Answer: After the mother has given birth, blood flows into the placenta (fetal membrane), causing it to become distended with blood, thus preventing its expulsion. If treatment is delayed even slightly, the distended placenta will rise sequentially from the abdomen towards the heart and chest, causing pain and panting. In acute cases, it is difficult to treat. One should simply administer Duoming Dan (Life-Stealing Pill) to rapidly remove the stagnant blood within the placenta. Once the blood dissipates and the distension resolves, the placenta will naturally descend without further complications.
Duoming Dan (Life-Stealing Pill)
Fuzi (Aconiti Lateralis Radix Praeparata, half tael)
Mudanpi (Moutan Cortex, one tael)
Ganxi (Resina Semecarpis Anacardii, crushed and stir-fried until smoke ceases, two qian five fen)
Grind the above ingredients into powder. Add one sheng of vinegar and one tael of Dahuang (Rhei Radix et Rhizoma) powder, and boil together into a paste. Mix with the medicine powder to form pills the size of paulownia seeds. Take fifty pills at a time with warm wine, regardless of meal times.
Niuxi Tang (Achyranthes Bidentata Decoction): Treats conditions where the fetus has been delivered but the placenta remains retained. Symptoms include hardness and fullness in the umbilical and abdominal regions, urgent distending pain, or situations where the fetus has died in the uterus and cannot be expelled. This formula is suitable for such cases.
Huashi (Talcum, eight liang)
Danggui (Angelicae Sinensis Radix)
Mutong (Akebiae Caulis, six liang each)
Dongkuizi (Malvae Semen, five liang)
Niuxi (Achyranthis Bidentatae Radix, remove sprouts, soak in wine, and bake)
Qumai (Dianthi Herba, four liang each)
Cut the above ingredients into small pieces. For each dose, take three qian, add two zhan of water, and decoct until eight fen remains. Strain the dregs and take while slightly warm, at any time.
Answer: There is a formed mass on the side of the fetus, separately called "Erzhen" (Child's Pillow). When labor begins, this mass ruptures, and the stagnant blood wraps around the fetus, making delivery difficult. Simply administer Shengjin San (Gold-Surpassing Powder) to drive out the stagnant blood, and the fetus will deliver itself spontaneously. This also treats malpresentation (breech or transverse lie).
Shengjin San (Gold-Surpassing Powder)
Shexiang (Moschus, one qian)
Yanchi (Fermented Soybean Paste, one tael; wrap in old green cloth, burn until red-hot, then immediately grind in a mortar into fine powder)
Grind the above into fine powder. Take a hammer weight, heat it until red-hot, quench it in wine, and use this liquid to mix one qian of the powder for oral administration.
Hupo Heisan (Amber Black Powder): Treats all diseases of postpartum women. It addresses antenatal fetal death, difficult labor, malpresentation, or transverse lie; postpartum retained placenta, premature rupture of membranes, generalized body pain, dry mouth, chest oppression, muteness at inappropriate times, symptoms resembling blood syncope or blurred vision (often misdiagnosed as latent wind/stroke), alternating chills and fever (often misdiagnosed as malaria), edema of the four limbs (often misdiagnosed as fluid retention/qi disorder), confused speech, hallucinations of ghosts and gods (often misdiagnosed as evil spirits), abdominal and hypochondriac fullness, irregular vomiting (often misdiagnosed as reverse reflux), constipation, hematuria (often misdiagnosed as the five stranguries), incomplete lochia discharge, and unreturned menstruation. If one does not observe dietary and lifestyle taboos, blood and qi disorders may arise; if they gather, they form masses that rush upward, causing shortness of breath, heart pain, cough, excessive sleepiness, deficiency-heat in the limbs, night sweats due to dream-disturbed sleep, metrorrhagia, and敗血 (decayed blood) symptoms; piercing pain around the navel or redness of the head and face may transform into bone-steaming consumption. All these conditions are suitable for frequent administration of this powder. However, if postpartum epistaxis occurs, with blackening of the nose and mouth and rising qi, wheezing in the throat, or stroke with jaw clenching, these are difficult-to-treat cases requiring urgent administration. Generally, taking one or two doses before delivery can calm the spirit and facilitate fetal positioning. Even if no disease exists after delivery, taking one or two doses within seven days can disperse various ailments. If other syndromes arise due to fright or shock, continuous administration should be taken to achieve efficacy.
Hupo (Amber, ground separately)
Zhusha (Cinnabar, ground separately)
Songmo (Pine Soot Ink)
Baicaoshuang (Soot from Hundred Herbs Burnt)
Xinluo Baifuzi (Baeckea Fructus from Silla)
Baijiangcan (Bombyx Batryticatus, stir-fried to remove silk threads)
Danggui (Angelicae Sinensis Radix, remove radicle)
Heiyi (Household soot/dust from kitchen hearth, half tael each; alternatively described as chimney soot)
Shexiang (Moschus, ground separately)
Xueyutan (Calculus Hominis / Urine Stone Charcoal)
Liyulin (Cyprini Carassii Squama / Carp Scale, two qian five fen each)
Grind the above into fine powder. For each dose, take two qian, mix with stir-fried ginger and warm wine, and administer combined with children's urine. Take before meals.
