Shaofu Ju (Abscess of the Lower Abdomen) arises at the edge below the umbilicus. The syndrome is caused by fire pathogen constrained and tangled due to emotional stagnation (Seven Emotions). It presents with high swelling, redness, pain, and pain radiating to the back. If it is diffusely swollen and hard, or sunken and putrid with thin watery pus, it is difficult to treat.
[Note] This syndrome arises from internal injury to the Seven Emotions causing fire pathogen constraint. It frequently occurs at the Qihai (Sea of Qi), Dantian (Elixir Field), and Guanyuan (Gate of Origin) acupoints. Qihai is located 1.5 cun below the umbilicus, Dantian is 2 cun below the umbilicus, and Guanyuan is 3 cun below the umbilicus; all belong to the Ren Mai (Conception Vessel). Swelling occurring in any one of these three acupoints constitutes Shaofu Ju. If the lesion is highly swollen with vibrant red color, painful with radiation to the back, easy to ulcerate, and discharging thick pus, it is easy to treat. If it is diffusely swollen and hard, slowly ulcerating and putrid with thin watery pus, it is difficult to treat. Upon initial onset of this syndrome, moxibustion should be urgently applied to the apex of the swelling, seven to twenty-one moxa cones (Zhuang), until a sensation of pain and itching penetrates thoroughly. Xianfang Huoming Yin (Immortal Life Elixir Decoction) is indicated. For individuals with excess constitution presenting with intense thirst and constipation, Neishu Huanglian Tang (Internal Purging Coptis Decoction) is indicated to promote unblocking and purging. For elderly and weak individuals, Neibu Shixuan San (Internal Tonification Ten-Auspicious Powder) is indicated to facilitate rapid ulceration; if ulceration is delayed, there is a risk of penetrating the inner membrane. External treatment follows the methods outlined in the chapters on Ulcerative Sores and Ulcers.

Fupi Yong (Carapace Carbuncle of the Abdomen) arises within the abdominal skin, swelling and causing dull pain between the skin and the fascia. Unremitting abdominal pain is a sign that pus has formed. The syndrome is caused by fire pathogen generated from dietary constraints due to rich food intake.
[Note] This condition arises between the abdominal skin and the fascia, regardless of the left or right side. After long-standing dull pain, a carbuncle subsequently appears on the external skin surface. If the pulse at the Right Guan position is deep and rapid, accompanied by severe abdominal pain, these are the signs. It results from fire pathogen constraint due to consumption of rich and greasy foods. In the initial stage, for those with robust constitution, Shuangjie Guijin Wan (Double-Resolving Noble Gold Pill) is used for purgation; for those with weak constitution, the dose is halved. If ineffective, another half dose is administered. Following purgation, if abdominal pain persists, it indicates that pus is about to form. At this point, Tuoli Tou Nong Tang (Supporting Interior and Penetrating Pus Decoction) should be urgently administered. After ulceration, the treatment method is the same as that described in the chapter on Ulcerative Sores. Overuse of harsh draining agents must be avoided. If one seeks resolution of the swelling but excessively damages Stomach Qi, the swelling will fail to ulcerate, and once ulcerated, it will fail to heal, leading immediately to critical danger.
