Dan (Erysipelas) refers to the skin turning red, as if cinnabar were applied to the surface of the skin. This condition also arises from the accumulation of heat-toxin and stagnation of evil blood in the Mingmen (Gate of Life) and Xianghuo (Ministerial Fire), which combine and manifest together. It often occurs concurrently with smallpox. Its appearance may resemble patches of rosy clouds; those that are superficial and thin are easy to treat, while those that are dense and purple are difficult to cure. Treatment should follow the seasonal climate: in summer heat, use acrid-cool formulas such as Tongsheng San (Universal Salvation Powder) to resolve the exterior and clear heat; in severe cold, use acrid-warm formulas such as Gegen Shengma Tang (Pueraria and Cimicifuga Decoction) to resolve the exterior and disperse cold. When smallpox has erupted, these methods are appropriate. However, if care is neglected, exposure to wind-heat causes blood to boil, while exposure to cold causes blood to coagulate and become stagnant. For boiling blood, use acrid-cool formulas to scatter wind; for coagulated blood, use acrid-warm formulas. Externally, topical applications can also be used. For instance, if Dan spreads from the abdomen to the limbs, the prognosis is good (survival); if it spreads from the limbs into the abdomen, there is no hope of rescue (death).
In children who develop smallpox, if their original Qi (Vital Energy) is strong and solid, the eruption will likely proceed smoothly. Some children have deficient original Qi, or their eruptions fail to fully push out to the surface, compounded by excessive consumption of sweet and fatty foods. This leads to damp-heat accumulating in the stomach and ascending into the meridians, penetrating the mouth, nose, ears, eyes, and upper and lower teeth, causing canker sores (Gan disease). Prolonged exudation of fluids allows damp-heat to generate parasites/worms, corroding the teeth, leading to tooth loss and alveolar bone resorption. Internally, administer formulas that clear the middle and remove damp-heat. Externally, use cocoons from which moths have already emerged, mixed with a pinch of alum (Bai Fan) and wood ash, ground into fine powder, and applied to the affected area for immediate healing.
After a child's smallpox has fully erupted and the fetal toxin has been transformed, why do carbuncles, ulcers, and hard boils (Ding Du) reappear? The answer is: although the fetal toxin has been resolved, Qi and Blood are now depleted. Coupled with neglect in care, this leads to stagnation between Yin and Yang, obstruction of the meridians, and failure of Ying Qi (Nutritive Qi) to flow properly, reversing within the muscle tissue. This generates swelling, carbuncles, and hard boils, resulting in various abnormal symptoms. This applies even more so to ordinary sores and ulcers. Many die prematurely in vain. Mild cases become severe, severe cases become critical. Alas! Is it fate, or is it due to lack of care?
After smallpox scabs fall off, cleanliness should be maintained. If one mistakenly consumes "stimulating" foods (Fa Wu) or frequently bathes the child in windy places, fine sores resembling but not being true scabies may reappear, which are extremely difficult to heal. Various medicines prove ineffective. The best remedy is to take 1–2 liang of Sichuan honey, boiled in water until rolling, and sipped occasionally. Externally, dry the feces of a pregnant sow and burn it to ash. Take an unspecified amount, add 5 fen of Calomel (Qing Fen) and 5 fen of Talcum (Hua Shi), grind them together into a fine powder, mix evenly with wine, and apply to the sores using a goose feather brush for immediate healing.
