For conditions such as flowing abscesses (Liuzhu), phlegm nodules (Tanke), crane's knee (Hexi, a type of knee joint disease), limb masses, or when wind-cold invades the meridians causing tendon spasms and bone pain, as well as for traumatic injuries, apply the Analgesic and Blood-Resolving Powder. Apply heat to the affected area and iron it; swelling and pain will cease immediately.
Another method involves taking a handful of green onions, bundling them into several sections, slicing them into cakes, placing them on the affected area, and ironing with hot objects, or applying moxibustion over them. It is advisable to change the onion cakes frequently and perform multiple sessions of moxibustion and ironing for optimal results.
Castor Bean Extraction Method (This is only used when redness and swelling are extremely severe and as a last resort. It is contraindicated during pregnancy/postpartum or if the lesion is white in color.)
Mash castor bean kernels into a paste, spread them on silk fabric, cover the affected area according to its size, secure tightly with cloth, and remove immediately once pain subsides.
Yellow Wax Extraction Method (Suitable for chronic ulcers that have been open for a long time, emitting foul odors, rotting with black discoloration, presenting with fungating growths, or stubborn non-painful ulcers around the anus; it is excellent for extracting toxins.)
Grind yellow wax into shavings. First, use wet flour to form a circle around the base of the swelling, about one cun high, adhering it firmly to the skin like the rim of a well. Wrap several layers of cloth around the outside of the circle to prevent the fire from heating the wax shavings too quickly. Add wax shavings as they melt until the "well" is full. Remove the heat source, sprinkle a little cold water onto the wax with a ladle, wait for it to cool, and then lift it off. If the bottom of the wax turns dark blue-black, it indicates that the toxin has been extracted.
After an yin abscess ulcerates, when applying external plasters, cover them with four to five layers of silk gauze. Then place two pieces of cotton cloth, each eight cun square, connected at the corners with soft cotton yarn and fine leather membranes, to promote smooth qi and blood circulation and facilitate tissue regeneration.
After carbuncles and sores ulcerate, if the pus and fluid are fishy and foul, failure to wash them will allow toxins to spread across the skin, or filth may accumulate and breed maggots, preventing qi and blood from harmonizing and relaxing. How can one expect new tissue generation? Clinical management should be adjusted accordingly. For severe cases, wash once daily; for mild cases, wash every other day. Use the second rinse water from washing rice, preferably from glutinous rice. Take six or seven bowls of this rice water, add herbs, boil in a pot until the decoction is rich, strain out the dregs. Dip fresh cotton wool into the decoction to pour over and wash the affected area, replacing with fresh warm liquid as it cools. Gently press beside the affected area to help old pus exit with the liquid, stopping when clean. After washing, dab dry with soft cotton paper, then apply the appropriate medicated plaster. Washing with herbal decoction helps to unblock Ying and Wei (nutritive and defensive qi), achieving two goals with one action; using rice water for decoction leverages its ability to expel filth and generate new tissue, offering benefit without cost. Human qi and blood flow smoothly with fragrant aromas but become stagnant with foul odors; therefore, among washing agents, Ji Fang San (Collect Fragrance Powder) is considered the best.
Jie Du San (Antidote Powder) (Used to wash all chronic, foul-smelling, and severely corrupted ulcers. In severe cases, wash with this powder first, followed by Ji Fang San.)
Bai Fan (Alum, 4 liang) Xiong Huang (Realgar, 1 liang) Guan Zhong (Cyrtomium Rhizome, 2 liang)
Grind into fine powder, mix with boiling water, let cool to a suitable temperature, and wash the affected area.
Ji Fang San (Collect Fragrance Powder) (Used to wash all ulcerated carbuncles and sores.)
Guan Bai Zhi (Angelica Dahurica), Chuan Xiong (Ligusticum Chuanxiong), Huo Xiang (Pogostemon Cablin), Mu Xiang (Aucklandia Root), Fang Feng (Saposhnikovia Divaricata) (each 3 qian), Gan Cao (Licorice Root, 3.5 qian), Cong (Green Onion, a large bunch).
She Jiao Shen Xiao San (Snake Pepper Divine Efficacy Powder) (Used to wash various sores caused by wind, yin-dampness, etc.)
