Buttock carbuncle occurs behind the lower abdomen. Its location is deep and remote, where Qi rarely reaches and Blood rarely flows. Any case arising here is formed by the coagulation, stagnation, and accumulation of Damp-Heat. Upon contracting this pathogenic toxin, it must manifest outwardly to prevent internal invasion. In the initial stage, if the toxin accumulates from the Zang organs, the affected area will present with redness, heat, swelling, and pain, feeling heavy like a stone. Internally, there will be dry mouth and fever. It is appropriate to use Neixiao Woxue Tang to clear accumulated heat internally, while applying an ointment to the top of the sore externally, and surrounding it with Ruyi Jinhuang San to draw out stasis, pus, and purple blood. Concurrently using tonic drugs to support the body's resistance will lead to self-healing. Often, those unaware of proper treatment administer cooling and toxic-clearing herbs internally and apply cooling medicines externally. This causes the freezing and coagulation of Qi and Blood, preventing the toxin from expelling outward, leading instead to internal invasion. The sore head becomes soft and sunken without pus, the base spreads flat without pain, accompanied by internal heat, dry mouth, restlessness, delirium, phlegm-panting, coarse breathing, and mental confusion. These are all considered intractable cases. However, most buttock carbuncles arise from internal toxin accumulation, spreading from interior to exterior in eight or nine out of ten cases. Cases originating from external Wind-Cold, Summerheat, or Dampness invading inward account for only one or two out of a hundred. If the pathogen emerges but re-enters the interior, it ultimately results in death.
In the initial stage, if there is a distinct head, redness, swelling, pain, high elevation of the top with fever, and a raised base, it is easy to treat. In the established stage, if heat causes swelling, pain is intermittent, the skin is thin and shiny, redness indicates suppuration, it is a favorable progression. After ulceration, if the pus is thick and yellow, resembling pig brain, swelling subsides, pain stops, and movement becomes free, it is easy to heal. In the initial stage, if it resembles a millet grain or a single yellow blister, is flat and itchy with a diffuse base, the condition is severe. In the established stage, if the color is purple and hard, with swelling of both the buttock and thigh, difficult urination, and no pus formation after a long time, the condition is dangerous. After ulceration, if there is no thick pus, the top is hollow and the base is soft and sunken, insensitivity to touch, and coma, there is no cure.
