Wu Mengzhai stated: In cases of cold damage (Shanghan) during pregnancy, the treatment principles for the six meridians are the same as in non-pregnant individuals, but the primary objective must be to calm the fetus. Any medication that has a detrimental effect on the fetus should not be used. For instance, in formulas such as Huoxiang Zhengqi San (Agastache Powder to Regulate the Qi and Transform Dampness), Shiwei Xiong Su San (Ten-Ingredient Ligusticum and Perilla Powder), Shen Su Yin (Ginseng and Perilla Drink), and Xiao Chaihu Tang (Minor Bupleurum Decoction), Banxia (Pinelliae Rhizoma) is present; since Banxia can disturb the fetus, it must be removed if these formulas are employed. Medications such as Guizhi (Cinnamomi Ramulus), Fuzi (Aconiti Lateralis Radix Praeparata), Xiaoshi (Natrii Sulfas), and Dahuang (Rhei Radix et Rhizoma) all have the potential to agitate the fetus and require careful consideration when used. Generally, when treating cold damage during pregnancy with decoctions, Huangqin (Scutellariae Radix) and Baizhu (Atractylodis Macrocephalae Rhizoma) must be added, as they possess the ability to calm the fetus; alternatively, a decoction of these two herbs may be administered alone. If the pregnant woman has a constitutionally weak physique, Siwu Tang (Four Substances Decoction) should be incorporated into the prescription and must not be omitted. When employing Xiao Chaihu Tang (Minor Bupleurum Decoction), Banxia (Pinelliae Rhizoma) should be removed, Baizhu (Atractylodis Macrocephalae Rhizoma) added, and combined with Siwu Tang (Four Substances Decoction). This approach can preserve the fetus and clear heat, yielding miraculous results. I have applied this method with excellent efficacy.
Shen Zhai commented: Baizhu (Atractylodis Macrocephalae Rhizoma) and Huangqin (Scutellariae Radix) are the supreme medicines for calming the fetus, a brilliant theory proposed by Zhu Danxi. Danxi believed that during pregnancy, heat must be cleared, hence the use of Huangqin (Scutellariae Radix); and that the spleen must be strengthened to dry dampness, hence the use of Baizhu (Atractylodis Macrocephalae Rhizoma). However, if cold damage occurs during pregnancy and involves the three yang meridians, presenting with symptoms such as headache, aversion to cold, and fever, can Baizhu (Atractylodis Macrocephalae Rhizoma) be used immediately? To claim that it calms the fetus and clears heat in this context is akin to "locking the door to chase away the thief" (a metaphor for mismanagement that traps the pathogen inside).
Yile Yuanxu stated: For diseases occurring before childbirth, the treatment should focus solely on calming the fetus and regulating qi. If external contraction of the four qi (wind, cold, summerheat, dampness) or internal injury from the seven emotions leads to other diseases, the treatment methods are no different from those for men and should be sought within the respective syndrome categories. However, medications that disturb the fetus in pre-partum conditions must be scrutinized carefully. Siwu Tang (Four Substances Decoction) and Jiajian Liuhe Tang (Modified Six Harmonies Decoction) should be used.
Shen Zhai commented: The above three passages outline the presentation of cold damage syndromes during pregnancy. The transmission and transformation of cold damage through the six meridians are identical in women and men. The only difference is that for pregnant women suffering from this condition, when employing sweating, purging, or warming therapies, the preservation of the fetus must be the priority. All medications that harm the fetus must be used with caution; one cannot blindly administer toxic drugs under the pretext of "when there is an excess pathogen, there is no destruction [of the healthy qi]," thereby injuring the fetus or causing miscarriage.
Chanbao Baiwen stated: Malaria (Nue) is a disease characterized by alternating chills and fever, caused by wind and cold. Wind is a yang pathogen that transforms into heat; cold is a yin pathogen that transforms into cold. Since yin and yang are opposing, the pathogenic factor and the righteous qi alternate in their struggle. If yin merges with yang, yin becomes excess and yang becomes deficient. If yang merges with yin, yang becomes excess and yin becomes deficient. If injured by wind first and then by cold, fever precedes chills; if injured by cold first and then by wind, chills precede fever. The onset and cessation occur at regular intervals, hence the name malaria. The classics state: "Summerheat contracted in summer leads to malaria in autumn." This is due to the influence of summerheat. However, chills and fever during pregnancy are caused by deficiency and depletion of qi and blood, allowing wind and cold to invade. This results in the entanglement of yin and yang, with simultaneous manifestation of cold and heat. The classics state: "Deficient yang leads to aversion to cold; deficient yin leads to fever." These are all caused by deficiency, not by summerheat. If the chills and fever persist, they will scorch and injure the fetus. Ren Shen Qianghuo Tang (Ginseng and Notopterygii Decoction) is indicated for treatment.
