At the initial stage, presenting with swelling, pain, and fever, along with difficult and obstructed urination, this is caused by damp-heat in the Liver channel; the appropriate treatment is to purge the Liver and leach out dampness. If the swelling and pain are hard and firm, accompanied by alternating chills and fever, dry mouth and throat, and constipation, it is necessary to unblock and promote the discharge of both urine and feces. For swelling and pain of the *Yujing* (penis), urinary symptoms resembling strangury, spontaneous sweating or night sweats, and occasional hematuria, the method is to clear the Heart and nourish the Kidney. For itching at the urethral orifice, frequent discharge of white turbid fluid, fever, dry mouth, and decreased body fluids, the method is to benefit the Kidney and clear the Heart. After ulceration, if swelling and pain persist, urine is red and astringent, the patient is quiet during the day but suffers severe pain at night, the method is to nourish Yin and purge fire. If ulceration does not heal, swelling and pain recur, pain deepens daily, and foul odor persists, the method is to clear the Liver and detoxify.
A fifteen-year-old boy presented with swelling and pain of the penis, external skin edema, and the organ was twice as thick as usual. Other physicians treated him with herbs for detoxification and clearing the Liver, resulting in increased swelling and pain. Upon examination, I also used formulas for purging fire, clearing heat, and leaching out dampness, but they were ineffective. His pulse was fine, rapid, and weak, indicating deficiency of Middle Qi, where damp-water from the Spleen channel flowed into the deficiency and stagnated without dispersing. Therefore, the "following treatment" method (treating according to the apparent false syndrome) should be employed. A combination of *Siwu Tang* (Four Substances Decoction) and *Pingwei San* (Powder for Leveling the Stomach), with each of *Muxiang* (Aucklandiae Radix), *Shu Fuzi* (Prepared Aconite Root), and *Renshen* (Ginseng Radix et Rhizoma) added in 5 fen doses, resulted in immediate reduction of swelling and pain after one dose, and complete resolution after four or five doses.
Another male patient presented with swelling, pain, chills, and fever. Two doses of *Jingfang Baidu San* (Schizonepeta and Saposhnikovia Powder for Overcoming Toxins) resolved the fever and swelling. Subsequently, *Longdan Xiegan Tang* (Gentian Liver-Purging Decoction) reduced the swelling and pain; finally, *Siwu Tang* (Four Substances Decoction) with added *Huangbai* (Phellodendri Cortex) and *Zhimu* (Anemarrhenae Rhizoma) eliminated the symptoms.
A forty-year-old man, due to excessive sexual activity, experienced difficult and painful urination, discharging white paste-like fluid that coagulated into clumps. This was caused by Kidney injury and damp-heat. In the morning, he took *Bawei Wan* (Eight-Ingredient Pill); at noon, he took *Bazhen Tang* (Eight-Treasure Decoction) with added *Shanyuzi* (Corni Fructus), *Mudanpi* (Moutan Cortex), *Huangbai* (Phellodendri Cortex), and *Zhimu* (Anemarrhenae Rhizoma). After more than a month of medication, the difficulty and swelling subsided, and urination gradually cleared. However, seminal emission persisted, with occasional leakage, leading over time to weakness in the lower back and knees. He happened to take *Sanyin Shengjun Wan* (Three Causes Victory Horse Pill); after a month of use, all previous symptoms were completely cured.
A male patient had ulceration of the glans penis with difficult and painful urination. Externally, *Zhenzhu San* (Pearl Powder) was applied; internally, *Mutong Tang* (Akebia Decoction) was administered. After four doses, the difficulty and pain ceased. Further administration of *Siwu Tang* (Four Substances Decoction) with added *Huangbai* (Phellodendri Cortex) and *Huafen* (Trichosanthis Radix) led to recovery.
A male patient suffered injury to the penis upon first marriage, with bleeding and severe pain. Initially, the area was soaked and washed for a long time in green onion soup, followed by the application of *Zhenzhu San* (Pearl Powder) mixed with *Bingpian* (Borneolum Syntheticum). The necrotic skin gradually dried, and the pain subsided. Internally, *Siwu Tang* (Four Substances Decoction) with added *Mutong* (Akebiae Caulis) and *Shanzhi* (Gardeniae Fructus) was taken; after four doses, the swelling and pain gradually ceased, and he recovered.
A male patient presented with shiny, swollen skin, fever, and severe pain. *Ruyi Jinhuang San* (Ruyi Golden Yellow Powder) was applied externally, and *Longdan Xiegan Tang* (Gentian Liver-Purging Decoction) with added *Dahuang* (Rhei Radix et Rhizoma) was taken internally. Bowel movements and pain improved slightly; after removing *Dahuang*, continued administration led to resolution.
