Jue (Cold Extremities) refers to reversal or counterflow. It denotes the coldness of the hands and feet. This condition arises from a sudden decline of Yang Qi and an exclusive predominance of Yin Qi, which overcomes Yang. Consequently, the pulse becomes reversed and fails to reach the hands and feet, resulting in cold extremities. In Cold Damage (Shanghan), if Jue occurs from the first to the fourth or fifth day, fever will inevitably follow; if fever preceded the Jue, Jue will subsequently occur. Severe Jue corresponds with severe fever, while mild Jue corresponds with mild fever. If Jue has already been treated with purgation and is followed by sweating, it may result in oral injury with redness and ulceration. In Cold Damage, if one initially presents with Jue, fever, and diarrhea, the diarrhea will likely cease spontaneously. The subsequent appearance of sweating indicates that the disease is about to resolve.
For Cold Damage presenting with vomiting, diarrhea, cold extremities, and mental restlessness or fainting, Wuzhuyu Tang (Evodia Decoction) is indicated.
Ingredients: Evodiae Fructus (Wuzhuyu, 1 fen, soaked in boiling water seven times, dried, and lightly stir-fried), Ziziphus Jujuba (Jujube, 5 fruits), Glycyrrhiza Uralensis (Licorice Root, Gancao, 1 fen, honey-fried until slightly reddish and crushed), Zingiberis Rhizoma Recens (Fresh Ginger, Shengjiang, 0.5 liang), Ginseng Radix et Rhizoma (Ren Shen, 0.5 liang, radix removed), Magnoliae Officinalis Cortex (Cortex Magnoliae Officinalis, Houpo, 0.5 liang, outer bark removed, brushed with fresh ginger juice and fried until fragrant).
