"Shi Yi" (Supplementary Notes) refers to items that were omitted during the classification of clauses and categories, failing to be detailed sufficiently. However, this book establishes disease categories to rectify the main focus on diseases, resulting in some conditions being lost. Now, these omissions are checked, supplemented, and annotated for easier observation and treatment.
Bi Ding (Nasal Carbuncle) arises inside the nose, with pain radiating to the forehead, causing difficulty in breathing and nasal congestion; in severe cases, the lips and cheeks swell. Ya Ding (Dental Carbuncle) occurs between the teeth, presenting as a high, protruding peak, with pain extending to the cheeks and neck; upon rupture, it bleeds. Hei Ding (Black Carbuncle) develops within the ear canal, appearing black, hard, and rotting; upon rupture, it discharges bloody fluid, with pain affecting the cheeks and zygomatic bones. For the above symptoms, acupuncture should be performed first, followed by inducing sweating, and then treated according to the protocols for Ding-type lesions.
Hou Yong (Throat Abscess) forms at the center of the external throat, causing swelling and pain that hinder eating. It presents with redness, swelling, and fever, and must suppurate. If it becomes soft and distending-painful, it should be punctured with a needle. Internally, administer tonifying and expelling-toxin medicines; externally apply Yuhong Gao (Jade Red Ointment) to promote tissue regeneration and wound closure. In cases where ulceration penetrates internally, allowing liquids to exit through the perforation, several such patients have been treated without adverse outcomes.
Ju (Deep-seated Abscess) appears on the upper arm, spreading to the shoulder with generalized swelling, becoming long, firm, and hard. Dui Ju arises at the location of the radial artery in the hand, accompanied by swelling, pain, chills, and fever, with pain radiating through the arm, making movement difficult. Feng Mei Ju (Phoenix Eyebrow Abscess) erupts between the two eyes, characterized by a long shape and reddish skin, with pain radiating to the occiput, causing the eyes to close tightly, bulging, swollen, and hot. Tou Nao Ju (Brain-Penetrating Abscess) develops between the hairline on the forehead, often accompanied by chills and fever, and severe headache as if chopped, which is unbearable. Initially, use Wanling Dan (Ten Thousand Spirit Pill) to induce sweating and dissipate wind pathogens, followed by clearing and supporting methods. Fu Yin Ju (Attached Yin Abscess) forms three cun above the medial malleolus, starting as small blisters that gradually become red and swollen, rupturing to discharge bloody fluid, with bone-piercing pain preventing walking. Yao Gu Ju (Bone-Biting Abscess) occurs in the inner thigh, without swelling or redness, but with pain penetrating the bone marrow. Initially, treat with Leihuo Zhen (Thunder Fire Needle); internally, administer Wanling Dan mixed with wine for efficacy. Yin Ju (Yin Abscess) appears three cun below the crease of the right leg, with pain radiating to the genitals and lower abdomen. Xuan Ju (Mysterious Abscess) forms three cun below the crease of the left leg, presenting with diffuse swelling extending to the genitals and pain reaching the thigh. Yuan Ju (Abyss Abscess) erupts under the hypochondrium, initially non-red and hard, eventually rupturing with sounds resembling an infant's cry when covered with plaster; if the sound persists after removing the plaster, it is indeed difficult to treat, eliciting a helpless smile. Yu Zhen Ju (Jade Pillow Abscess) arises in the occipital bone at the back of the head, hard and difficult to suppurate, with pain radiating to the shoulders and neck, nasal congestion, and coarse breathing. This is caused by the stagnation of damp-heat in the Taiyang Bladder meridian. Generally, among the aforementioned conditions, those with both name and distinct clinical presentation are rare. If exterior symptoms are present at onset, release the exterior; if interior symptoms exist, promote unblocking and purgation; after suppuration, use tonification and expulsion of toxins; for chronic cases, employ astringents. These are fixed treatment principles. Do not indiscriminately administer medications, leading to critical danger or death.
