**Kujin Jian (Withered Tendon Arrow)**
Kujin Jian arises from Liver blood deficiency failing to nourish the sinews, causing Sinew Qi to manifest externally in a shape resembling a red bean. If it ruptures and protrudes like the stamen of a flower, tying it off or applying moxibustion will achieve a cure.
[Note] This condition is also known as "Youzi" (Warts). It is caused by Liver blood deficiency failing to nourish the body, resulting in Sinew Qi manifesting externally. Initially, it resembles a red bean; as it withers, it becomes slightly shriveled. After a long time, it ruptures, and tendon-like heads emerge, appearing fluffy and dry, similar to flower stamens. It mostly occurs between the hands, feet, chest, and breasts. For those with thin roots, medicinal threads should be tied tightly at the base; after seven days, the lesion will naturally fall off. Apply Yuebai Zhenzhu San (Moon-white Pearl Powder) to promote scar contraction. For those with large roots and small tops, place a copper coin over the wart, use paper straw cores instead of mugwort, and apply three moxa cones until the lesion withers and falls off. For larger warts, wet paper strips can be placed over the wart for moxibustion.
**Jie Chuang (Scabies)**
Jie Chuang presents in five types: Dry Scabies, Wet Scabies, Insect Scabies, Sand Scabies, and Pustular Scabies. These are caused by toxin accumulation in various meridians, transforming into fire due to wind-dampness. Treatment involves distinguishing based on upper/lower body involvement and fat/thin body constitutions, employing methods to clear wind, drain dampness, and kill parasites.
[Note] This condition is categorized into Dry, Wet, Insect, Sand, and Pustular types. Although there are five morphological variations, they all stem from toxin accumulation in the meridians, generating heat over time, combined with wind-dampness invasion. It may also be contagious. Scabies typically begins in the finger webs and spreads throughout the body, causing intense itching.
If Lung Meridian dryness is excessive, Dry Scabies occurs, characterized by dry, itchy skin with white scales.
If Spleen Meridian dampness is excessive, Wet Scabies occurs, presenting with swelling, pain, yellow discharge upon rupture, and potentially black fluid in severe cases.
If Liver Meridian wind is excessive, Insect Scabies occurs, with itching penetrating to the bone, where scratching does not relieve pain.
If Heart Blood stagnates, Sand Scabies occurs, resembling fine sand, with redness, itching, pain, and fluid upon scratching.
If Kidney Meridian damp-heat is present, Pustular Scabies occurs, resembling bean grains, with itching when bowel movements are smooth, and pale, thin pus.
If Spleen Meridian dampness is excessive, Pustular Scabies may also occur, but with thick pus and simultaneous itching and pain, which distinguishes it.
Although Scabies is generally an excess syndrome, deficient individuals may also contract it. Constipation indicates excess, while normal bowel movements indicate deficiency. However, deficiency can also lead to dry stools; for instance, Wind Constipation causes dry stools, and dryness in the Blood aspect causes difficult,涩 (astringent/difficult) defecation. Differentiation is further made by observing the depth of color in the lesions and the strength or weakness of the pulse.
For initial cases of excess constitution, Fangfeng Tongsheng San (Saposhnikovia Decoction for Unimpeded Flow) is indicated. For deficient cases, Jingfang Baidu San (Aucklandia and Saposhnikovia Decoction for Resolving Toxin) is used to vent the pathogen.
Once the condition stabilizes, regardless of excess or deficiency:
Dry Scabies is treated with Xiaofeng San (Wind-Expelling Powder);
Wet Scabies with Cangzhu Gao (Atractylodes Ointment);
Insect Scabies with Lushui Wan (Aloe Pill);
Sand Scabies with Xijiao Yinzi (Rhinoceros Horn Drink);
Pustular Scabies with Qinjiao Wan (Gentian Pill);
For chronic cases involving blood dryness, Danggui Yinzi (Angelica Drink) is administered.
External treatments: Dry Scabies is rubbed with Xiuyuan Wan (Embroidered Ball Pill); Wet Scabies with Chouling Dan (Stinky Spirit Elixir), both effective for moisturizing, drying, and killing parasites.
Scabies predominantly on the upper body suggests excessive Wind-Heat; predominantly on the lower body suggests excessive Damp-Wind. Obese individuals tend toward Damp-Wind, while thin individuals tend toward Blood-Heat. Detailed differentiation and treatment are required.
