Wrist carbuncle (a carbuncle occurring on the wrist) is caused by the condensation of wind and fire pathogens in the three yang meridians of the hand, manifesting as a carbuncle shape on the dorsal aspect of the wrist. If the swelling is high and it suppurates rapidly, the condition is favorable (Shun) and easy to treat; if it ulcerates with exposed bone, it is adverse (Ni) and difficult to cure.
[Note] This syndrome occurs on the dorsal aspect of the wrist, belonging to the three yang meridians of the hand, formed by the condensation of wind and fire pathogens. If the area is red, swollen, high, and vibrant in color, and suppurates within fourteen days with pus discharge and pain relief, it is a favorable case (Shun) and easy to treat; the wrist is an area where skin and flesh are thin. If it lingers for a long time without suppurating, or presents with diffuse swelling and flat collapse, and once ulcerated, becomes rotten with exposed bone, it is an adverse case (Ni) and difficult to heal. Initially, administer Jingfang Baidu San (Atractylodes and Saposhnikovia Decoction modified with Scutellaria and Isatis) to induce sweating. Externally apply Taiyi Zijin Ding (Taiyi Purple Gold Pill). When pus has formed and is about to rupture, follow the treatment principles for early-stage (Zhongyang) and late-stage (Kuyang) sores and ulcers.

Taiyi Zijin Ding (See Chest and Spleen Carbuncle section)
Dui abscess (Dui Ju) arises between the arteries of the wrist, presenting as hard and diffusely swollen along the edge of the Dui bone. Pain radiating through the arm indicates a dangerous syndrome; its location belongs to the Taiyuan acupoint of the Lung Meridian.
[Note] This syndrome occurs on the palmar aspect of the wrist, between the arteries slightly anterior to the transverse crease, on the medial side of the Dui bone, belonging to the Taiyuan acupoint of the Lung Meridian. It is formed by qi stagnation due to worry and thought, combined with the condensation of wind and fire. It presents as hard, diffusely swollen, with pain penetrating to the bone and inability to rotate the arm. This arterial area is the gateway of the Lung Meridian. If this abscess develops here, deep ulceration leading to massive leakage of Lung Qi is extremely dangerous. Internal and external treatments follow the categories for early-stage (Zhongyang) and late-stage (Kuyang) sores and ulcers.
Bone-penetrating abscess (Chuan Gu Ju) arises at the Jianshi acupoint, located three cun posterior to the palm on the Pericardium Meridian. It is caused by the condensation of accumulated heat, leading to hardness and diffuse swelling. When the pathogenic toxin is severe, it ulcerates deeply, penetrating the bone and causing pain.
[Note] This syndrome occurs at the Jianshi acupoint, in the depression between two tendons three cun above the transverse crease of the wrist, belonging to the Pericardium Meridian, formed by the condensation of accumulated heat. Initially, it appears like a millet grain, gradually increasing in hardness, with diffuse swelling, slight redness, heat, and pain. If it suppurates rapidly according to the expected timeline, it is a favorable case (Shun); if ulceration is delayed, and pus/toxin penetrates the bone gaps, discharging from the lateral aspect of the arm, it is a dangerous case. Internal and external treatments follow the categories for early-stage (Zhongyang) and late-stage (Kuyang) sores and ulcers.
