After childbirth, if there are no other symptoms, medication is not necessary. Within three days, decoct Jingjie Tan (Scrophulariae Radix Carbonized), Yimucao (Leonuri Herba), and Shatang (Sucrose) in water, and frequently administer the liquid to the puerpera to ensure the complete discharge of E Luo (lochia), thereby preventing Xue Yun (blood syncope) and Fu Teng (abdominal pain). Alcohol, as well as eggs, beef, mutton, and pork must strictly be avoided. The diet should consist of white rice porridge and dried vegetables, or steamed Songjiang Danxiang (light salted fish). Fresh meat may only be consumed after half a month, gradually increasing the quantity to avoid conditions of Yang excess and Yin deficiency. If blood loss has been excessive and lochia remains, or if there is dietary injury, wind-cold invasion, emotional distress, or onset of lactation on the third day, these can all cause fever with aversion to cold, body ache, and abdominal pain. Treatment must be tailored to the specific situation; one must not rigidly apply medications. Zhu Danxi stated: "For postpartum conditions, the primary approach is to greatly tonify Qi and Blood. Even if other miscellaneous symptoms exist, they should be treated secondarily." Postpartum patients should not undergo diaphoresis (sweating therapy), nor should Baishao (Paeoniae Radix Alba) be used early, as its sour and cold nature may damage the vital Qi. Caution is essential.
In general, after childbirth, Yin Blood is deficient, causing Yang to have no attachment and float/scatter outward, resulting in frequent fever. Treatment should utilize Siwu Tang (Four Substances Decoction) to tonify Yin Blood, combined with Paoganjiang (Prepared Dried Ginger) for its acrid-warm nature to treat the condition according to its apparent presentation, gathering the floating Yang and returning it to Yin. However, since the Spleen and Stomach are weak postpartum, fever caused by food stagnation from overeating is common; misdiagnosing this as Blood deficiency will yield no effect. When encountering postpartum fever, one must inquire about the cause of injury, check for food retention, and assess for chest/diaphragmatic fullness. If there is anorexia and diarrhea, treat solely as Food Injury. Only if fever occurs with normal appetite should blood-tonifying herbs be used. Postpartum Xue Yun (blood syncope), caused by deficient fire drawing blood upward, presents with gradual dizziness and fainting. Emergency treatment involves burning Lujiao (Deer Antler) to ash, removing fire toxicity, grinding into fine powder, and administering mixed with boiled wine and Tongbian (infant urine), which quickly awakens the patient by promoting blood circulation.
Another method uses an iron scale weight heated red-hot in charcoal, quenched in vinegar, allowing the puerpera to inhale the fumes to regain consciousness.
Zheng Liangdong stated: New-onset postpartum Xue Yun, characterized by unconsciousness resembling Zhong Feng (stroke), must not be immediately treated as stroke. Urgently administer Hupo Wan (Amber Pill) for recovery. If syncope occurs immediately after the infant's birth, rapidly lift and hold the mother upright, do not let her lie down, and quickly administer Tongbian (infant urine). If she does not awaken, throw red-hot charcoal into vinegar and allow the vinegar fumes to enter the nose until she revives. Applying treatment according to symptoms yields consistent results.
After delivery, while the infant is still on the bed, the mother must not lie down immediately; supporting her to sit for a moment is sufficient. Because the Qi and Blood of a new mother are unstable, there is a risk of blood rushing upward due to deficiency. Caution is required.
Drinking alcohol immediately postpartum is a major taboo, the first prohibition. Administering Tongbian (infant urine) is excellent. Other bitter-cold medicines, although descending, can congeal and stagnate defeated blood. Furthermore, fever occurring immediately postpartum must not be treated with cooling medicines, as they damage vital Qi and cause blood coagulation. Tongbian, formed from human Yuan Qi, is named Huanyuan Dan (Return to Origin Pill); it only descends fire, embodying the principle of using humans to supplement humans. Digestion is most difficult postpartum; foods like Red Bean, Sha Ji (Sand Chicken), and Duck Egg are hard to digest. Since massive bleeding occurs immediately postpartum, the Spleen and Stomach are very weak and prone to illness.
The pulse in the immediate postpartum period: if slow, slippery, knotted, solid, large, wiry, or tight, it suggests external pathogenic invasion; if heavy, deep, and small, life is preserved; if suddenly firm and secure without root, death ensues. A choppy, rapid, and irregular pulse at Cun Kou (radial artery) indicates death; a deep, fine pulse felt deeply against the bone indicates survival. Record these signs clearly and keep them in mind.
If the placenta (Bao Yi) fails to descend, accompanied by unrousable Xue Yun (syncope), stabbing abdominal pain, defeated blood attacking the Heart, closed eyes and locked jaws, delirious speech, or critical exhaustion, administer Hubei Heilong Dan (Amber Black Dragon Pill) for immediate effect, or Heishen Dan (Black Divine Pill) is also effective.
There is a distinction between defeated blood attacking the Stomach versus the Heart in postpartum women; treatments must not be confused. Attacking the Heart manifests as mania and chaos; attacking the Stomach manifests as fullness, nausea, and vomiting. Both are critical conditions requiring rapid medication; delay renders them incurable.
Wang Kentang stated: Postpartum defeated blood attacking the Stomach, Heart, or Lung are all critical conditions requiring urgent medication. Attacking the Stomach causes vomiting; attacking the Lung causes panting; attacking the Heart causes madness and jumping. Urgently have two people restrain the patient, and administer a decoction of Tongbian (infant urine), Jiangxiang (Aquilariae Lignum Resinatum), Chenxiang (Aucklandiae Radix), and Danggui (Angelicae Sinensis Radix). The evil blood will naturally descend, new blood will return to its vessels, and peace will be restored immediately.
Huangfu Zhong stated: For postpartum defeated blood stagnation affecting the Heart, causing Qi blockage, stiff tongue, and inability to speak, use Bazhen San (Eight Treasures Powder). For internal phlegm-Qi stagnation causing closed eyes and inability to speak, use Mingfan (Alumen) one qian ground to powder, taken with plain boiling water.
For postpartum excessive blood loss leading to fainting and unconsciousness, use Xionggui Tang (Ligusticum and Angelica Decoction). For postpartum deficient fire drawing blood upward, causing blood confusion and fainting, use Qinghun San (Clear Soul Powder). If the patient does not awaken, crush one to two jin of Chinese Chive, place in a pot, pour boiling vinegar inside, cover, and fumigate the nose with the spout until revival. Alternatively, burn Ganqi (Dried Lacquer) to smoke and inhale; if still unresponsive, urgently press Renzhong (GV26), pull the hair at the vertex, and administer Tongbian mixed with Ginger Juice for natural recovery.
Heilong Dan (Black Dragon Pill): Treats all postpartum blood disorders and retained placenta in critical conditions.
Danggui (Angelicae Sinensis Radix) (2 qian), Wulingzhi (Trogopteri Dung) (2 qian), Chuanxiong (Ligustici Chuanxiong Rhizoma) (2 qian), Shudi (Rehmanniae Radix Preparata) (2 qian), Liangjiang (Alpiniae Officinarum Rhizoma) (2 qian).
Take one liang of the above herbs. Place Chishizhi (Halloysitum Rubrum) in a clay box, wrap in paper, seal with salt mud, and calcine in charcoal fire until red-hot. Cool, open, and grind until black. Add the following herbs:
Baicaoshuang (Soot) (5 liang), Liuhuang (Sulfur) (1.5 qian), Ruxiang (Olibanum) (1.5 qian), Huaruishi (Flower Porphyry Stone) (1 qian), Hupo Xie (Amber Fragment) (1 liang).
Grind posterior herbs with anterior herbs to extreme fineness. Form pills with vinegar paste, size of a bullet. Take one pill per dose. Calcine the pill red-hot, dip in fresh Ginger Juice and Tongbian, filter through wine, dry, and grind finely. Swallow only with this wine.
Qinghun San (Clear Soul Powder): Treats blood confusion and syncope with coma.
Zelan (Lycopus Herba) (1 liang), Renshen (Ginseng Radix) (1 liang), Jingjie (Schizonepetae Spica) (4 liang), Zhi Gancao (Prepared Glycyrrhizae Radix et Rhizoma) (8 fen).
Take 2 qian per dose, mix with half a cup of hot water and half a cup of warm wine.
