When a pregnant woman reaches the ninth month of pregnancy and experiences aphonia (loss of voice), medication is not necessary. Her voice will recover naturally upon full term. This is because the Foot-Shaoyin Kidney Meridian passes through the kidney and connects to the root of the tongue. The tension in the uterine ligaments causes distortion, leading to an inability to speak.
For this condition, Liu Jun Zi Tang (Six Gentlemen Decoction) serves as the principal formula. If the cause is rice stagnation, add Gu Ya (Sprouted Rice); if caused by wheat flour stagnation, add Mai Ya (Sprouted Wheat); if caused by meat stagnation, add Shan Zha (Hawthorn Fruit). If accompanied by alternating chills and fever with vomiting, indicating Liver Wood insulting Spleen Earth, add Chai Hu (Bupleurum Root) and Sheng Jiang (Fresh Ginger). If accompanied by vomiting, abdominal pain, and cold extremities, indicating Cold insulting Earth, add Gan Jiang (Dried Ginger) and Rou Gui (Cinnamon Bark). If there is no response, administer Qian Yi Huang San (Qian's Golden Yellow Powder) next. If the stool is yellow watery diarrhea, representing the true color of Spleen Earth, add Mu Xiang (Aucklandia Root), Sheng Jiang (Fresh Ginger), and Rou Gui (Cinnamon Bark). If diarrhea occurs in the early morning hours (Wu Geng), with poor appetite, it indicates Spleen-Kidney deficiency; administer Si Shen Wan (Four Spirits Pill) at dawn and Bai Zhu San (Atractylodes Powder) during the day. If there is no response, or if the illness recurs after recovery with poor appetite, use Ba Wei Wan (Eight-Ingredient Pill) to tonify Ming Men Fire (Gate of Vitality) to generate Spleen Earth. In cases of Yuan Qi (Original Qi) sinking, presenting with fever, thirst, and limb fatigue, use Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction).
Dysentery with yellow watery discharge reflects the color of Earth, indicating Spleen Qi sinking; treatment should focus on raising and tonifying Middle Qi. If mixed with green and yellow, it indicates Wood overcoming Earth, requiring soothing the Liver and tonifying the Spleen. If yellow is mixed with white, it indicates the Son draining the Mother, necessitating tonification of the Spleen and Stomach. If yellow is mixed with black, it indicates Water insulting Earth, requiring warming and tonification of the Spleen and Stomach. If yellow is mixed with red, one should tonify the Mother and benefit the Son, preferably using Bu Zhong Yi Qi Tang. For weak intestines and stomach invaded by Wind-Cold, Wei Feng Tang (Stomach Wind Decoction) is indicated. If fetal instability occurs, urgently tonify the Stomach to calm the fetus. All fetuses-calming herbs should be applied according to specific clinical circumstances, not limited to formulas like Jiao Ai Tang (Collagen and Moxa Decoction).
Dang Gui (Angelica Sinensis Root), Bai Shao (White Peony Root) (each 2 qian), Huang Qin (Scutellaria Root) (8 fen), Bai Zhu (Atractylodes Macrocephala Rhizome) (3 qian), Chuan Xiong (Ligusticum Chuanxiong Rhizome) (3 qian), Ze Xie (Alisma Rhizome) (1.5 qian). Decoct in water and take orally. Do not use Hua Shi (Talcum). If the condition is urgent and Hua Shi must be used, this rule does not apply.
Pregnant women suffering from malaria often injure the fetus, so early treatment is advisable. If caused by food stagnation, use Liu Jun Zi Tang (Six Gentlemen Decoction) with added Jie Geng (Platycodon Root), Cang Zhu (Atractylodes Lancea Rhizome), and Huo Xiang (Patchouli Herb). If external pathogens are predominant and food stagnation is minor, use Huo Xiang Zheng Qi San (Patchouli Powder for Rectifying the Qi). If external pathogens are minor and food stagnation is significant, use Ren Shen Yang Wei Tang (Ginseng Decoction for Nourishing the Stomach). If caused by overwork damaging Original Qi, use Bu Zhong Yi Qi Tang (Tonify the Middle and Augment the Qi Decoction). If caused by emotional depression and anger, use Xiao Chai Hu Tang (Minor Bupleurum Decoction) combined with Gui Pi Tang (Restore the Spleen Decoction). If Liver Wood excess insults Earth and persists without cure, use Liu Jun Zi Tang (Six Gentlemen Decoction) assisted by fetuses-calming herbs, while also considering the Six Channels (Taiyang, Yangming, Shaoyang, Taiyin, Shaoyin, Jueyin) for differentiation and treatment.
