcai. (Original formula lacks Fuzi; Shanfan includes it. Both from Volume 7).
Another treatment for "Rou Ji Xu Re" (Deficiency-Heat type Extreme Muscle Disorder): Symptoms include a creeping sensation in the skin and muscles, depletion of body fluids, open interstices, profuse sweating, Bi syndrome resulting in numbness, and acute pain and stiffness in the four limbs. The formula used is Xizhou Xuming Tang (Xizhou Reviving Life Decoction).
Ingredients: Ephedrae Herba (Mahuang, 麻黄) [nodes removed], Zingiberis Rhizoma Recens (Shengjiang, 生姜) [each 3 liang], Angelicae Sinensis Radix (Danggui, 当归), Gypsum Fibrosum (Shigao, 石膏) [crushed and wrapped in silk, each 2 liang], Ligustici Chuanxiong Rhizoma (Xiong, 芎), Cinnamomi Ramulum (Guixin, 桂心), Glycyrrhizae Radix et Rhizoma (Gancao, 甘草) [processed], Astragali Radix (Huangqi, 黄芪), Scutellariae Radix (Huangqin, 黄芩), Saposhnikoviae Radix (Fangfeng, 防风), Paeoniae Radix Alba (Shaoyao, 芍药) [each 1 liang], Pruni Semen Armeniacae Amarum (Xingren, 杏仁) [40 seeds, roasted, tips and skins removed, single kernel].
Chop the above eleven ingredients. Add nine sheng of water. First boil Mahuang, remove the foam on top, then add the other herbs. Boil until two sheng remain. Strain the dregs. Divide into four doses. Take twice daily. Avoid seaweed, Songcai, and raw green onions. (Identical to Shanfan, both from Volume 8).
