In cases where postpartum hemorrhage and excessive fatigue are severe, Qi lacks support, leading to respiratory arrest or cessation that deviates from normal physiological standards. Some erroneously attribute this condition to Phlegm-Fire (Tan Huo) and mistakenly administer formulas for dispersing Qi and resolving Phlegm, resulting in numerous fatalities due to misdiagnosis. The Lung receives nourishment from the Spleen, regulates Qi generation and pulse formation, unblocks water passages, facilitates respiration, clears and descends turbidity in the upper and lower Jiao, and harmonizes Ying (Nutritive) and Wei (Defensive) Qi; this constitutes the normal state of a healthy individual. However, following childbirth, when Blood and Qi are severely depleted, symptoms such as shortness of breath, inability to speak continuously, and a condition resembling asthma may appear as critical signs. The primary treatment should focus on massively tonifying Qi and Blood. If abdominal masses with pain are present, Bai Zhu (Atractylodis Rhizoma) should not be added. If there are no masses or pain, Shenghua Tang (Life-Restoring Decoction) can be used, supplemented with Shu Di Huang (Prepared Rehmanniae Radix, 2 qian), Fu Zi (Prepared Aconiti Lateralis Radix, 1 slice), while removing Tao Ren (Persicae Semen). If cold feet are present, add Shu Fu Zi (Prepared Aconiti Lateralis Radix, 1 qian), Huang Qi (Astragali Radix), Bai Zhu (Atractylodis Rhizoma, each 1.5 qian), and Chen Pi (Citri Reticulatae Pericarpium, 0.4 qian).
Shortness of breath occurring immediately after delivery accompanied by mass-related pain:
Chuan Xiong (Ligustici Chuanxiong Rhizoma, 2 qian), Dang Gui (Angelicae Sinensis Radix, 4 qian), Zhi Gan Cao (Prepared Glycyrrhizae Radix et Rhizoma, 0.5 fen), Jiang Tan (Charred Zingiberis Rhizoma, 0.4 fen), Tao Ren (Persicae Semen, 10 grains), Ren Shen (Ginseng Radix, 1 qian), Da Zao (Jujubae Fructus, 2 pieces). After taking two or three doses, adjust the prescription according to changes in symptoms.
Shortness of breath and rapid breathing after childbirth without mass-related pain:
Huang Qi (Astragali Radix), Bai Zhu (Atractylodis Rhizoma), Chuan Xiong (Ligustici Chuanxiong Rhizoma, each 1 qian), Dang Gui (Angelicae Sinensis Radix, 4 qian), Ren Shen (Ginseng Radix, 3 qian), Chen Pi (Citri Reticulatae Pericarpium), Zhi Gan Cao (Prepared Glycyrrhizae Radix et Rhizoma), Jiang Tan (Charred Zingiberis Rhizoma, each 0.4 fen), Shu Di Huang (Prepared Rehmanniae Radix, 2 qian). If cold feet are present, add Shu Fu Zi (Prepared Aconiti Lateralis Radix, 0.5 fen).
Postpartum delirium and hallucinations arise from dual deficiency of Qi and Blood, causing the Hun (Ethereal Soul) and Shen (Spirit) to lack foundation. The Heart stores the Shen and governs Blood; speech is the voice of the Heart. When the Heart has sufficient Blood, the Shen remains intact, and speech does not become erratic. The Liver stores Blood, and the eyes are its external orifice. When the eyes receive adequate Blood, vision functions correctly, and the pupils maintain proper visual alignment. If Blood and Qi are suddenly exhausted after childbirth, the Heart Shen loses its residence, leading incoherent speech; if the Liver Hun lacks foundation, visual disturbances occur. Since the Heart is the master of the entire body and the eyes reflect the state of all meridians, deficiencies manifesting in the Heart and Eyes indicate dysfunction of the Twelve Organs. Consequently, vision, hearing, speech, and movement may all exhibit false or erratic behaviors. The specific treatment method must first determine the presence or absence of abdominal masses and pain, and assess the urgency of the syndrome. If masses and pain persist, administer Shenghua Tang (Life-Restoring Decoction) consecutively for two or three doses to resolve masses and alleviate pain, followed by Jia Shen Shenghua Tang (Shenghua Tang with Ginseng) or Buzhong Yiqi Tang (Tonify the Middle and Augment the Qi Decoction), combined with herbs that calm the Spirit and stabilize the will. If delivery occurred long ago and both physical form and Qi-Blood are deficient, massive tonification is required to generate and nourish Qi and Blood, calming the Spirit and stabilizing the will. Continue medication until therapeutic potency is sufficient, at which point symptoms will resolve. Families should not seek rapid effects, and physicians should not attribute symptoms to evil influences. Sprinkling talisman water or frightening patients with ritual rulers often leads to untreatable conditions. In multiple treatments of this syndrome, efficacy was observed only after more than ten doses. Zhu Danxi stated, "When disease causes severe deficiency, symptoms may resemble possession by evil spirits." He also said, "To drain evil, one must first tonify deficiency, regulate Qi, and then address other diseases." This reflects the ancient approach to treating postpartum deficiency syndromes, elderly deficiency-induced asthma, and delirium in weak individuals.
