There are four causes for the protrusion of wound edges resembling a snake's head: exposure to wind, contact with water, emotional anger, and manual squeezing. All can lead to this condition. If caused by wind pathogen invasion, use Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) with Fangfeng (Saposhnikovia Root) and Tianma (Gastrodia Rhizome). If caused by cold-wind stagnation, use Shixuan San (Ten Acupoints Powder) with Qianghuo (Notopterygium Rhizome) and Tianma (Gastrodia Rhizome). If caused by liver fire and blood dryness generating wind, use Jiawei Xiaoyao San (Modified Free and Easy Wanderer Powder) with Qianghuo (Notopterygium Rhizome) and Tianma (Gastrodia Rhizome). If in children it is due to the mother's liver fire generating wind, first use Jiawei Xiao Chai Hu Tang (Modified Minor Bupleurum Decoction), followed by Jiawei Xiaoyao San (Modified Free and Easy Wanderer Powder) with Loulu (Euphorbia Root) and Tianma (Gastrodia Rhizome), combined externally with moxibustion or warming therapy. Alternatively, use Wumei (Mume Fruit) charred to retain its properties, mixed with a small amount of Qingfen (Calomel), applied to the wound, which will promote healing. If there are fungating granulations resembling a pomegranate, puncture the black blood from the nodule with a silver needle; after three to five applications, it will flatten and heal.
For stubborn sores, Ding Chuang (Furuncle/Carbuncle), Fistula sores, and beating wounds that fail to regenerate tissue over a long period, it is because a membrane covers the wound. Use Yuzanhua Gen (Ursulina Radix) pounded into a paste in a stone mortar and applied to the site, secured tightly. Change the dressing every three days; healing occurs after five to seven treatments.
If a wound fails to close due to deficiency, substantial tonification is required. Externally, apply equal parts of Ren Shen (Ginseng Radix) and Zhen Zhu (Pearl) powdered together to promote scabbing. All tissue-regenerating medicines must be soaked in Ren Shen (Ginseng Radix) decoction, dried, then soaked in human milk before use to ensure rapid efficacy.
Although the wound has closed, if the skin remains tender and sensitive to external wind-cold, cook millet rice, place it hot in a bowl, cover with silk gauze, and steam the affected area. Healing occurs after three to five sessions.
When an ulcer is mistaken exposure to toxic water or penetrated by thorns reaching the bone, causing swelling and pain, use seven seeds of Zaojiao Zi (Gleditsia Sinensis Spina), one large dried Toad (Chan Su), and fifteen grains of Hu Jiao (Pepper). Place them in a pot, cover with a tile, and roast over gentle heat until smoke ceases. Grind into powder. First wash the area with warm water and dry it, then sprinkle the powder, cover with ointment for a while. Toxic water will drain out, and any thorns will become visible. Another method uses Huang Dan (Red Lead) and Ge Fen (Concha Meretricis) in equal parts, stir-fried until color changes, then applied. For pain caused by accidental entry of various toxic waters, soap pod water, donkey/horse urine/feces, mix Nuomi Fen (Rice Flour) and Bai Sha Tang (White Rock Sugar) each 1.5 taels, with Sheng Gan Cao (Raw Licorice Root) 0.1 tael, make into a paste and apply. Toxic water will naturally discharge.
For swelling and pain caused by horse sweat entering a wound, take the handle part of a horse whip, burn to ash retaining its properties, grind into powder, and mix with various ointments for application, or mix with pig lard, or apply powder made from sun-dried winter melon peel.
Zhang Jingyue stated: For all carbuncles and toxins with severe redness and pain, although internal treatment methods exist, decoctions act slowly. When acute pain is unbearable, external poultices are necessary to stop pain and dissipate toxicity. Nothing is more miraculous than Jiang Yong San (Reduce Carbuncle Powder).
Jiang Yong San (Jingyue New Formula) treats various carbuncles, ulcers, and toxins, reducing swelling, stopping pain, and dissipating toxicity. It eliminates unformed lesions quickly and promotes rapid drainage and healing of formed ones. For fierce Yang toxicity with intense pain, use this formula first for its rapid detoxifying and dissipating effects. For hard, stubborn, and deep-seated lesions, use the following formula.
Bohe (Menthae Haplocalycis Herba, leaves preferred), Ye Ju Hua (Chrysanthemi Indici Flos, including roots and leaves, one handful), Tu Bei Mu (Fritillaria Thunbergii Bulbus, half handful), Mao Gen (Imperatae Rhizoma, one handful).
Dry herbs may be powdered; fresh herbs may be pounded. Mix evenly with Tu Bei Mu (Fritillaria Thunbergii Bulbus). Decoct Mao Gen (Imperatae Rhizoma) to make a strong decoction, strain, and use this liquid to mix with the powder. Apply warm to the affected area. Keep the remaining decoction warm and periodically moisten the medicine. Do not use cold decoction, as cold prevents dispersion and movement, potentially increasing pain. Replace after approximately half a day. This is a truly excellent formula. The following formula is suitable for hard, stubborn, deep-seated carbuncle toxins, as well as nodules and phlegm stagnation.
Bohe (Menthae Haplocalycis Herba, double dose), Ban Nan Xing (Arisaema Cum Bile), Tu Bei Mu (Fritillaria Thunbergii Bulbus), Piao Xiao (Natrii Sulfas), Duan Shi (Lapis Fulguritis, weathered form also doubled or quadrupled).
Grind all into powder. Mix with salt and vinegar to form a thick paste. Apply to the affected area. Replace when dry overnight; no need to leave a center. If pus has formed, leaving a center is acceptable. Alternatively, heat and spread on silk gauze for application. Can also be mixed with Ma You (Sesame Oil) or hot Mao Gen (Imperatae Rhizoma) decoction. For rapid pain relief, add a small amount of She Xiang (Moschus) or Bing Pian (Borneolum Syntheticum) for better effect.
Qinzhou Shi stated: Ancient formulas for treating Ding Chuang (Furuncle/Carbuncle) all use Ye Ju Hua (Chrysanthemi Indici Flos). I treat similar toxin-sores using only Ye Ju Hua (Chrysanthemi Indici Flos), both internally and externally, with great efficacy. Contraindicated in Yin-Cold syndromes. Yi Mu Gao (Leonurus Ointment) effectively invigorates blood and disperses stasis.
