At the initial stage, the swelling resembles a small hard nodule (Ehe), gradually enlarging to the size of a fist. Immediately administer Yanghe Decoction and Xihuang Pills daily in rotation, which can resolve it. If treatment is delayed until the swelling becomes as large as a sheng or dou (ancient volume units), remaining hard like stone and painless, it is said that if red veins appear after prolonged illness, it is incurable. Further delay results in the appearance of macules on the affected area, indicating imminent spontaneous ulceration. Once ulcerated, massive bleeding occurs, leading to death within three days. If blue veins appear, it is treatable. Internally administer Yanghe Decoction; externally apply fresh Phytolaccae Radix (Shanglu) mashed into a paste with a little salt. After several days, itching occurs; after half a month, the skin wrinkles. Daily application softens the skin and forms an abscess sac. Puncture with a silver needle. Subsequently, administer Qianjin Tuoli San, adding Rehmanniae Radix Preparata (Shudi) and Rehmanniae Radix (Shengdi), one tael each, decocted in water instead of plain water. After ten doses, apply Yanghe Jieying Plaster fully over the affected area, leaving the puncture site exposed to promote blood circulation. If a pus cavity seems to form within the subcutaneous membrane, bind tightly with cloth to keep the membranes connected. Internally administer Dabu Decoction, Baoyuan Decoction, etc. Ginseng should not be honey-fried. Continue administration until recovery. If pathogenic factors are not completely eliminated, avoid tonifying agents.
Ma stated: Cases presenting with blue veins are also incurable. Although Phytolaccae Radix can dissipate hardness, its use causes skin necrosis; adding salt increases pain, merely damaging muscle tissue and exacerbating pain without necessarily resolving the swelling. Only by administering formulas that tonify Qi and Blood to resolve Yin stagnation can life expectancy potentially be extended.
In children, white masses occur within the hair. Initially, the mother mistook them for contusion-induced swelling. Medical attention was sought only when severe pain developed. Physicians, considering the head as the meeting place of all Yang meridians, used only cold-natured detoxifying herbs. Consequently, in ulcerated cases, pus re-accumulated internally, and new lesions emerged frequently. It was overlooked that the white color of the lesion and non-red pus indicate a syndrome of Yin-cold deficiency and weakness. This condition is treated with Xiaojin Dan. Three doses at the initial stage resolve it; seven pills after ulceration lead to healing. Externally apply Yanghe Jieying Plaster. In adults, this condition is called Fa Ju (Hair Abscess), cured by Yanghe Decoction.
Ma stated: At the initial stage of this syndrome, formulas that disperse Wind and clear Phlegm-Heat should be administered; Yanghe Decoction seems inappropriate.
Furthermore, Shan Tou is caused by the knotting of Wind, Heat, and Phlegm. Adults may also suffer from it, and Yanghe Decoction should not be used.
