Xu Renze stated that this disease has two names: one is Gan Huoluan (Dry Cholera), and the other is Shi Huoluan (Wet Cholera). More patients die from Gan Huoluan, while fewer die from Shi Huoluan. Both are caused by irregular diet and improper nursing.
The symptoms of Gan Huoluan include abdominal distension and fullness, twisting and stabbing pain in the abdomen, unbearable restlessness and oppression, cold extremities, and in severe cases, profuse sweating like water. There is obstruction of both urination and defecation; the patient desires to vomit but cannot, and desires to have diarrhea but cannot. If not rescued promptly, it poses a threat to life.
The symptoms of Shi Huoluan also include twisting pain in the heart and abdomen. Some manifestations are similar to those of Gan Huoluan, but vomiting and diarrhea occur incessantly. At the onset, this disease may resemble Tianxing (Epidemic Disease), accompanied by headache, soreness and pain of bones and muscles, cold extremities, and heat in the four limbs.
For Gan Huoluan characterized by obstruction of urination and defecation, extreme restlessness, and a desire to die, one should urgently administer pills made from Radix Crotonis Fructus (Badou) and two other ingredients. The medication should be taken until rapid purgation is achieved.
Ingredients: Radix Crotonis Fructus (Badou, 100 seeds, roasted, hearts and skins removed); Rhizoma Zingiberis Preparatum (Ganjiang, 3 liang; Cui's formula uses Natrii Sulfas/Mangxiao 5 liang instead; Qianjin Fang is the same); Radix et Rhizoma Rhei (Dahuang, 5 liang).
First, pound the Ganjiang and Dahuang into a powder. Then, separately pound the Badou into a paste. Mix the paste with the two powders thoroughly. Gradually add honey to form pills. Administer with warm water. Initially, take three pills, sized like Sophora japonica seeds (Wutongzi). After ingestion, massage the abdomen to promote qi movement and induce rapid diarrhea. If no effect is felt after a long time, administer hot drinks. If still no effect after another period, take one more pill; diarrhea should occur shortly thereafter. After purgation, carefully regulate diet and beverage temperature according to one's comfort. If thirsty, drink small amounts of cooked Jiangshui Zhou (fermented rice gruel). Avoid wild boar meat, Asparagus cochinchinensis (Lusun), and other such foods. (From Zhang Wenzhong's collection)
Another prescription for treating Shi Huoluan with incessant vomiting and diarrhea involves a decoction of Gao Liangjiang and two other ingredients.
Ingredients: Rhizoma Alpiniae Officinarum (Gaoliangjiang, 2 liang); Semen Cardamomi Rotundae (Doukouzi, 12 seeds); Cortex Cinnamomi (Guixin, 2 liang).
Chop the herbs, boil in 4 sheng of water until reduced to 1 sheng, strain, and sip slowly. Some variations add Rhizoma Zingiberis Recens (Ganjiang) and Radix Ginseng (Renshen). Avoid raw green onions.
Another prescription consists of a decoction of Fructus Chaenomelis (Mugua) and Cortex Cinnamomi (Guixin).
Ingredients: Fructus Chaenomelis (Mugua, one branch, fresh or dried); Cortex Cinnamomi (Guixin, 2 liang).
Boil in 2 sheng of water until reduced to 7 he, strain, and drink slowly. Some versions substitute Semen Cardamomi Rotundae (Doukouzi) for Cortex Cinnamomi (Guixin), or simply boil Fructus Chaenomelis (Mugua) alone to extract the juice for drinking. Avoid raw green onions.
