Whenever a carbuncle is mistakenly treated with purgative or drastic drugs, causing toxic heat to penetrate inward, resulting in abdominal distension and fullness, it is called "carbuncle transformation" (Chuang Bian), which is primarily treated with Zaocant Wan (Date and Silkworm Pill). If the pain is severe and the condition critical, leaving no time to prepare the aforementioned formula, one should first apply the method of "Quan Xiao" (Complete Elimination) to resolve symptoms, and then treat according to standard carbuncle protocols. Since carbuncle transformation is a fatal syndrome while ordinary carbuncles are easy to heal, one must not fear its eruption and thereby bring disaster upon oneself.
Zaocant Wan (Date and Silkworm Pill)
Bai Jiangcan (White Stiff Silkworm, *Bombyx batryticatus*) and Hong Zao (Red Date, *Jujubae Fructus*) (each 4 liang).
First, boil the red dates in water for one or two boils, take the decoction to wash the silkworms, and discard the washing water. Peel and remove the pits from the red dates, mash them into a paste. Dry the silkworms and grind them into a fine powder (2 liang). Mix the powder with the date paste and knead into pills. Take 3 qian each time, three times a day (morning, noon, evening), delivered with red date decoction. The course of treatment is complete when the dose is finished, leading to recovery.
Every autumn and winter, carbuncles may erupt on the neck, back, or entire body, resembling scabies or tinea, or appearing as itchy nodules. They crust over after contact with water and heal naturally when spring warmth arrives. This is called "Ao Dong Chuang" (Winter-Braving Carbuncle). It is treated internally with Shibiao Tang (Solidify Exterior Decoction) to reinforce the exterior, and externally with Xiuzhu Wan (Embroidered Ball Pill) applied topically for cure.
Jingfang Baidu San (Scutellaria and Saposhnikovia Powder to Overcome Toxin) (General Formula No. 4)
Buzhong Yiqi Tang (Decoction to Supplement the Middle and Augment Qi) (General Formula No. 30)
Xiuzhu Wan (See Scabies section above).
When damp-heat stagnates, a red halo appears first, followed by swelling and pain. The lesion is less than one cun in diameter and is called "Jie Du" (Boil Toxin). It is primarily treated with Jieshu Tang (Relieve Summerheat Decoction). At the initial stage, apply a piece of fermented dough thinly over the boil, leaving a small hole in the center; this will cause it to subside. Alternatively, Dian Du Dan (Toxin-Pointing Pellet) can be applied to achieve the same result. If it suppurates and becomes painful, apply Dongtian Gao (Cave Heaven Plaster).
1. For soft boils on the head, applying mashed large taro root externally yields excellent results.
2. For boils that fail to heal and close up after a long time, use Bai Mujin Hua (White Hibiscus Flower, *Hibisci Mutabilis Flos*) calcined to retain its nature, ground into powder, and sprinkled on the wound; this is miraculous.
Another prescription:
Calcinate Bie Jia (Turtle Shell, *Testudinis Carapax et Plastrum* ) until it retains its nature, grind into powder, and sprinkle on the wound; this is also effective.
Jieshu Tang (Relieve Summerheat Decoction)
Qiaoyao (Forsythiae Fructus), Jinyinhua (Lonicerae Japonicae Flos), Chishao (Paeoniae Radix Rubra), Tianhuafen (Trichosanthis Radix), Huashi (Talcum, processed), Cheqianzi (Plantaginis Semen, stir-fried and ground), Gancao (Glycyrrhizae Radix et Rhizoma), Zexie (Alismatis Rhizoma).
Add ten leaves of Danshenye (Light Bamboo Leaf, *Phyllostachys nigra* var. *henonis* leaves, commonly referred to as Danzhuye in this context). Decoct in water and take warm at any time.
Note: This formula clears summerheat and promotes diuresis to drain dampness. Regardless of gender, age, or whether boils are present, it is suitable for consumption during periods of intense summer heat.
Further Note: In extreme summer heat, human healthy qi is inevitably damaged. Therefore, when using summerheat-relieving formulas, adding 5 to 7 qian of Zhi Huangqi (Honey-fried Astragalus Root, *Astragali Radix*) to support primary qi is particularly beneficial, named Huangqi Jieshu Yin (Astragalus Relieve Summerheat Drink). If the boil toxin has suppurated, raw Huangqi (*Astragali Radix*) can be added in equal proportion to the other herbs.
Dian Du Dan (Toxin-Pointing Pellet)
Grind Huangbai (Phellodendri Cortex, *Phellodendri Chinensis Cortex*) into an extremely fine powder. Mix with bile from a male pig (*Suis Vesica Bile*) to form a viscous paste. For early-stage hot boils that have not yet come to a head, apply a small amount to the center of the boil to make it subside.
Feilong Dan (Flying Dragon Pellet)
Hanshui Shi (Calcined Aragonite, *Aragonitum*), Xuejie (Dragon's Blood, *Daemonorops Draconis*), Moyao (Myrrha Resina, defatted), Hong Pi (Realgar, *Realgar*), Ruxiang (Olibanum Resina, defatted), Danfan (Alumen), Xiongjing (Arsenic Trioxide, purified), Wugong (Centipede, *Scolopendra*, 3 qian, each ingredient 3 fen).
Process each herb into a fine powder. Dissolve 3 qian of Chansu (Toad Venom, *Bufonis Venenum*) in wine. Mix all powders with the dissolved venom and knead into pills the size of plump mung beans. Coat with Jinbo (Gold Foil, *Aurum*). Each time, wrap one pill in Congbai (Scallion White, *Allii Fistulosi Bulbus*), chew thoroughly, and swallow with wine. Cover with blankets to induce sweating after feeling intoxicated. Taking this at the initial stage of a small boil will cause it to completely disappear. Contraindicated for Bai Ju (White Gangrene).
