Nongke Chuang (Crusted Pustules) is caused by heat in the Lung Meridian (Lung Channel) and dampness in the Spleen Meridian (Spleen Channel), where these two pathogenic factors interact. The condition initially presents as small blisters accompanied by itching, which then transform into pustules causing pain, thereby forming Nongke Chuang. In severe cases, treatment should employ methods to clear heat and disperse wind, cool blood and remove dampness, using Liangxue Qingfeng San (Cool Blood and Disperse Wind Powder). Externally, Shechuangzi San (Cnidium Seed Powder) or Zhuchuang Yisaoguang (All Sores One-Wipe Light) can be applied with efficacy. Dietary restrictions are also required for spontaneous recovery.
Shechuangzi San
Shechuangzi San utilizes Huangdan (Lead Monoxide), Qingfen (Calomel), Kufan (Alum), and Dafengzi Rou (Seed of *Aleurites fordii*), along with a substantial amount of Dahuang (Rhubarb Root and Rhizome) and Songxiang Mo (Pine Rosin Powder). Mixed with a small amount of Mayou (Sesame Oil), it treats pustular sores.
This formula treats Nongke Chuang occurring on the hands, feet, and entire body, characterized by hard roots, swelling, and intense itching and pain.
Herbs: Shechuangzi (Fructus Cnidii), Dafengzi Rou (Seed of *Aleurites fordii*), Songxiang (Pine Rosin), Kufan (Alumen) (each 1 liang); Huangdan (Lithargyrum), Dahuang (Radix et Rhizoma Rhei) (each 5 qian); Qingfen (Calomel) (3 qian).
Grind all herbs into fine powder. Mix with sesame oil for external application; if the lesion is wet and ulcerated, apply the dry powder directly.
Another Formula for Nongke Chuang
This formula for Nongke Chuang uses Shigao (Gypsum Fibrosum), Huangbai (Phellodendri Cortex), Huangdan, and Qingfen in generous amounts. Additionally, Kufan helps to dry dampness; upon application, pain and itching quickly subside.
Herbs: Huangbai (Phellodendri Cortex) (1 liang, rubbed with bile from a male pig and roasted over fire, use 3 qian); Shigao (Gypsum Fibrosum) (1 liang); Qingfen (Calomel) (2 qian); Huangdan, Kufan (each 3 qian).
Grind into powder. Mix with sesame oil for application; if wet, apply dry powder.
Liangxue Xiaofeng San (Cool Blood and Eliminate Wind Powder) and Zhuchuang Yisaoguang (see section on Scabies).
Dongfeng (Frostbite/Cold Wind Injury) refers to extreme cold in the muscles and stagnation of Qi and Blood, termed "Muscle Death." It initially presents as purple spots, which later turn black, rot, and suppurate. Biyu Gao (Jade Green Ointment) is the primary treatment to promote tissue regeneration and close wounds.
Biyu Gao (see section on Chancre).
Used to treat skin and muscle damage and ulceration due to Dongfeng, with continuous discharge of pus and unceasing pain.
Dusheng Gao
Dusheng Gao has potent warming and cold-dispersing properties, providing relief for both frostbite and chilblains on the ears.
Apply during the days ending in six in the sixth lunar month (6th, 16th, 26th). Rubbing this ointment prevents winter frostbite.
Treats frostbite on heels and ears, which flares up during winter cold.
Method: On the 6th, 16th, and 26th days of the sixth lunar month, crush a single clove of garlic into a paste, heat it in the sun at noon, and rub it onto areas prone to frostbite in winter. Avoid contact with water. Apply three times a day to prevent recurrence. Additionally, for chronic frostbite, soak the affected area in a decoction of Qiegen (Eggplant Stem) and Congbai (Scallion White) to prevent reoccurrence.
