There are cases where the fire in the heart is deficient, failing to descend and communicate with the Kidney, leading to sudden collapse; this is Yang deficiency. The appropriate formula is Quansheng Tang (Complete Life Decoction):
Ren Shen (Ginseng Radix) [1 liang], Bai Zhu (Atractylodis Macrocephalae Rhizoma) [1 liang], Ban Xia (Pinelliae Tuber) [3 qian, raw], Fu Zi (Aconiti Lateralis Radix Praeparata) [3 qian], Shi Chang Pu (Acori Tatarinowii Rhizoma) [1 qian], Sheng Zao Ren (Ziziphi Spinosomi Semen) [1 liang], Fu Shen (Poria with Hostwood) [1 liang].
Decoct in water and take orally. Once the medicine touches the throat, the phlegm subsides and speech returns. After taking several doses continuously, one will return to normal health.
There are also cases where stomach heat prevents the Heart Fire from settling, leading to sudden collapse; this is also considered a form of Yang deficiency (specifically, false cold due to extreme heat). The formula is:
Ren Shen (Ginseng Radix) [1 liang], Yuan Shen (Scrophulariae Radix) [1 liang], Shi Gao (Gypsum Fibrosum) [5 qian], Hua Fen (Trichosanthis Radix) [5 qian], Mai Dong (Ophiopogonis Radix) [3 qian], Shi Chang Pu (Acori Tatarinowii Rhizoma) [1 qian]. Decoct in water and take orally.
After one dose, the heart settles; after two doses, the fire clears; after three doses, recovery occurs.
There are cases involving thirst with desire for drinking, red eyes, panting, a surging pulse at the Heart position, and inability to speak due to tongue stiffness; these should not be treated as Qi deficiency. This is actually extreme Kidney deficiency, unable to ascend and nourish the Heart Fire. The formula is:
Shu Di Huang (Rehmanniae Radix Praeparata) [1 liang], Mai Dong (Ophiopogonis Radix) [5 qian], Dang Gui (Angelicae Sinensis Radix) [1 liang], Sheng Di Huang (Rehmanniae Radix) [5 qian], Wu Wei Zi (Schisandrae Chinensis Fructus) [3 qian], Yuan Shen (Scrophulariae Radix) [1 liang], Shan Yu Rou (Corni Fructus) [1 liang]. Decoct in water and take orally.
After taking several doses, recovery occurs.
There are cases involving sudden anger and jumping, followed by suddenly falling to the ground, spasms in both arms, deviation of the mouth and eye, and tight closure of the left eye; this is a syndrome of Liver Fire and Blood deficiency, with internal heat generating Wind.
If there is spontaneous urination, and the pulse at the left Guan position is wiry, surging, and rapid, this indicates Liver Fire and Blood dryness. One should use Liuwei Wan (Six-Ingredient Pill with Rehmannia) base, adding Gou Teng (Uncariae Ramulus cum Uncis), Wu Wei Zi (Schisandrae Chinensis Fructus), Mai Dong (Ophiopogonis Radix), and Chuan Xiong (Ligustici Chuanxiong Rhizoma).
If a woman suffers from this syndrome, Xiaoyao San (Free and Easy Wanderer Powder) added with Gou Teng (Uncariae Ramulus cum Uncis) and Liuwei Wan (Six-Ingredient Pill with Rehmannia) is most effective.
In Cold Jue (Cold Extremities), the hands and feet must turn blue/cyan, drinking water causes vomiting, abdominal pain is present, and warmth from fire is preferred. In contrast, in Heat Jue (Heat Extremities), although the hands and feet are cold, they do not turn blue/purple; drinking water does not cause vomiting, and applying warmth causes abdominal pain. These must not be confused.
Heat Jue involves sudden extremity coldness (reversal) and unbearable pain. People often mistake it for Wind syndrome of the four limbs, not realizing it is caused by steaming heat within the Heart that cannot vent outward, thus leading to this condition. The formula is:
Chai Hu (Bupleuri Radix) [3 qian], Dang Gui (Angelicae Sinensis Radix) [2 qian], Jing Jie (Schizonepetae Herba) [1 qian], Huang Lian (Coptidis Rhizoma) [2 qian], Zhi Zi (Gardeniae Fructus) [2 qian, stir-fried], Ban Xia (Pinelliae Tuber) [1 qian], Zhi Ke (Aurantii Fructus Immaturus) [1 qian]. Decoct in water and take orally.
