[English Translation]
The ancients have passed away, but their words and virtuous deeds remain immortal. A scholar who cannot become a good minister should become a good physician. The medical literature is extremely extensive; this is how the Yellow Emperor (Xuanyuan) and Qibo utilized medicine to benefit the world. Baoyuanzi, whose name was Changshui Mingyi, possessed keen intelligence and excellent scholarship. He enjoyed writing and incorporated the profound mysteries of the Hetu and Luoshu diagrams, as well as the hidden implications of Fuxi and King Wen's I Ching, into his medical practice, exploring deep meanings and extending far-reaching insights. Medicine has always had renowned masters in each generation who expounded their theories, resulting in a vast number of publications. Baoyuanzi believed that since he had entrusted his theories to empty words, it would be better to record cases from clinical practice that were precise and clear. Thus, he collected strange symptoms and explained them using common principles, elaborating on astronomy, geography, human affairs, and various categories, naming the compilation "Hui" (Collection), emulating the medical ethics and skills of Lu Bian (Bian Que) and Han Dong (Han Kang and Dong Feng). Qian Tingshi Duntang from Danghu was his relative and wrote a preface twenty years ago to encourage its publication. However, he lacked confidence and consulted widely wherever he traveled, revising characters and sentences throughout the year. Inspiration sometimes came from conversations with friends or from nights spent alone under a lamp during wind and rain; these thoughts permeated his eating, drinking, dreaming, and sleeping. Now, physicians practicing in urban areas, examining others' diseases without having seen or heard of such cases, would rarely fail to frown in distress and act clumsily. Obtaining this book to verify conditions makes everything clear as seeing one's palm, which must be greatly satisfying! How can secrets kept in pillows compare to prescriptions carried at the elbow? Therefore, I request that this be carved onto pear and jujube wood blocks for publication, attaching it to immortal words.
Li Chi, nephew by marriage, bows respectfully. Early spring of the Bingwu year of the Qianlong reign.
In the *Suwen* (Basic Questions) chapter "Strange Diseases" attributed to the Yellow Emperor, it is stated: "There is a person suffering from headache that persists for several years without cessation. How did this occur, and what is the disease named?" (Headaches should not persist for years, hence the question arises from surprise.) Qibo replied: "This must result from exposure to severe cold, allowing the pathogen to penetrate inward to the bone marrow. Since the brain is the master of the bone marrow, the cold qi rebels upward, causing headache and toothache. This disease is named Jue Ni (Rebellious Counterflow)." The Emperor said: "Excellent."
Xia Ziyi, in *Qiji Fang* (Prescriptions for Strange Diseases), states: "A person suffers from sores on the crown of the head, displaying five colors and resembling cherries. Upon rupture, the skin splits from the crown and peels off down to the feet. This is named 'Rou Ren' (Flesh Person). Regular consumption of cow's milk leads to self-healing."
[Shen Yuan's Commentary] The onset of sores on the crown indicates involvement of the Foot-Taiyang Bladder Meridian, which runs from the vertex down to the feet. The *Suwen* states: "Wind pathogens enter together with the Taiyang channel, circulating through various meridians and scattering between the muscle intervals, interfering with Wei Qi. When the pathway becomes obstructed, muscles swell and develop ulcers. Muscles belong to the Yangming channel; thus, when Taiyang is diseased, it inevitably transmits to Yangming. In this case, the rupture of the sore crown and peeling of skin down to the feet indicate excessive Wind-Toxin in both meridians circulating through acupoints and scattering in muscle intervals, interfering with Wei Qi. This obstruction prevents Ying Blood from penetrating and moistening. Consequently, dry muscles split, allowing skin to peel down to the feet, not merely swelling and ulceration. Cow's milk moistens the Stomach and resolves toxins; once toxins dissipate and the Stomach is moistened, Qi and Blood resume circulation, leading to recovery."
*Qiji Fang* also states: "Some people grow bird-like formations on their heads or arms, covered by a layer of skin. These may resemble tumors or present as raised skin patches, emitting sounds like birds inside. They crow at dawn, chirp during rainy weather, and hurt when hungry or cold. All medicines fail. One must cut open the skin with a knife to expose the form, allowing it to emerge from the hole, resembling a bird but lacking feathers. After the bird emerges, apply Shengji San (Flesh-Regenerating Powder) externally, followed by Sheng Gao (Divine Ointment). The wound heals completely in three days."
[Shen Yuan's Commentary] Abnormal growths in the body, whether feathered, shelled, naked, or scaled, correspond to the Five Zang Organs. The *Suwen* states: "The East generates Wind, Wind generates Wood... corresponding to the Liver... its insects are hairy [Mao]. The South generates Heat, Heat generates Fire... corresponding to the Heart... its insects are feathered [Yu]. The Center generates Dampness, Dampness generates Earth... corresponding to the Spleen... its insects are naked [Luo]. The West generates Dryness, Dryness generates Metal... corresponding to the Lung... its insects are shelled [Jie]. The North generates Cold, Cold generates Water... corresponding to the Kidney... its insects are scaly [Lin]." The human body is a microcosm of heaven and earth. Heaven corresponds to the Five Directions and transforms accordingly; humans correspond to the Five Zang and generate accordingly. The previous symptom resembling a bird without feathers corresponds to the 'naked insect' imagery, associated with Central Damp-Earth pathology, originating in the Spleen.
Hua Tuo said: "A patient presented with a head and face swollen as large as a bucket, perceiving people as only three inches tall, loss of appetite, groaning while lying still. This is Phlegm. Administer Goudi San (Melon Peduncle Powder) to induce vomiting. The facial swelling subsided. After a second dose, vision normalized. Subsequently, administered Renshen Baizhu San (Ginseng and Atractylodes Macrocephala Decoction) for three doses, resulting in cure."
*Xiaoshan Guaizheng Fang* (Formulas for Strange Symptoms from Xiaoshan) states: "A person experiences scalp itching with maggots emerging. Apply crushed loofah leaves to the area. Once all maggots are expelled, the root cause is eliminated."
*Yangshi Yifang* (Medical Formulas by Mr. Yang) states: "A person hears sounds in the head like insects chewing, named 'Tian Baiyi' (Heavenly White Ants). This is caused by Liver Fire. Grind tea seeds into fine powder and insufflate into the nose for cure."
*Jishen Lu* (Record of Miracles) states: "Scholar Kuai Liang reported knowing of a case where a tumor on the temple was incised, revealing a black stone Go piece that could not be damaged by a heavy axe. Another case involved a tumor on the lower leg bitten by a relative's dog; over a hundred needles were extracted from the tumor, all usable. The disease healed."
[Shen Yuan's Commentary] Tumors containing stones or needles indicate pathology in the Lung. The Lung belongs to Metal in the West, representing firmness and rigidity, as described in the classics: "Its nature is rigid, its transformation is firm and contracting." Annotations state: "Due to rigidity, it corrodes iron. Firm contraction is the transformation of Metal."
A person suffered from water droplets forming on the scalp like sweat, flowing continuously. One jin (approx. 500g) of Licorice Root (Gancao) was decocted in water to make three to four bowls, taken in three to four doses. The flow stopped. This condition resulted from excessive medication intake since childhood.
[Shen Yuan's Commentary] This condition stems from excessive medication, likely due to youthful sexual excess combined with the use of mineral drugs (Shishi). Toxicity accumulated in the Kidneys and manifested later. Hair belongs to the Kidney. Drug toxicity forces Kidney Water Qi to rise outward, causing fluid exudation from hair follicles. Licorice effectively resolves mineral drug toxicity, justifying its sole use. Since the Kidney essence was not fully depleted, Kidney Qi could still rise; otherwise, a more severe toxic reaction would have occurred.
Wang Jiezhai treated a woman who initially complained of headache. After the headache ceased, heart pain began. After heart pain ceased, eye pain started, with no interval between episodes. Severe headache made her desire a heavy stone to press upon her head. Heart pain made her scratch walls until blood filled her palms. Eye pain made her dig at her eyes with both hands. This persisted for ten days. Physicians were helpless. Administering half a pill of Heilong Dan (Black Dragon Pill) reduced symptoms slightly. A second dose at midnight induced sleep. By dawn, she passed approximately three liters of stool resembling locust larvae. All three ailments halved. Another bowel movement at Si hour led to complete recovery.
Zhang Zihe treated a monk suffering from hot, painful head and photophobia. Ice was placed in cloth wrapped around the vertex, changed frequently. This indicated Yang heat accumulation. Zhang applied moxibustion in a warm room, induced sweating, and performed emesis simultaneously. The three methods worked together, curing him in seven days.
Tang Yu Zheng treated a young girl with Fengdan Disease (Wind-Dampness Disease). It started in the chest and spread upward, presenting with redness, swelling, pain, and itching. Previous treatments for upper burner wind-heat failed. Tang diagnosed extreme Liver-Lung Wind-Heat. He prescribed Qiang Huo, Jing Jie, Niu Bang Zi, Chi Shao Yao, Dan Zhu Ye, Jie Geng, and Ge Gen. The swelling receded downward but gathered at the vertex, rising several inches. Diet and sleep were unaffected, but the disease persisted. Tang's mother, Lady Wu, noted the wet nurse drank hot wine, including dregs. Tang realized Ge Gen (Kudzu Root) resolves alcohol toxicity and clears fire-toxin. He tripled the Ge Gen dosage in the formula. Swelling resolved in two days.
He also treated a youth bedridden with dizziness. Physicians treated it as deficiency, worsening the condition so the head could not move without fainting. This lasted years, with normal diet. Tang diagnosed Liver-Gallbladder Fire generating Phlegm. Movement triggered internal Phlegm-Fire agitation, causing syncope. Six Huang Tang (Six Yellow Decoction) for four doses cured him.
Zhu Zhongning treated a woman in her twenties with an opening in the brain, coughing up pus and blood synchronously with the opening. This was Fei Wei (Lung Withering). Treatment with Ren Shen, Mai Dong, and Dang Gui to clear heat and drain pus resulted in recovery. Another case involved a chest cavity draining pus and blood via the mouth, also treated as Lung Withering with similar success.
Zhai Wenbing treated Mother Lu, aged seventy, suffering from head ringing, tinnitus, neck pain, vertigo, facial numbness, swollen cheeks, loose teeth, dry lips, bitter taste, stiff tongue, swollen throat, shortness of breath, palpitations, fearfulness, chest fullness, phlegm stagnation, hypochondriac pain, lower back pain, weak legs, and knee pain for two years. She could not lie flat, requiring support to sit; slight reclining caused near-syncope. Zhai diagnosed Qi constrained by Liver Fire. Yi Qing Wan (Suppress Green Pill) induced immediate sleep. Upon waking, symptoms vanished. Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction) was then used for regulation and stability.
Zhu Danxi treated a man with a horn-like protrusion at the Left Sizhukong (SJ23) acupoint, resembling a chicken spur. This indicated Shaoyang Meridian excess blood and deficient Qi. Zhu advised abstaining from alcohol and meat and using acupuncture/moxibustion to resolve stagnation. Other physicians applied cold herbs like Rhubarb, Mirabilite, and Borneol. The lesion ruptured overnight like a clam shell, bleeding profusely for over a chi, leading to death. This resulted from external cold forcing heat inward, preventing its release.
A seventeen-eighteen-year-old son of the Hu family lost all hair, including eyebrows, with normal diet and lifestyle. Pulse was slightly wiry and choppy. This was caused by rich foods generating heat and damp-phlegm in the diaphragm. Excessive consumption of sour plums exacerbated damp-heat phlegm rising to the head, steaming and drying the blood at the hair roots, causing sudden total hair loss. Treatment involved blood-nourishing and ascending-dispersing herbs. Fangfeng Tongsheng San (Wind-Barrier Universal Rescue Powder) was modified by removing Mang Xiao (Mirabilite) and stir-frying Da Huang (Rhubarb) three times with wine. Combined with Siwu Tang (Four Substance Decoction) prepared with wine, taken for over two months. Once the pulse showed resolving damp-heat, medication stopped. Two years of light diet restored hair growth.
Xue Ji treated a twenty-year-old man with total scalp hair loss and rapid pulse. First, Tongsheng San dispersed wind-heat. Then, Liuwei Dihuang Wan (Six-Ingredient Pill with Rehmannia) was administered. Hair grew one cun in a few days and returned to normal in two months.
A scholar lost eyebrows and hair due to dietary irregularities, overwork, and anger. Xue attributed this to essence-blood depletion and defying Yin fire rising. Buzhong Yiqi Tang plus Maidong and Wuweizi, along with Liuwei Dihuang Wan plus Wuweizi, restored hair growth immediately.
A man contracted epidemic sores and took Huanji San (Skin-Replacing Powder), leading to sudden eyebrow loss, generalized pruritus, and erythema. This was dryness damaging Yin-blood and Yang hyperactivity. Morning: Siwu Tang with doubled Shudi, plus Fuling, Baizhu, Mudanpi, Zhizi, and Gancao. Evening: Liuwei Wan ingredients plus Danggui and Huangqi. Sores healed, and eyebrows regrew.
A fourteen-year-old boy engaged in sexual activity, developing fever and phlegm. Entering rooms caused photophobia and head distension. Despite continuing sexual activity, the head enlarged, and fontanelle opened. Dihuang Wan and Yiqi Tang were used alongside abstinence. Recovery took over a year.
Another fourteen-year-old boy presented with Jielu (delayed closure of fontanelle), feeling his head was large, objects appeared large, and he feared sunlight. His elder brother diagnosed weak Kidney endowment. Liuwei Wan plus Wuweizi and Lurong, combined with Buzhong Yiqi Tang plus Shanyao and Shanzhuyu, cured him in six months, closing the fontanelle in two years. Post-marriage, he felt the fontanelle reopen and soles burn. Due to his abstinence and light diet, taking two doses daily of the previous formula cured him in three years.
I treated the son of the Ye family from Shanleft, aged twenty-three, with eyebrow and hair loss. Pulse was deep and slow in both Chi positions. The etiology was Kidney Cold. He reported drinking cold water after diarrhea a month prior, followed by gradual hair loss. I cited the *Suwen*: "Hair brilliance resides in the Kidney." And *Caomu Zi*: "Qi nourishes eyebrows; Blood nourishes hair." Hair is the surplus of Blood; Blood is a category of Water. Within Water resides Ministerial Fire. Sexual intercourse causes this fire to descend. Adding cold drinks suppresses fire and congeals water, stagnating the Twelve Meridians. Thus, the Kidney fails to shine, and Qi fails to nourish. The Monthly Ordinances state: "In mid-autumn, Yin invades, Yang declines, water begins to dry." This is the death of water and earth, leading to plant decay. "In mid-winter, springs move, one Yang is born." This is the life of water and earth, leading to plant growth. For eyebrows and hair to regrow, Yang birth is required for Yin growth. Guizhi and Fuzi (pure Yang fire) added to Liuwei Dihuang Wan (pure Yin water) warms the Kidney, akin to one Yang returning to earth in winter, fostering all things. Following this advice, he recovered.
Question: Why does Qi nourish eyebrows (short) and Blood nourish hair (long)? Answer: This relates to Zang associations. Eyebrows relate to Qi, belonging to the Lung (Metal, Qian trigram). Hair relates to Blood, belonging to the Liver (Wood, Xun trigram). In I Ching logic, Metal is short, Wood is long; hence eyebrows are short, hair is long.
Question: Why do eyebrows/hair grow daily when cut, but appear static when retained? Answer: Cutting allows Qi and Blood to nourish the roots, promoting daily growth. Retention nourishes existing length, stabilizing growth.
Question: Why do plants sprout in mid-winter when one Yang is born, but wither in mid-autumn when Yin invades? Why do pines and cypresses not shed leaves? Answer: Not only pines/cypresses retain leaves; any plant leaves lacking juice survive winter. Leaves without juice rely on Qi for function; those with juice rely on fluids. Roots/stems with little juice also rely on Qi, allowing flowering in winter (e.g., plum, orchid). Qi resists cold. Elderly individuals resisting cold possess robust Kidney Qi, indicating longevity.
Neighbor surnamed Gu, due to youthful sexual excess, experienced scalp bleeding like sweat. This was Liver-Kidney Fire rebelling upward due to severe Blood Heat, exiting through hair follicles. Sweat is the fluid of Blood, transformed by Qi. *Neijing* describes Jinu (Muscle Ecchymosis) where scattered Qi fails to transform, causing blood to sweat from skin. Here, Qi transformation is insufficient, not absent. Ganlu Yin (Sweet Dew Decoction) and similar formulas cured him.
Zhang Jinggao's father, Yuchen, in middle age, repeatedly dreamed of a white man cutting his head until bleeding, waking abruptly. Even with eyes closed, visions persisted. Physicians were helpless. Consulted Shen Luzhen in Songjiang. Shen stated: Seeing white men in sleep indicates Lung Deficiency. Ancients used Dushen Tang (Single Ginseng Decoction), one liang of Ginseng per dose, curing in one dose. It proved effective.
